Per 15A NCAC 02T.0105(c)(6), directly related environmental permits or certification applications are being prepared, have

Per 15A NCAC 02T.0402, "high -priority sewer" means "any aerial sewer, sewer contacting surface waters, siphon, or sewer
positioned parallel to streambanks that is subject to erosion that undermines or deteriorates the sewer.

portions of this submittal package, inclusion of these materials under my signature and seal signifies that I have reviewed this
material and have judged it to be consistent with the proposed design.
NOTE — In accordance with General Statutes 143-215.6A and 143-215.613, any person who knowingly makes any false
statement, representation, or certification in any application package shall be guilty of a Class 2 misdemeanor, which may
include a fine not to exceed $10,000, as well as civil penalties up to $25,000 per violation.

With regard to the referenced project, and on behalf of the Town of Jamestown, we have
enclosed the following for your review:
Original and one copy of the Fast Track Sewer Application
$480.00 Application Fee
8.5" X 11" color USGS topographic map and street level map
This permit is for the removal and replacement of an existing sewer line. The line is old
and in poor condition. The horizontal location of the line is not changing; however, the
depth is being lowered to allow for gravity service to the adjoining Magnolia on Main (aka
Magnolia Farms) development. Since there are not additional users for these line
segments, a FTSE has not been included in this application package. The private sewer
system for the adjoining development was previously permitted separately. Upon
completion of the sewer line installation and testing, the system will be turned over to the
Town for maintenance. The system will flow into gravity sewer lines owned by the Town
of Jamestown and a treatment facility owned by the City of High Point.
