In accordance with your application for discharge,the Division is forwarding herewith the subject Certificate of Coverage to discharge under the subject state-NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General Statue 143-215 .1 and the Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated October 15, 2007 (or as subsequently amended). The following information is included with your permit package: • A copy of the Certificate of Coverage for yew treatment facility • A copy of General Wastewater Discharge Permit NCG550000 • A copy of a Technical Bulletin for General Wastewater Discharge Permit NCG550000 • A Professional Engineer's Certification to be completed and returned. If any parts,measurement frequencies or sampling requirements contained in this general permit are unacceptable to you, you have the right to request an individual permit by submitting an individual permit application. Unless such demand is made,the certificate of coverage shall be final and binding. Please take notice that this Certificate of Coverage is not transferable except after notice to the Division of Water Quality. The Division of Water Quality may require modification or revocation and reissuance of the certificate of coverage. This permit does not affect the legal requirements to obtain other permits which may be required by the Division of Water Quality or permits required by the Division of Land Resources,Coastal Area Management Act or any other Federal or Local governmental permit that may be required. Authorization to Construct Permit. In accordance with your application for discharge, the Division is also authorizing the construction of a 240 GPD wastewater treatment system consisting of a 1000 gallon septic tank with riser, one 6' by 35' primary sand filter, one 6' by 17.5' secondary sand filter, a tablet chlorinator with contact chamber, a tablet dechlorinator, and rip-rap outlet ditch liner for erosion control protection with a discharge of - treated wastewater into an unnamed tributary to Mill Timber Creek, Class C, Trout waters in the Catawba River Basin. All elbow piping must be of the long.sweeping type. All cleanouts are to be housed in meter boxes below the surface. This system must be at least 50 feet from the property lines and at least 100 feet from water supply wells on and off the site, and at least 100 feet from any habitable residence under separate ownership. The system must also be constructed and located above a 100 you flood. The installer should be requested to provide you with a 100 pound container of chlorination tablets and a 100 pound container of dechlorination tablets. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permitwe shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement wastewater treatment or disposal facilities. Failure to abide by the requirements contained in this Authorization to Construct may subject the Permittm to an enforcement action by the Division of Water Quality in accordance with North Carolina General Statute I43-215.6A to 143-215.6C. The Asheville Regional Office,telephone number(828)296-4500,shall be notified at least forty-eight(48)hours in advance of operation of the installed facilities so that an in-place inspection can be made. Such notification to the regional Surface Water Protection supervisor shall be made during the normal office hours from 8:00 a.m.until 5:00 p.m.on Monday through Friday,excluding State Holidays. Upon completion of construction and prior to operation of this permitted facility, a Professional Engineer's Certification must be received certifying that the permitted facility has been installed in accordance with the NPDES Permit, the Certificate of Coverage,this Authorization to Construct and the approved plans.and specifications. A leakage test shall be performed on the septic tank and dosing tank to insure that any exfilhation occurs at a rate which does not exceed twenty (20) gallons per twenty-four (24) how per 1,000 gallons of tank capacity. The Installer's Certification will serve as proof of compliance with this condition. Mail the completed Installer's Certification to the NPDES Unit,1617 Mail Service Center,Raleigh,NC 27699-1617. Box 596 Newland North Carolina 28657 Re: 728 'Fennant Road, Newland,NC Dcar Miss Tallent, On Thursday, August 31, 2008 a Licensed Soil Scientist from Foothills Soil Consulting, LLC (FSC)performed a brief inspection of the soils on the above referenced lot. Property lines were identified in the field by Mr. Tom Burleson. I'he property is bounded on the front by Tennant Road and in the back by Mill Timber Creek, which is atributary to the Linville River. A drainage ditch with perennial flows runs along one Side property line into the creek. Over the course of the investigation 6 post hole pits were the property,and five in the front. In all pits, lbserved observed—one in the back part of features with chroma two or less or other sign of soil wetness condition (SWC) were o within 12" of the surface. Free water was observed in several of the holes. These results were corroborated by Joe Lynn,the Regional Soil Specialist with the North Carolina DF.NR Onsite Water Protection section. Based on these results, no subsurface septic system of any kind can be permitted on this site. State rules require at least 15"of soil for even the most advanced types of engineered systems. With the likelihood that the water table is closer to the surface than 12" in parts of the property, even water reuse is not a good option on this site. I hope this information is helpful to you.
