submittal package, inclusion of these materials under my signature and seal signifies that l have reviewed this material and
have judged it to be consistent with the proposed design.

On behalf of the City of Durham Department of Water Management, we submit herewith the "Fast -
Track" Application (FTA 01-16) and the Variance Alternative Design Request (VADC 10-14) for the
above referenced project for your review and approval.
Please find attached the following documents associated with the City of Durham's 54-Inch New
Hope Creek Parallel Interceptor project for your review:
• One (1) original and one (1) copy of fully executed FTA 04-16 form
• One (1) original and one (1) copy of fully executed VADC 10-14 form (not sure it's required)
• Two (2) sets of construction drawings
• Two (2) USGS site maps
• Two (2) street level maps (See Drawings)
• Check for application fee of $480
Due to recent multiple significant breaks in the existing 36-Inch Eno River Force Main as a result of
internal and external corrosion, this project is classified as a "high -priority" project by the City of
Durham, and accordingly falls under the City's Emergency Action Plan for Critical Sewer Main
Crossings. We ask that this project be permitted in the most expeditious manner practical, so
construction of the project can be started as soon as possible before another break occurs and
potentially a very large volume of wastewater is discharged onto the adjacent ground.
