The review u(the above referenced erosion control plan has been completed and the plan submitted has been found
acceptable.
Amendments to the plan should be submitted to this office under the same procedure as followed for the original plan.
Title 15^wCAC4B .01 18(a) and the NCGO1 permit require that the following documentation be kept on file at the job site:
1. The approved E&SCplan aswell aoany approved deviation.
2.The woGO1permit and the COC.once itioreceived.
o. Records ofinspections made during the previous nodays.
This project is subject mthe National Pollutant Discharge Elimination System (NPDES). As of April 1.2019.all new construction activities
are required mcomplete and submit unelectronic Notice n{Intent (wm)from requesting a Certificate mrCoverage (:OC)under the
wC6o1VOooConstruction Somnvate,General Permit. This form MUST hesubmitted prior mthe commencement many land disturbing
activity onthe above named project. The w0|form may beaccessed atdeqoc.guwwCG01. After you submit acomplete and correct N0|
Form, a COC will be emailed to you within three business days. DEMLR will charge a fee for coverage under the NCGOI permit. This fee
iomuosent to the oswLR8mnn*otc,Central Office staff mRaleigh. Please see North Carolina Department ofEnvironmental Quality
(NCosQ) documentation regarding procedures for obtaining permit woGO100oO. Refer mthe wCosQwCGo1Ground Stabilization and

1. There is no substantial construction, erection, alteration, excavation, demolition or similar work authorized by such permits within one (1)
year after issuance.
o. Work has commenced, but there |unosubstantial progress onthe project for operiod ofone (1)year.
3. The project has not been completed within two (2) years of issuance of the permit.
