Re: Church on the hill
There were concerns that the cM1�mh on the hill may be tied into the WWTP at the school. John Silver
with Mitchell Co Health Dept. contacted me today by phone to inform me that a dye test had been
conducted and the church does not discharge to the WWTP but to the creek.
He was approached about the church being used as office space and investigated accordingly. He
stated if the church is to be used for office space the adjacent land may be used for a conventional
system.

On February 24, 2014, The Division received your request to change from coverage under individual
NPDES permit NCO066729 to a Certificate of Coverage (CoC)under General Permit NCG550000. This CoC
is issued pursuant to the requirements of North Carolina General Statue 143-215.1 and the Memorandum of
Agreement between North Carolina and the US Environmental Protection agency dated October 15, 2007 [or
as subsequently amended].
Issuance of this CoC automatically rescinds NCO066729. Your annual fee is now $60.00
per year. If any parts, measurement frequencies or sampling requirements contained in this General
Permit are unacceptable to you,you have the right to request an individual permit by submitting an
individual permit application. Unless such demand is made, the certificate of coverage shall be final and
binding.
Please take notice that this Certificate of Coverage is not transferable except after notice to the
Division. The Division may require modification or revocation and reissuance of the certificate of coverage.
Contact the Asheville Regional Office Prior to any sale or transfer of the permitted facility.
Regional Office staff will assist You in documenting the transfer of this CoC.
This permit does not affect the legal requirements to obtain any other State, Federal, or Local
governmental permit that may be required. If you have any questions concerning the requirements of the

On February 17, 2014, I conducted a Compliance Evaluation Inspection of the Tipton Hill School Wastewater
Treatment Plant (WWTP). Both of you were present for the inspection and we discussed areas that needed
attention. I have received photo documentation indicating the areas in need of attention have been taken care
of,therefore the facility is considered in compliance with permit NC0066729.
This property will no longer be used as a school; it will be used by the Bradshaw Fire Department and will not
discharge more than 1,000 gallons of wastewater per day. A General Permit(NCG550000)has been applied for
and issued(NCG551618).Ay

Present during the inspection were Steve Adkins with the Bradshaw Fire Department and Richie Crowder
with Mitchell County Schools.
Bradshaw Volunteer Fire and Rescue, Inc. is now the owner of the school property. It is a small Fire
Department, so there will be limited flow to the wastewater system.(<1,000 gallons per day GPD). They may
host a couple of community events during the year, but still expect the flow to be <1,000 GPD.Therefore,
they have applied for coverage under the General Permit NCG550000,which has been issued
(NCG551618).
The facility has not been in use for a couple of years so during the inspection there were some maintenance
needs pointed out to Mr.Adkins and Mr. Crowder. The maintenance needs included vegetation growth in
the sand bed and chlorine chamber,a crack in the 2nd lateral, the tablet cylinders'broken bottom and the
effluent pipe was buried in sediment. I also pointed out an eroding streambank that should be repaired.
Photo documentation was received via email 3/11/2014 from Richie Crowder verifying these maintenance.

The inspector discussed the importance of operation and maintenance with the new owner;such as the
pumps and alarms(checking floats), keeping vegetation from growing in the sandbeds, and making sure
chemicals are used correctly so they don't cause a problem in the receiving stream. It was also suggested
the chemicals be stored inside. The effluent pipe is at a culvert and the junction of two streams.This is
naturally a depositional area for a stream,so checking on the effluent pipe will be an ongoing maintenance
need to keep it from being covered up. Finally there was some streambank erosion at the corner of the
security fence. If this erosion is not repaired it could get worse and compromise the fence.

Comment: This system has not been in operation for over a year, and now has anew
owner. Repairs to the 2nd lateral will have to be made;there is a crack midway up the
line. Also the vegetation that has grown in the filter will have to be removed. As
discussed,vegetation roots can cause a direct pathway for water through the sand. The
water needs to be well dispersed across the sand so it can flow through the largest area
possible for treatment.

Comment: The effluent pipe is at the junction of two streams and a culvert, which is an
area of a stream that will accumulate sediment buildup. We had to dig to find the
effluent pipe.The effluent pipe needs to remain exposed. Keeping sediment buildup off
of the effluent pipe will be an ongoing maintenance issue. Also there is some bank
erosion by the corner of the fencing around the chlorine/dechlorination units.This
should be repaired to avoid further erosion which could compromise the fence.
