Attached is our Rescission Request form for our plant and mine site in Sanford, NC. Permit
Number NCG070089. This site has been closed for a number of years, but the mine permit was
just terminated last year. I've included the mine permit release letter for reference.
To aid in the review of this rescission request, also attached is a 2018 aerial photo of the site
from Google Earth (Lat: 35.5, Long:-79.7937). It shows that the plant has been tom down and
that there is no industrial activity at the site. We have subsequently sold this location*.
Our Annual Stormwater Permit Fee (6/l/20 to 5/31/21) is due on June 1. During these reduced
economic times we are concerned about cash flow. Therefore, with the pending rescission of this
permit, we request release from the annual fee at this time.

Facility closed or is closing on 2/11/19 . All industrial activities have ceased such that no discharges of
stormwater are contaminated by exposure to industrial activities or materials.

may be more appropriate to request an ownership change to reissue to permit to the new owner.
❑✓ Other: This site was a brick manufacturing plant and shale mine. The mine was reclaimed and the mining permit released
on February 11, 2019. The permit release is attached. The plant was tom down and removed.

The mine was found to be satisfactorily reclaimed. Therefore, your company is

1971.
