One more follow up to my guidance earlier this week. My coworker reminded me that we have the following form on
our website:

The form is normally used to report a new exceedance and/or to submit with the monthly monitoring required under
Tier 2, but there's a question asking if the facility had 4 or more exceedances from a single outfall, which triggers Tier
3. You could also include this form with any communication to the Raleigh Regional Office.

Hi, Tim,
We don't have a standard form or memo for Tier 3 Notification. A written letter or even email would be sufficient (I
think the permit just says notify in writing). Just include all the necessary details about the parameter(s) results and
circumstances. The Raleigh Regional Contacts are Bill Denton (Regional Supervisor), and Thad Valentine (primary
Stormwater contact), both copied on this message.

I am with the City of Raleigh Yard Waste Center. Based on a review of historical data of our stormwater discharge, it
looks like the YWC has moved to a Tier 3 facility. We are preparing to contact the Raleigh Regional Office to inform them
of this change. However, I would like to know if there is a standard form or memo format that we need to use for this
communication. Can you assist me with this or point me in the right direction?
