received January 2, 2020. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on April 8, 2020.

rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A, or failure to provide the additional information on or before the above requested date may
result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper

A. Application:
Site RC-019 and Site RC-118 — Please provide the Landowner Agreement forms (LOA) for the
renewal of these sites. IF the LOA forms are not available, a modification could be requested in the
future to add them when the forms are available.
2. Site RC-019: Field #1 and Field 29 areas were increased by 5.19 acres and 2.46 acres respectively.
Please provide an explanation for these changes.
3. Site Rc-119: Field 89 was increased 3.14 acres. Please provide an explanation for these changes.
4. Site RC-004: Division of Water Resources received a letter dated January 15, 2020 requesting
immediate removal of this site from the permit. The letter was signed by the current property owner
i.e. Mr. Matthew Johnson. As the LOA forms submitted are dated prior to the date of this letter,
this site cannot be renewed unless updated and recent forms are submitted.
