continuing nature, not related to operation and/or maintenance problems, and you anticipate remedial
construction activities, then you may wish to consider applying for a Special Order by Consent (SOC).
If you have any of the frequency violation/parameter missing documentation to report, please make the necessary
revisions (write revisions on the front page) to the original Non -Discharge Monitoring Report (NDMR) and return
the form(s) by email to Non-Discharge.Reports(o�ncdenr.gov.
