FACILITY OVERVIEW:
This facility is 100% domestic waste. The design capacity of the treatment system is 0.010 MGD and consists of
the following wastewater treatment components:
• Influent bar screen
• Aeration basin
• Clarifier with sludge return and skimmer
• Wet well with effluent pumps
No modifications were noted or proposed in the current application. In 2015 a Collection System permit
(WQ0037567) was issued to West Caldwell Health Council to allow the Collettsville Medical Center to discharge
to the Collettsville Elementary School WWTP. This collection system permit was rescinded in July 2019. No
information on this process was in the application, but an email from Dr. Church (3/9/2020) verified that there
was no connection to this, or any other, school system WWTP, also verified that the medical facility never used
their facility in a second email (3/11/2020).
Most recent inspection (11/16/2017) found no significant issues and that the facility was well maintained and
operated.
COMPLIANCE:
There were no compliance actions during the permit cycle and there have been no enforcement actions since
2010 for this facility.

These facilities don't have flow meters. And I believe if you ask Jerry, he will tell you they pretty much just put the same
number every time and don't do calculation based on well records or a bucket and stop watch.

I believe you are correct about the rounding, their flow meters probably do not go low enough to achieve precision
below that. We could discuss having them change meters, but I don't know if there would be a lot of value in that since
they are staying well below permit limits.

Tim,
I have attached three draft permit renewals that will be going to notice later this week. For each I have
included the draft permit and cover letter (in one file) and the Fact Sheet in a separate file. I have copied Linda
also since I think you may have mentioned getting her comments on these at some point earlier. If they need
to go to someone else then please forward on or let me know who to get them to.
• NCO030783 - Baton Elementary - NH3-N Monitoring - See the write up in the fact sheet and cover letter
- the monitoring has changed to be consistent with Rule 2 .0404(c) requiring winter limits to be no less
stringent than 2x summer limits. This one is to WS waters so I will also email to PWS for their
comment.
• NCO041220 - Oak Hill Elementary - Corrected map from previous version; added TRC language in
effluent footnote to be consistent with current language.
• NCO050075 - Collettsville Elementary - No major changes.
One question Tim. In all of these permits the DMR data showed that for flow there were only a very few
distinct values that were reported. Most likely this is due to the small nature of the reported values as
MGD. It just seems a little odd for example that for Colletsville there are 248 flow values reported but they
are all either .001 or .002? Maybe there is really no issue with this? Looks like they are just maybe rounding
everything off.

during the permit cycle when the Medical Center was actually discharging into your system. Possibly they just received
the permit to do so, but never actually did. If you were accepting flow from them I'd like to be aware of the timeframe
so I can consider that as I review data for your Collettsville permit renewal.

facilities connected to any of the district's treatment systems.

I am currently working on drafting the permit renewals for three Caldwell County Schools. In reviewing records for one
of the schools, Collettsville Elementary, I noticed that during the permit cycle this system was potentially accepting
discharge from West Caldwell Medical Center. It appears that this discharge ended in July 2019. 1 just wanted to check
with you to make sure that there were no other outside facilities that may be connect to this school's treatment system
or any of your other school's treatment systems. The renewal applications don't indicate this but I wanted to make sure
so we can accurately reflect the components of your schools treatment systems.

• Facility has eDMR contacts with ESA.
• In 2015 a Collection System permit (WQ0037567) was issued to West Caldwell Health Council to
allow the Collettsville Medical Center to discharge to the Collettsville Elementary School WWTP.
This collection system permit was rescinded in July 2019.

Review of Monitoring Data:
Unlike some other Caldwell County School System permits, the data for this facility was very consistent
and there were very few waiver during periods of no flow. Flow appears to be continuous.
• BOD —Values averaged 3.24 mg/L with no noted exceedances (31.30 max).
• TSS —Values averaged 9.40 mg/L with no noted exceedances (42 max).
• Flow average for permit term — 0.0019 MGD. No values exceeded the 0.010 MGD permit design
flow rate. Average flow is at 19% of permitted flow. Only two separate values reported in the
248 total values (0.001, 0.002). May be due to small nature of the values and eDMR significant
digits?
• Temperature — only monitor at the effluent. Values seem in line with seasonal variation.
• Fecal Values— not required
• pH — no issue from data (6.1 to 8.1).
• NH3-N — average of 0.83 mg/L overall. Monitoring requirement only in summer months.
• No WET requirement
Compliance / Enforcement:
• No compliance violations.
• No enforcement actions since 2010.

The North Carolina Division of Water Resources conducted an inspection of the Collettsville
Elementary School on 11/16/2017. This inspection was conducted to verify that the facility is
operating in compliance with the conditions and limitations specified in NPDES WW Permit No.
NC005007S. The findings and comments noted during this inspection are provided in the
enclosed copy of the inspection report entitled "Compliance Inspection Report",
There were no significant issues or findings noted during the inspection and therefore, a
response to this inspection report is not required.

Tim Heim of the Asheville Regional Office performed a Compliance Evaluation Inspection on November
15, 2017. Jerry Younce (ORC) and Marc Dillinger (Shop Foreman) assisted with the inspection and
facility review. The facility appeared well maintained and operated, and in compliance with Permit

A rescission request was recently received for Permit No. WQ0037567, Issued January 29, 2015. Staff
from the Division of Water Resources has confirmed that this Permit is'no Ianger required. Therefore, in
accordance with your request, Sewer Extension Permit No. WQ0037567 is rescinded, effective
immediately.
If in the future, if you wish again to operate a non -discharge system, you must first apply for and receive
a new Wastewater Collection System Permit. Operating a non -discharge system without a valid
Wastewater Collection Permit will subject the responsible party to a civil penalty of up to $10,000 per
day.

In accordance with your application received January 23, Z015, we are forwarding herewith Permit No.
WQ0037567 dated Issued January 29, 2015, to the West Caldwell Health Council, Inc. (Permittee) for
the construction and operation upon certification of the subject wastewater collection system
extension. This permit shall be effective from the date of issuance until rescinded, and shall be subject
to the conditions and limitations as specified therein. This cover letter shall be considered a part of this
permit and is therefore incorporated therein by reference.
Please pay particular attention to the following conditions contained within this permit:
Condition 11.1: This permit. shall not be automatically transferable; a request must be made and
approved.-.
Condition IIA: Requires that the wastewater collection facilities be properly operated and maintained
in accordance with 15A NCAC 2T .0403 or any individual system -wide collection system
permit issued to the Permittee.
It shall be responsibility of the Permittee to ensure that the as -constructed project meets the
appropriate design criteria and rules. Failure to comply may result in penalties in accordance with North
Carolina General Statute §143-215.6A through §143-215.6C, construction of additional or replacement
wastewater collection facilities, and/or referral of the North Carolina -licensed Professional Engineer to
the licensing board.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right
to request an adjudicatory hearing upon written request within 30 days following receipt of this permit.
This request must be in the form of a written petition, conforming to Chapter 150$ of North Carolina
General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail Service Center, Raleigh,
NC 27699-6714. Unless such demands are made, this permit shall be final and binding.

WASTEWATER COLLECTION SYSTEM EXTENSION PERMIT
In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina -as
amended, and other applicable laws, Rules, and Regulations, permission is hereby granted to the -
West Caldwell Health Council, Inc. (Town of Collettsville)
Caldwell County
for the construction and operation of approximately 40 linear feet of 6-inch gravity sewer; a 2,000—
gallon per minute pump station with duplex pumps, on -site audible and visual high water alarms,
telemetry, and a portable generator receptacle with telemetry; as well as approximately 1,S00 linear
feet of 2-inch force main to serve a 10 employee medical center with 7 exam rooms as part of the
Collettsville Medical .Center project, and the discharge of 810 gallons per day of collected domestic
wastewater into the Collettsville. Elementary's existing sewerage system,. pursuant to the application
received January 23, 2015, and in conformity with 15A NCAC 2T; the Division's Gravity Sewer Minimum
Design Criteria adopted February 12, 1996 as applicable; the Division's Minimum Design Criteria for the
Fast -Track Permitting of Pump Stations and Forte Mains adopted June 1, 2000 as applicable; and other
supporting data subsequently filed and approved by the Department of Environment and Natural
Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until rescinded and shall be subject to the
specified conditions and limitations contained therein.
