A Compliance Evaluation Inspection was performed at Davidson Water's Water Treatment Plants on March
3, 2015 by Jenifer Carter of the Winston-Salem Regional Office. Craig Koonts, ORC, were present for the
inspection. This type of inspection consists of two basic parts: a review of facility files and self -monitoring
data, and an on -site inspection.
Backwash from the filters now flows to a 500,000-gallon clearwell, and a new screw press has been built to
enable sending solids to the landfill.
The banks around the lagoons and at the discharge pipe have been stabilized with grass since the last
inspection.
The plant seemed to be operated effectively, with waste sludge being treated effectively and properly.

A. (2.) CHRONIC TOXICITY PASS/FAIL MONITORING (QUARTERLY)
The Permittee shall conduct quarterly chronic toxicity tests using test procedures outlined in the "North
Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised December 2010, or subsequent
versions.
The effluent concentration defined as treatment two in the procedure document is 90%. The testing shall be
performed as a Ceriodaphnia dubia 7-day pass/fail test. The tests will be performed during the months of
February, May, August and November. These months signify the first month of each three-month
toxicity -testing quarter assigned to the facility. Effluent sampling for this testing must be obtained during
representative effluent discharge and shall be performed at the NPDES permitted final effluent discharge
below all treatment processes.
All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge
Monitoring Form (MR-1) for the month in which it was performed, using the parameter code TGP3B.

Should any test data from this monitoring requirement or tests performed by the North Carolina Division of
Water Resources indicate potential impacts to the receiving stream, this permit may be re -opened and
modified to include alternate monitoring requirements or limits.
If the Permittee monitors any pollutant more frequently than required by this permit, the results of such
monitoring shall be included in the calculation & reporting of the data submitted on the DMR & all AT
Forms submitted.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control
organism survival, minimum control organism reproduction, and appropriate environmental controls, shall
constitute an invalid test and will require immediate follow-up testing to be completed no later than the last
day of the month following the month of the initial monitoring.

Proposed federal regulations require electronic submittal of all discharge monitoring reports
(DMRs) and specify that, if a state does not establish a system to receive such submittals, then
Permittees must submit DMRs electronically to the Environmental Protection Agency (EPA). The
Division anticipates that these regulations will be adopted and will begin implementing in 2013.

facility being physically`loca pd in an area where less than 10 percent of the households have
broadband access, then a temporary waiver from the NPDES electronic reporting requirements
may be granted and discharge monitoring data may be submitted on paper DMR forms (MR 1,
1. 1, 2, 3) or alternative forms approved by the Director. Duplicate signed copies shall be
submitted to the mailing address above.
Requests for temporary waivers from the NPDES electronic reporting requirements must be
submitted in writing to the Division for written approval at least sixty (60) days prior to the date
the facility would be required under this permit to begin using eDMR. Temporary waivers shall
be valid for twelve (12) months and shall thereupon expire. At such time, DMRs shall be
submitted electronically to the Division unless the Permittee re -applies for and is granted a new
temporary waiver by the Division.

Regardless of the submission method, the first DMR is due on the last day of the month
following the issuance of the permit or in the case of a new facility, on the last day of the month

These records or copies shall be maintained for a period of at least 3 years from the date of the report.
This period may be extended by request of the Director at any time [40 CFR 122.41].
A. (4.) PERMIT RE -OPENER - WHOLE EFFLUENT TOXICITY MONITORING
Whole Effluent Toxicity (WET) monitoring results indicate that this surface -water discharge may be toxic to
the aquatic environment. The Division of Water Resources therefore may reopen this permit, or request by
letter that the Permittee take further action. This action may include additional metals testing, a Toxicity
Identification Evaluation / Toxicity Reduction Evaluation (TIE/TRE), and additional assessment of
discharge alternatives and/or waste treatment alternatives.

design rate of 35.0 MGD while generating wastewater (filter -backwash). Discharge rates vary, reported over a
five-year period [n = 1156] average 0.375 MGD with a maximum at 1.144 MGD; minimum 0.001 MGD.

A Compliance Evaluation Inspection was performed at Davidson Water's Water Treatment Plants on March
5, 2014 by Jenifer Carter of the Winston-Salem Regional Office. Craig Koonts, ORC, were present for the
inspection. This type of inspection consists of two basic parts: a review of facility files and self -monitoring
data, and an on -site inspection of the lagoon.
There have been modifications since the last inspection, which were submitted with the permit renewal
application. Authorizations to Construct were obtained where necessary. Specifically, a new sludge
thickener receives wastewater from the pulsators and sediment basins. Solids from the thickener is
transferred to the lagoons and then currently land -applied by EMA Resources. Backwash from the filters
will eventually flow to a 500,000-gallon clearwell. A new screw press is being built to enable sending solids
to a landfill, when possible. This is projected to be completed within the year.
There was a substantial amount of erosion occurring around the lagoons, and at the discharge pipe. Grass
seed was sown at the outfall, and grass was starting to grow during the inspection. Please make sure that
erosion around the lagoons does not lead to a release of sludge to the environment.
The plant seemed to be operated effectively, with waste sludge being treated effectively and properly.

Comment: There have been modifications, which were submitted with the permit
renewal application. Specifically, a new sludge thickener receives wastewater from the

eventually flow to a 500,000 gallown clearwell. A new screw press is being built to
enable sending solids to a landfill, when possible.

Davidson Water, Inc, is requesting renewal of the NPDES permit NC0084425. There have been several
changes at the facility since issuance of the last permit. Your office approved in December 2010 a
modification to our permit moving the outfall location by approximately 600 feet. A new sludge
thigkew has been added to our process. Backwash water from our pulsators, sedbasins and filters will
now go to the sludge thickener and then decanted to our lagoons. Water will then be decanted from the
lagoons to the new outfall location approved in our modification December 2010.

7. Describe the treatment process(es) for raw water
Water is pumped from the Yadkin River into our three storage reservoirs going to our upper lake first
and then released to the far end of the southern reservoir through a 42" water line and then it is
released into our middle reservoir giving us the longest settling time possible. If needed for algae
control we add aluminum sulfate as it goes into the reservoirs. The water is pumped from our off
stream storage reservoirs to the plant where we add chlorine dioxide, sulfuric acid if needed for PH
control, and ferric sulfate for our coagulation process. Chlorine is added for disinfection and
iron/manganesecontrol. ontrol. After coagulation and sedimentation the water is filtered through our 26
mixed media filters. After filtration we add additional chlorine for disinfection , fluoride for prevention
of dental cavities, caustic or lime for PH stabilization and zinc orthophosphate for corrosion control.
Water goes into our 5 mg clearwell through a baffling system and then distributed to our other clearwell
storage and pumped into our distribution system.
8. Describe the wastewater and treatment process(es) for wastewater generated by the facility.
Wastewater from the pulsators and sedbasins go into a sludge thickener and wastewater is decanted
out of the thickener into the existing lagoons. Backwash water from the filters is going into the lagoons
at present but will shortly be going into 500,000 gallon clearwell taken out of service and modified to
handle filter backwash and become part of the treatment process. Water will be decanted out of
500,000 gallon containment into lagoons and sludge will be pumped out of containment into the sludge
thickener. The decant water from lagoons is allowed to return to the Yadkin River after it has been
dechlorinated using calcium thiosulfate. Flow to the Yadkin is measured and tested.
15. Solid Handling Plan
Solids generated in the 500,000 gallon containment from filter backwash will be pumped into sludge
thickener. Solids in sludge thickener is transferred to our lagoons and then are land applied by an
approved outside source. EMA Reseources, Inc. has handled land application for the past several years.

We have recently sent in plans for a screw press to DENR and have received all approvals. We hope to
have a screw press in operation within six months giving us the ability to take solids to the Davidson
County Landfill or any approved landfill as long as solids are 15% or greater. This will give us options in

At our water treatment facility located on Koontz Rd. near Yadkin College, we
cycle our filter wash water and our settling basin sediment to our two wash water
ponds where the sediment is removed and the decant water is returned to the river.
During the calendar year of Jan. 1, 2012 thru Dec. 31, 2012 we sent samples of
this decant water on a weekly basis to a certified laboratory for analysis. During this
time period there were no violations of any permit conditions, environmental
regulations or environmental laws.

This is to inform you that the Division of Water Quality did not receive your monthly monitoring report
for November 2011 until January 23, 2012. This is in violation of Part II, condition D(2) of the NPDES
permit, as well as 15A NCAC 2B .506(a), which requires the submittal of Discharge Monitoring Reports
no later than the last calendar day following the reporting period. Failure to submit reports as required
will subject the violator to the assessment of a civil penalty of up to $25,000 per violation.
Additionally, this letter provides notice that this office will recommend the assessment of civil penalties if
future reports are not received within the required time frame during the next twelve (12) reporting
months. The Division must take these steps because timely submittal of discharge monitoring reports is
essential to the efficient operation of our water quality programs. We appreciate your assistance in this

A Compliance Evaluation Inspection was performed at Davidson Water's Water Treatment Plants on
December 7, 2011 by Jenifer Carter of the Winston-Salem Regional Office. You, Mr. Ronald J.
Farnsworth and Mr. Tim Gwaltney were present for the inspection. This type of inspection consisted of a
review of facility files, a site inspection, and a review of field parameter equipment and records. The
following are the findings from the subject inspection:
A review of self -monitoring data since the last inspection did not reveal any limits violations. Lab data
correlated correctly with data on the submitted Discharge Monitoring Reports. Field parameter equipment
and records appeared to be operated and maintained properly.
The plant is currently going through major upgrades. Relating to this ongoing construction, it was
observed that silt -fencing along the Yadkin River was close to overtopping with sediment. If not already
done, please take all necessary steps to have them cleaned out in order to return them to working
condition.
The plant seemed to be operated effectively during the upgrade process, with waste sludge being treated
effectively and properly. Please review the enclosed inspection form for more detailed information. If

.The Division has reviewed the modification request submitted by your consultant
(Brian Sexton of Marziano & McGougan). The Division has no objection to your proposal.
The outfall relocation will not result in any change in discharge characteristics. The
minimal distance of the relocation [ca. 600 feet] does not change the basis for any current
permit limits or monitoring frequencies, which remain unchanged by this modification.
This modification is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the
U.S. Environmental Protection Agency dated October 15, 2007_ (or as subsequently
amended).
This modified permit includes the following changes from your current
permit:
➢ The permit map has been revised to show the new outfall location. Insert the
map into your permit after the relocation is completed..
.If any parts, measurement frequencies or sampling requirements contained in this
modification are unacceptable to you, you have the right to an adjudicatory hearing upon
written request within thirty (30) days following receipt of this letter. This request must
be in the form of a written petition, conforming to Chapter 150B of the North Carolina
General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service
Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision
shall be final and binding.

A Compliance Evaluation Inspection was performed at Davidson Water's Water Treatment Plants on
February 11, 2010 by Jenifer Carter of the Winston-Salem Regional Office. Mr. Ronald J. Farnsworth
and Mr. Tim Gwaltney, were present for the inspection. This type of inspection consists of two basic
parts: a review of facility files and self -monitoring data, and an on -site inspection of the facility. The
following are the findings from the subject inspection:
The permit was renewed as required and became effective on February 1, 2009 and expires on May 31,
2014. A review of self -monitoring data since the last inspection, for the period of December 2007
through December 2009, did not reveal any limits violations. Lab data correlates correctly with data on
the submitted Discharge Monitoring Reports.
Your staff worked with Tonja Springer in the Certification Unit to obtain a Field Parameter Certification,
as required. Your staff s quick action to correct this oversight was appreciated.
Overall the plant seemed to be operated effectively. Please review the enclosed inspection form for more

At our war:trseatment facilit cated on Koontz Rd. near Yadkin College, we
cycle our filth our settling basin sediment to our two wash water
ponds where the sediment is removed and the decant water is returned to the river.
During the calendar year of Jan. 1, 2009 thru Dec. 31, 2009 we sent samples of
this decant water on a weekly basis to a certified laboratory for analysis. During this
time period there were no violations of any permit conditions, environmental
regulations or environmental laws.
If you have any questions regarding this report or our treatment process please

As previously stated, the facility plans to construct a temporary
sediment basin. River water and sediment will be pumped from two
existing sediment ponds, into the proposed basin, where the
sediment will settle out. This needs to be done to allow proper
treatment of drinking water this facility provides. There should
be little, if any, water quality impact as a result of the
activities(as outlined in the application package). This office
recommends issuance of a permit to discharge.

A letter of request for an Authorization to Construct was received December 16, 1994 by the
Division and final plans and specifications for the subject project have been reviewed and found to be
satisfactory. Authorization is hereby granted for the construction of two 336,000 gallon sludge -settling
ponds each with bottom sludge drawoff and floating decanters a_polymer eed system, and two vacuum
sludge drain bg eds with discharge of treated wastewater into an unnamed tributary to the Yadkin River.
This Authorization to Construct is issued in accordance with Part III, Paragraph A of NPDES
Permit No. NCO084425 issued December 31, 1994, and shall be subject to revocation unless the
wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in
Permit No. NC0084425.
The sludge generated from these treatment facilities must be disposed of in accordance with G.S.
143-215.1 and in a manner approved by the North Carolina Division of Environmental Management.
In the event that the facilities fail to perform satisfactorily, including the creation of .nuisance
conditions, the Permittee-ghall take immediate corrective action, including those as may be required -by this
Division, such as the construction of additional or replacement wastewater treatment or disposal facilities.
The Winston-Salem Regional Office, telephone number [phone], shall be notified at least
forty-eight (48) hours in advance of operation of the installed facilities so that an in -place inspection can be
made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00
a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays.
Upon completion of construction and prior to operation of this permitted facility, a certification
must be received from a professional engineer certifying that the permitted facility has been installed in
accordance with the NPDES Permit, this Authorization to Construct and the approved plans and

Upon classification of the facility by the Certification Commission, the Permittee shall employ a
certified wastewater treatment plant operator to be in responsible charge (ORC) of the wastewater treatment
facilities. The operator must hold a certificate of the type and grade at least equivalent to or greater than the
classification assigned to the wastewater treatment facilities by the Certification Commission. The
Permittee must also employ a certified back-up operator of the appropriate type and grade to comply with
the conditions of Title 15A, Chapter 8A, .0202. The ORC of the facility must visit each Class I facility at
least weekly and each Class II, III, and IV facility at least daily, excluding weekends and holidays, and
must properly manage and document daily operation and maintenance of the facility and must comply with
all other conditions of Title 15A, Chapter 8A, .0202.
A copy of the approved plans and specifications shall be maintained on file by the Permittee for the
life of the facility.
Failure to abide by the requirements contained in this Authorization to Construct may subject the
Permittee to an enforcement action by the Division of Environmental Management in accordance with
North Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from complying
with any and all statutes, rules, regulations, or ordinances which may be imposed by other government
agencies (local, state, and federal) which have jurisdiction.
