NCG550000 [the General Permit for single family residence domestic wastewater]. An audit for expired unresolved
files noted that -the Certificate of Coverage (CoC) for your property's wastewater disposal system expired on
7/31/2013 and has overdue annual fees. The last CoC was issued to Ronald L Hopkins on 8/1/2012. The Division
needs information from you to renew coverage under NCG550000. Please respond by December 30, 2016 with one
the following:
➢ If your property still has a wastewater system like the ones described in the enclosed Technical Bulletin, you
must renew the subject CoC. Please complete the enclosed form and submit it to Charles Weaver at the
address on the form. In addition, a 180 in overdue annual fees is required to renew and maintain this permit.

Certificate of Coverage (CoC) NCG551057 to discharge under NCG550000. This CoC is issued pursuant to the
requirements of North Carolina General Statue 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection agency dated May 9, 1994 [or as subsequently amended].
If any parts, measurement frequencies or sampling requirements contained in this General Permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, the certificate of coverage shall be final and binding.
Please take notice that this Certificate of Coverage is not transferable except after notice to the
Division. The Division may require modification or revocation and reissuance of the certificate of coverage.
Contact the Asheville Regional Office prior to any sale or transfer of the permitted facility.
Regional Office staff will assist you in documenting the transfer of this CoC.
This permit does not affect the legal requirements to obtain other permits which may be required by
the Division of Water Quality or permits required by the Division of Land Resources, Coastal Area

Water Quality (DWQ) is forwarding herewith the reissued wastewater general permit Certificate of Coverage
(COC). This COC is reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between the state of North Carolina and the U.S. Environmental Protection Agency,
dated May 9, 1994 (or as subsquently amended). .

Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require

requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law rule, standard, ordinance, order, judgment, or
decree. '
Please note that effective January 1, 1999 the fees for all permits issued by DWQ were changed. This changed the
fee for your wastewater general permit coverage from a $240 fee paid once every five years to a yearly fee of $50.
If you have not already been billed this year for the yearly fee, you will receive a bill later this year.

Your residence or facility is currently covered for wastewater discharge under General Permit NCG550000. This
permit expires on July 31, 2002. Division of Water Quality (DWQ) staff is in the process of rewriting this permit
with a scheduled reissue in the summer of 2002. Once the permit is reissued, your residence or facility would be
eligible for continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the DWQ for renewal of
your permit coverage. To make this renewal process easier, we are inyou in advance that your permit will
be expiring. Enclosed you will find a general permit coverage renewal application form. This will serve as
your application for renewal of your permit coverage. The application must be completed and returned with the
required information by February 01, 2002 in order to assure continued coverage under the general permit. There is
no renewal fee associated with this process.
Failure to request renewal within this time period may result in a civil assessment of at least $250.00. Larger
penalties may be assessed depending on the delinquency of the request. Discharge of wastewater from your
residence or facility without coverage under a valid wastewater NPDES permit would constitute a violation of
NCGS 143-215.1 and could result in assessments of civil penalties of up to $10,000 per day.
If the subject wastewater discharge to waters of the state has been terminated, please complete the enclosed
rescission request form. Mailing instructions are listed on the bottom of the form. You will be notified when the
rescission process has been completed.

Dear Mr. Bradburn:
In accordance with your appl cation for an NPDES discharge permit received January 24,
2000 by the Division, we are herewith forwarding the subject Certificate of Coverage under the
state NPDES general permitfor Clarance Bradburn Property. Authorization is hereby granted
for the construction of a 125_GPD, sand filter systems consisting of a 1000 gallon septic tank, 108
square foot (3' X 36') primary saridfilter with a loading rate of not more than 1.15 GPD/square
foot, 54 square foot (3' x 18') secondary sand filter with a loading rate of not more than 2.3
GPD/square foot, chlorine disinfection unit, chlorine contact chamber, dechlorination unit, and
rip rap aeration with a discharge of treated wastewater into Hickory Creek classified C-Tr waters
in the Broad River Basin. This discharge must enter the receiving waters downstream of the
Bearwallow Baptist Church baptismal area as discussed with Mr. Roy Davis, Asheville Regional
Office. We recommend that the collection lines be vented and all elbow piping must be of the
long sweeping type. This Certificate of Coverage is issued pursuant to the requirements of North
Carolina and the U.S Environmental Protection Agency Memorandum of Agreement dated
December 6, 1983 and as subsequently amended.

In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may
be required by this Division, such as the construction of additional or replacement wastewater
treatment or disposal facilities.
The Asheville Regional Office, telephone number [phone], shall be notified at
least forty-eight (48) hours in advance of operation of the installed facilities so that an in -place
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Upon completion of construction and prior to operation of this permitted facility, a
certification must be received certifying that the permitted facility has been installed in
accordance with the NPDES Permit, the Certificate of Coverage, this Authorization to Construct
and the approved plans and specifications. Mail the Certification to the Stormwater and General
Permits Unit, 1617 Mail Service Center, Raleigh, NC 27699-1617.
A copy of the approved plans and specifications shall be maintained on file by the
Permittee for the life of the facility.
The sand media of the sandfilters must comply with the Division's sand specifications.
The engineer's certification will be evidence that this certification has been met.
A leakage test shall be performed on the septic tank and dosing tank to insure that any
exfiltration occurs at a rate which does not exceed twenty (20) gallons per twenty-four (24) hour
per 1,000 gallons of tank capacity. The engineer's certification will serve as proof of compliance
with this condition.
Failure to abide by the requirements contained in this Authorization to Construct may
subject the Permittee to an enforcement action by the Division of Water Quality in accordance
with North Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this permit does not preclude the Permittee from complying with any and
all statutes, rules, regulations, or ordinances which may be required by the Division of Water
Quality or permits required by the Division of Land Resources, the Coastal Area Management
Act or any Federal, Local or other governmental permit that may be required.

On the existing properly in Gerton there is no suitable land for any type or subsurface system without a
discharge into Hickory Nut Creek. Roy Davis from the regional office in Asheville, N.C. has reviewed this
location and recommended a discharge system into Hickory Nut Creek. There is no discharge system for Gerton
and the closest discharge would be 20 miles away (Hendersonville).
The usage is based on 5 employees (possibly a high figure) to cover the maximum anticipated number
of people. The church wanted the discharge by the bridge to avoid their baptisamal area just above. It will not be
possible for the 4" PVC pipe to be 3 feet deep in this area. since the pipe is gravity a 1/16" fall per foot with the
pipe initially at 3 feet of depth and discharging for a cascade area into the creek.

if you needed to build a pipe to carry treated wastewater
from a proposed sewage treatment system on your Gerton
property downstream to Hickory Creek.
Were you able to get gravity flow from the proposed
sewage treatment system you would need to build a 411
diameter schedule 40 PVC pipe to Hickory Creek.- Were you to
need to pump the treated wastewater, you would want to
install a 211 diameter schedule 40 PVC pipe to Hickory Creek.
In either case the pipe would be buried to a depth of
approximately 3 feet.

to receiving waters designated as Hickory Creek, Class C Tr, in the Broad River Basin in accordance with
the effluent limitations, monitoring requirements, and other conditions set forth in Parts I, II, III and IV
hereof.
This certificate of coverage shall become effective December 17, 2003.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

Dear Mr. Bradburn:
You asked that I inform you just what would be required
if you needed to build a pipe to carry treated wastewater
from a proposed sewage treatment system on your Gerton
property downstream to Hickory Creek.
Were you able to get gravity flow from the proposed
sewage treatment system you would need to build a 411
diameter schedule 40 PVC pipe to Hickory Creek. Were you to
need to pump the treated wastewater, you would want to
install a 211 diameter schedule 40 PVC pipe to Hickory Creek.
In either case the pipe would be buried to a depth of
approximately 3 feet.
