Response: The property is located within Zone X, not in the 100-YR flood, by FEMA Firm Map No. 3720064900J Panel 0649 Last Revised May 2, 2006. Documentation is provided as required. B. Property Ownership Documentation (15A NCAC 02U. 0201(e)l: Provide copies of all easements (executed and filed in the Register of Deeds in the county in which the land is located) indicating the intended use of the property and meeting the requirements of 15A NCAC 02L .0107(f). If an easement has not established at this time, the permit can be issued as "construction only". The permittee would be required to submit the easement(s) in a permit modification to have the permit re -issued as "construction and operation". Response: The proposed reclaimed water line will be located in a right-of-way that will be dedicated during the close-out phase of the project. The designer will provide the plat showing this recorded right-of-way at that time. C. Engineering Plans: For clarity, include a table on each relevant plan and profile sheet with the required utility setbacks (15A NCAC 02U .0701) and minimum cover (15A NCAC 02T .0305(g)(4)). Setbacks to other utility lines are edge of pipe to edge of pipe. Response: A table has been added to the plan set showing the required setbacks, minimum cover and separation requirements as requested. A note has also been added to this table that these minimum requirements are from edge of pipe to edge of pipe. Refer to sheets C1.2, C1.3, C2.0 & C2.1. 2. Reclaimed Water Systems — Distribution Lines form RWDL 06-16: Part C. Engineering Plans, as per 15A NCAC 02U .0201(c)(1) or .0202(c), submit 11 X 17-inch plan sets that have been signed, sealed and dated by a North Carolina licensed Professional Engineer. At a minimum, the engineering plans shall include the following items: • Table of contents Response: refer to sheet C0.0 (cover sheet). • A general location map with at least two geographic references and a vicinity map. Response: Per the designer's communication with the reviewer, the provided existing conditions plan shows the reference as required. The cover sheet also Ms. Kendra D. Plan and profile views of all pump stations and reclaimed water line extensions as well as the proximity of the reclaimed water line extensions to other utilities and natural features. Response: The scope of this project is to install reclaimed water line extensions only. There are no pump stations proposed with this project. Refer to sheets C1.2, C1.3, C2.0 & C2.1 for plan and profiles of the proposed reclaimed water lines. • Depict minimum separation distances required by 15A NCAC 02U .0403(h), (i), and 15A NCAC 02T .0305). Response: Minimum separation distances are shown in the added table as requested. Dimensions have also been added to the plan set to provide verification of that the reclaimed water line has the required separations. • Minimum cover in accordance with 15A NCAC 02T .0305(g)(4). Response: The minimum cover has been added to the plan and profile of the reclaimed water line for verification. Refer to sheet C1.2. • Location and details of all pump stations. Response: The project scope is to only install reclaimed water line extensions. This project is to utilize the existing pump station as installed and permitted through the Town of Holly Springs. • All labeling and color -coding requirements per 15A NCAC 02U .0403. Response: A reclaimed water general note section has been added to sheet C0.1 to provided the required labeling and color -coding as required per 15a NCAC 02U .0403. Ms. Kendra D. Adherence to these instructions and checking the provided boxes will help produce a quicker review time and reduce the amount of additional information requested. Failure to submit all of the required items will lead to additional processing and review time for the permit application. General —This form is required for all projects involving the generation, utilization, and or distribution of reclaimed water. At least one of the following forms must also be included in the application package. However, multiple forms may be applicable. Unless otherwise noted, the Applicant shall submit one original and two copies of the application and supporting documentation. Please check the boxes below as appropriate to indicate which additional forms are included in the application package: ❑ Reclaimed Water Generation (FORM: RWG) — for wastewater treatment facilities producing reclaimed water. ❑ Non -Conjunctive Utilization (FORM: RWNC) — for reclaimed water utilization activities, when the reclaimed water utilization activity is required in order to meet the wastewater disposal needs of the facility. ❑ Conjunctive Utilization (FORM: RWCU) — for reclaimed water utilization activities, when the reclaimed water option is not necessary to meet the wastewater disposal needs of the facility and other wastewater utilization/disposal methods are available. ❑ Bulk Distribution (FORM: RWBD) — for distribution of reclaimed water in a bulk manner. ® Distribution Lines (FORM: RWDL) — for construction of reclaimed water distribution lines. ❑ Wetland Augmentation (FORM: RWWA) — for projects seeking to utilize reclaimed water for the purpose of wetland augmentation. ❑ Local Program Approval (FORM: RWLPA) — for projects seeking permitting delegation for reclaimed water users.
