Division staff has confirmed that the subject permit is no longer required, as groundwater remediation
is no longer underway at this site. Therefore, in accordance with your request, Certificate of
Coverage NCG510396 is rescinded, effective immediately.
If in the future you wish to again discharge remediation wastewater from this site to the
State's surface waters,you must first apply for and receive a renewed Certificate of
Coverage. Discharge of wastewater without a valid NPDES permit will subject the responsible party
to a civil penalty of up to $25,000 per day.

The facility listed above is covered under NPDES permit NCG510000 for discharge of remediated
groundwater. NCG510000 expires on September 30, 2006. Federal(40 CFR 122.41) and North Carolina(15A
NCAC 2H.0105(e))regulations require that permit renewal applications must be filed at least 180 days prior
to expiration of the current permit. If you have already mailed your renewal request,you may
disregard this notice.
To satisfy this requirement, the Division must receive your renewal request postmarked no later than
April 3, 2006. Failure to request renewal by this date may result in a civil assessment of at least$500.00.
Larger penalties may be assessed depending upon the delinquency of the request.
If any discharge from the remediation system will occur after September 30, 2006, your CoC must be
renewed. Discharge of wastewater without a valid permit would violate North Carolina General Statute 143-
215.1; unpermitted discharges of wastewater may be assessed civil penalties of up to $25,000 per day.
If you have halted remediation activity due to lack of trust fund money, but your site ranking from the
Aquifer Protection Section requires future remediation&discharge,your CoC must be renewed.

This is to inform you that,as of the date of this letter,the Division of Water Quality has not received a renewal
request for the subject permit. This is a violation of NCGS s 143.215.1 (c)(I) which states " All applications shall
be filed with the commission at least 180 days in advance of the date on which it is desired to commence the
discharge of wastes or the date on which an existing permit expires,as the case may be". Any permittee that has
not requested renewal at least 180 days prior to expiration,or any permittee that does not have a permit after the
expiration and has not requested renewal at least 180 days prior to expiration,will be subjected to enforcement
procedures as provided in NCGS 3143-215.6 and 33 USC 1251 at. seq.
In order to prevent continued,escalated action, including the assessment of civil penalties you must submit a
completed permit renewal application to the attention of the "Stormwater and General Permits Unit'at the
letterhead address within 10 days of your receipt of this letter(renewal application enclosed). If the subject
groundwater remediation project has been terminated,please complete the enclosed rescission request form.
Mailing instructions are listed on the bottom of the form. You will be notified when the rescission process has
been completed.

Your facility is currently covered for wastewater discharge under General Permit NCG510000. This permit expires
on September 30,2001. We are currently in the process of rewriting this permit and are scheduled to have the
permit reissued by the summer of 2001. Once the permit is reissued your facility would be eligible for continued
coverage under the reissued permit.
In order to assure your continued coverage under the general permit,you must apply to the Division of Water
Quality(DWQ)for renewal of your permit coverage. To make this renewal process easier,we are informing you in
advance that your permit will be expiring. Enclosed you will find a general permit coverage renewal application
form. This will serve as your application for renewal of your permit coverage. The application must be
completed and returned with the required information by February 28,2001 in order to assure continued coverage
under the general permit.
Failure to request renewal within this time period may result in a civil assessment of at least$250.00. Luger
penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your Facility
without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and
could result in assessments of civil penalties of up to$10,000 per day.
If the subject groundwater remediation project has been terminated,please complete the enclosed rescission
request form. Mailing instructions are listed on the bottom of the form. You will be notified when the rescission
process has been completed.

The wastewater system, which is known as, a single family sand filter appears to be operating properly,
there is no surfacing wastewater in the yard. The systen consists of a septic tank, sand filter and discharge
pipe. The discharge pipe could not be found by the inspectors.
We recommend;
-that you have your septic tank pumped every 3 to 5 years to protect your sand filter.
-that you not drive or park vehicles on your septic tank or sand filter,which may damage each.
-that you find, mark, keep clear of debry and protect your discharge pipe, this will prevent backup into your
j yard.

The pipes that stand in your yard may be shortened in order to mow your lawn, but be aware you should
cap these pipes and not run vehicles over them.
You asked about composting toilets, these are regulated/permitted by the County Health Department, to the
best of my knowledge.
You also asked about treating grey water, by way of a manmade wetland. If you are interested in a
non-discharging system please contact Ms. Bev Price at 828.296,4500.

Dear Permittee:
NPDES General Permit NCG510000, under which the subject facility is covered, expires on
September 30, 2006. North Carolina Administrative Code (15A NCAC 211.0105(e))requires that an
application for permit renewal be filed at least 180 days prior to expiration of the current permit. To satisfy
this requirement,your renewal package should have been sent to the Division postmarked no later than
April 3, 2006.
The Division has not received a renewal request for the subject CoC. This is a violation of your
permit at Part II.B. 10, which states"Any permittee that has not requested renewal at least 180 days prior
to expiration...will subject the permittee to enforcement procedures as provided in NCGS 143-215.6 and 33
USC 1251 et. seq.".
To prevent an assessment of civil penalties you must submit a completed permit application(see
