Please find enclosed, the completed Fast Track Application and supporting documentation,
submitted for the above -mentioned project. One original and one copy of the following are included
for your review and approval.

Proiect Narrative
The enclosed documents are submitted for the relocation of 280 linear feet of 4" force main sewer
crossing NC 42 at Commerce Parkway. This force main is owned by Johnston County and is
associated with existing permit WQ0012285 (Southgate Business Park). This relocation is due to
impacts to the existing 4" force main by the roadway construction associated with the NCDOT
project I-5111, 1-40 Widening including improvements to NC-42 and SR 1010 (Cleveland Rd). This
permit modification is for relocating 280 linear feet of 4" force main This relocation will include
abandoning 222 linear feet of the existing 4" force main. This relocation will not result in any
additional flow.
The existing sewer permit WQ0012285 was issued May 15, 1996. As far as we are aware no
modifications to this permit have been done since. The original permit I believe included a pump
station, pumping 80gpm through 685 linear feet of 4" force main, and receiving flow through 2,170
linear feet of 8" gravity sewer line. The attached permit application is only for relocating 280 linear
of the 4" force main, everything else in the original permit is remaining the same. The plans have
been reviewed and approved by Mike Keen with Johnston County and NCDOT.
