Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:* The site is currently being developed as a residential subdivision. The owners are building infrastructure. Land use in the area is a combination of residential, commercial and timber production. 4b. Explain the purpose of the proposed project:* The purpose of the project is to construct infrastructure to allowfor development of this site, and to conduct remediation work on areas improperly impacted during road construction 41. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:* Direct impacts originally permitted for this project totaled 149 linear feet. During the construction process, additional unpermitted impacts were constructed, leading to a Notice of Violation against the site. The unpermitted actions include a temporary impact of 20 linear feet of a Ur to Stratford Creek, located downstream of the arch culvert. This impact was put in place by the contractor to access both sides of the arch culvert for installation. The updated plan calls for the removal and restoration of this channel, as well as the restoration of the temporary roadway. A box culvert was installed at the site, however, there are issues Wth the sizing. The box culvert is slated to be temporarily removed, so that a LOMAR/CLOMAR study can be conducted, and a new crossing designed and installed. An additional unauthorized impact involved a previously isolated wetland, and a small nearby stream channel. The wetland was not found during the original delineation effort, as it had no connection to the nearby stream channels, and was under brush piles at a logging deck during the delineation effort. The contractor installed a small culvert at the roadway, and connected this wetland area to a nearby stream channel. The contract excavated the stream head, and put a culvert into the excavated channel, un-isolating the wetland area. This action included the excavation of approximately 30 feet of land, opening up new stream channel. Large equipment, including excavators was used in the construction process. 4j. Comments: Stream channel 4 impact is the result of an excavation at a stream head. The result is the creation of an additional 30 feet of stream channel. We propose to stabilize this channel in place, which includes removing the culvert within the now -open channel, sloping and vegetating the banks. Additionally, the contractor installed a culvert in a UT to Stratford Creek to allow temporary access across the channel to install the arch culvert. At this time, the contractor also installed a culvert connecting an isolated wetland area to a nearby stream channel.
