POST OFFICE BOX 1396 OXFORD, NORTH CAROLINA 27565 Dear Mr. Smith: NORTH CAROLINA Environmental Quality June 5, 2020 Subject: Permit No. WQ0035595 Granville Farms RLAP Attachment A and B Certifications Edgecombe County In accordance with your permit major modification request received December 9, 2019, and subsequent additional information received March 16, 2020, we are forwarding herewith modified Attachments A and B certified June 5, 2020, to Granville Farms, Inc. for the subject residuals management program. Modifications to the subject Attachments are as follows: 1. Addition of six new residuals sources totaling 75 dry tons/year (dt/yr), those sources being the Arbor Hills MHP (Permit No. NC0037869), Birchwood RV park (Permit No. NC0042803), Chapel Ridge WWTP (Permit No. WQ0022870), Cottonwood WWTP Permit No. NC0065706), Lake Royale WWTP (Permit No. NCO042510), and Lewiston Woodville WWTP (Permit No. NC0023116). 2. Increased tonnage for two residual sources: Hominy Creek WRF (Permit No. NC0023906) from 1,000 to 1,500 dry tons and Perdue WWTP (Permit No. NC0028835) from 600 to 800 dry tons per year, respectively. 3. Addition of four fields in Martin County EB-7, EB-8, EB-9, and EB-10 totaling 68.17 acres. The modified Attachments A and B shall be effective from the date of certification until September 30, 2021 (i.e., expiration date of the current permit), shall void Attachments A and B of Permit No. WQ0035595 issued October 10, 2016, that were previously certified on April 17, 2019, and shall be subject to the conditions and limitations as specified therein, as well as those specified in the above -referenced permit. Please replace Attachments A and B of Permit No. WQ0035595 that were certified on April 17, 2019 with the modified Attachments A and B. Please note that the Permittee shall submit a renewal application no later than April 3, 2021. Please note that only the maps for new land application sites are attached to this letter. It is the Permittee's responsibility to ensure that a complete permit is created by inserting these documents into the previously issued permit. Water Treatment Plant residuals are exempt from meeting the pathogen & vector attraction reduction requirements in Condition IVA, provided they are not mixed with residuals that have been generated during the treatment of domestic wastewater, the treatment of animal processing wastewater, or the biological treatment of industrial wastewater. The Water Treatment Plant residuals must be treated and processed in a manner as to not meet the definition of "Biological Residuals" as defined in 15A NCAC 02T .1102(6). The annual report shall include an explanation for missing sampling data. Those required to submit the annual report to United States Environmental Protection Agency (US EPA) may be required to make up the missed sampling. Contact the US EPA for additional information.
