Basin to be overflowing at the Town of Sparta WWTP. On investigation Mr. Shaw found the PLC Control panel to the SBR was completely shut down. All other components of the system were found to be operating properly. After diagnosing / checking the system, Bob disconnected the power at the main breaker and upon reconnecting the power, all systems started functioning properly including the SBR. At approx. 7:43 am all systems were back in service. First Bob called Gary Hudson and got no answer, and then George Smith was notified of the bypass and made aware of the situation. Bob Shaw estimates that approximately 200,000-250,000 gallons of sewage was spilled into the strom drainage system that discharges into the Little River. Bob went and made a visual inspection of the discharge point and found no evidence of waste or any debris from the spill. Also he went downstream on Choate Dairy Road Bridge and found no evidence of debris or material from the spill. It was noticed that minnows were present and swimming at the bridge. No environmental impacts or fish kills were observed. The first corrective measure taken was to clean up debris around the EQ Basin. Bob has also been in touch with Aqua Aerobic Systems personnel in which Bob was informed on how to have our electrician to wire in a backup alarm for a power failure within the SBR system. This is the first time this problem has ever occurred. Electrician will be onsite to install the necessary alarm light to prevent this from happening again. Box 99 Sparta, NC 28675 SUBJECT: Compliance Evaluation Inspection (CEI) Town of Sparta WWTP NPDES Permit # NCO026913 Alleghany County Dear Mr. Edwards On October 2, 2013 George Smith performed a Compliance Evaluation Inspection. Mr. Bobby Shaw, ORC, was present for the inspection. The inspection is comprised of an overall evaluation in order to document compliance and noncompliance of: facilities equipment, capability to meet effluent limits, and terms & conditions with the NPDES permit. Permit The permit became effective June 1, 2013 and expired March 31, 2016. A correct copy is kept on file. The Town has no issues with the new permit. Records & Reports The operators log documents the following: rainfall, plant operations, operation & maintenance records, and troubleshooting. The daily worksheet documents process control of the aeration basin, such as: MLSS, Dissolved Oxygen, Temperature, pH, 30-minute Settleable Solids, and Clarifier Blanket Height. These daily logs also document general comments concerning the condition of the plant and record the generator test every Wednesday. The chain -of -custody reports are complete, and they clearly document the sample conditions. The 2012 Annual Performance Report from July 1, 2012 through June 30, 2013 was sent to Raleigh by Mr. Kevin Dowell. The aeration basin had a light brown color and a good earthy odor indicating good treatment. There was a good gentle rolling aeration action with less than 5% light brown foam. The effluent from the clarifier was clear, with a small amount of pin flock over the weirs. No algae present. The skimmer was operating. The flow splitter sends 50% to sequential batch reactor (SBR) and 50% to the conventional treatment plant both are function properly. The lower part of the contact chamber was clear with no foam or floating solids. The chlorine and sulfur dioxide gases are maintained and there are 5 full cylinders in storage. Flow Measurement The flow recording charts are kept on file for the required minimum of three years. Continuous flow measurement at the plant is performed using a 90' V-notch weir with an ultrasonic device mounted before the effluent weir. The flow measuring device was calibrated by Carolina Technical Services on September 4, 2013 Paperwork documentation was maintained in the file. The instantaneous flow was 0.123 MGD at 12:15 pm. Laboratory The municipal lab received field certification (#5233) for Residual Chlorine, pH, and Temperature. The Sension, Hach 156 pH meter is routinely calibrated and documented using a two -point calibration (pH 4 and pH 10), and a midrange check at pH 7. Temperature is recorded with a Sension Hach 156, which is NIST traceable. The Total Residual Chlorine is performed with a Hach DR2500 and the annual 5-point calibration was performed by Pace Analytical on July 25, 2013. The lab measures and documents the field data in an excellent manner. Effluent/Receiving Waters The effluent from the POTW was very slightly cloudy with no solids, no color, or no odor. The visual appearance of the effluent is considered excellent. CEI October 3, 2013 Page 2 of 3 The unnamed tributary to Little River, Class C waters showed no adverse impacts. Operations & Maintenance The following Town personnel operate the facility: Bobby Shaw ORC, Grade III; Eddie Hall, Grade II; Andy Blevins, Maintenance. Please insure that Andy Blevins obtains a Grade II if you intend for him to act as a Back-up ORC. The MLSS is approximately 2599 mg/L and the settleability is 17%, which is low because Mr. Shaw just wasted some solids into the digester. Self -Monitoring Program A review of the monthly self -monitoring reports from October 2011 through July 2013 revealed no limit violations. The Town currently performs influent composite sampling with an Isco sampler. A sample aliquot of 50 ml is collected every 15 minutes. The Town currently performs effluent composite sampling with an Isco sampler. A 100 mL sample is collected every 2,400 gallons. This complies with the effluent flow proportion sample requirement. Sludge Disposal The digester is currently maintained at approximately 50% capacity to waste solids. The compliance evaluation inspection is satisfactory. The quality of the effluent is considered to be excellent at the time of the inspection, and the plant personnel are considered to perform their jobs in an excellent manner. Currently, Eddie Hall, Grade H is listed as a back-up operator. Please insure that Andy Blevins obtains a Grade II if you intend for him to be a Back-up ORC. The clarifier is maintained with an 8 -foot blanket. The MLSS is approximately 4900 mg/L. Self -Monitoring Program A review of the monthly self -monitoring reports from October 2011 through July 2012 revealed no limit violations. The Town currently performs influent composite sampling with an Isco sampler. A sample aliquot of 50 ml is collected every 15 minutes. The Town currently performs effluent composite sampling with an Isco sampler. A sample is collected every 2,400 gallons. This complies with the flow proportion sample requirement. Sludge Disposal The digesters are maintained at approximately 60% capacity to waste solids. The compliance evaluation inspection is satisfactory. The quality of the effluent is considered to be excellent at the time of the inspection, and the plant personnel are considered to perform their jobs in an excellent manner. CHRONIC TOXICITY PERMIT LIMIT (QRTRLY) The effluent discharge shall at no time exhibit observable inhibition of reproduction or significant mortality to Ceriodaphnia dubia at an effluent concentration of 9.0%. The permit holder shall perform at a minimum, quarterly monitoring using test procedures outlined in the "North Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or subsequent versions or "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or subsequent versions. The tests will be performed during the months of March, June, September and December. Effluent sampling for this testing shall be performed at the NPDES permitted final effluent discharge below all treatment processes. If the test procedure performed as the first test of any single quarter results in a failure or ChV below the permit limit, then multiple -concentration testing shall be performed at a minimum, in each of the two following months as described in "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or subsequent versions. The chronic value for multiple concentration tests will be determined using the geometric mean of the highest concentration having no detectable impairment of reproduction or survival and the lowest concentration that does have a detectable impairment of reproduction or survival. Mr. Bobby Shaw, ORC, and Mr. Andy Blevins, Back-up ORC were present for the inspection. The inspection is comprised of an overall evaluation in order to document compliance and noncompliance of: facilities equipment, capability to meet effluent limits, and terms & conditions with the NPDES permit. Permit The permit became effective March 1, 2007 and expired March 31, 2011. A correct copy is kept on file. A permit renewal application was submitted. The POTW is rated as a Grade III facility because it has a permitted flow of greater than 0.5 through 2.5 MGD. The sampling for a Grade III is 3 times per week. Records & Reports The operators log documents the following: weather, plant operations, operation & maintenance records, and troubleshooting. The daily worksheet documents process control of the aeration basin, such as: MLSS, Dissolved Oxygen, Temperature, pH, 30-minute Settleable Solids, and Clarifier Blanket Height. These daily logs also document general comments concerning the condition of the plant and record the generator test every Wednesday. The chain -of -custody reports are complete, and they clearly document the sample conditions. The Annual Performance Report from July 1, 2010 through June 30, 2011 was sent to Raleigh and received in this office. The aeration basin had a brown color and a good earthy odor indicating good treatment. There was a good gentle rolling action and the dissolved oxygen runs between 3 — 4 mg/L. The effluent from the clarifier was clear, with a small amount of pin flock over the weirs. No algae present. The skimmer was operating. The flow splitter, equalization tank and sequential batch reactor (SBR) are function properly. The upper section of the chlorine contact chamber had some foam and floating solids. The lower part of the contact chamber was clear with no foam or floating solids. Flow Measurement The flow recording charts are kept on file for the required minimum of three years. Continuous flow measurement at the plant is performed using a 90' V-notch weir with an ultrasonic device mounted before the effluent weir. The flow measuring device was calibrated by Carolina Technical Services on September 28, 2011 Paperwork documentation was maintained in the file. Laboratory The municipal lab received field certification (#5233) for Residual Chlorine, pH, and Temperature. The Sension, Hach 156 pH meter is routinely calibrated and documented using a two -point calibration (pH 4 and pH 10), and a midrange check at pH 7. Temperature is recorded with a Sension Hach 156, which is NIST traceable. The lab measures and documents the field data in an excellent manner. Effluent/Receiving The effluent from the POTW was very slightly cloudy with no solids, no color, or no odor. The visual appearance of the effluent is considered very good. The unnamed tributary to Little Buffalo Creek, Class C-Trout + waters showed no adverse impacts. VCE October 17, 2011 Page 3 of 3 Operations & Maintenance The following Town personnel operate the facility: Bobby Shaw ORC, Grade III; Andy Blevins, Back-up ORC. Currently, Tim Church, Grade IV and Eddie Hall, Grade II are listed as back-up operators. It appear that Tim Church is the designated Back-up ORC. Please insure that Andy Blevins obtains a Grade II if you intend for him to be a Back-up ORC. The clarifier is maintained with a 7 -foot blanket. Self -Monitoring Prog am A review of the monthly self -monitoring reports from October 2010 through July 2011 revealed no monitoring or limit violations. The Town currently performs influent composite sampling with an Isco sampler. A sample aliquot of 50 ml is collected every 15 minutes. The Town currently performs effluent composite sampling with an Isco sampler. A sample aliquot of 50 ml is collected every 2,400 gallons. This complies with the flow proportion sample requirement. Sludge Disposal The digesters are maintained at approximately 30% capacity to waste solids. This is considered an excellent operations strategy, especially through the winter months. The inspection is satisfactory. The quality of the effluent is considered to be very good and the plant personnel are considered to perform their jobs in an excellent manner. Sometime during the week ending on the 30th of JUly we experienced an electrical storm here in Town. During this event, we had an electrical surcharge that ran in on our Flow Meter and also took out the "electronic board" of the Regal Smart Valve of our Chlorine fed system. Technicians were here on August 8,2011 and have started the process of getting us back online. We are told that it will be approximately 2 weeks to get everything back up and going. Until that time, please be advised that there will be no flow readings and that chlorine and sulfur dioxide will have to be manually fed until system is repaired. Box 99 Sparta, NC 28675 SUBJECT: Compliance Evaluation Inspection, Town of Sparta WWTP NPDES Permit No. NCO026913 Alleghany County Dear Mr. Edwards: On February 16, 2011, Sue White and Gary Hudson of this office met, with Bobby Shaw, ORC, to perform a Compliance Evaluation Inspection on Sparta's wastewater treatment system. This type of inspection consists of two basic parts: an in office file review and an on -site inspection of the treatment facility. The attached EPA inspection form notes the eight (8) areas that were evaluated for this inspection. The findings and observations are outlined below: I. Permit The NPDES permit was reissued with an effective date of March 1, 2007. This permit is due to expire on March 31, 2011. The town's application for renewal should have been mailed in 180 days prior to the expiration date or September 30, 2010. II. Self -Monitoring Program The monthly self -monitoring reports (DMR's) were reviewed for the period November 2009 through October 2010. The facility was found to be in complete compliance with all final NPDES effluent limits. In addition, all monitoring was performed and recorded per the frequencies specified in the permit. III. Flow Measurement The permit requires continuous flow recording and is measured by an ISCO 4250 meter. Carolina Technical Services last calibrated the meter on June 29, 2010. IV. Sludge Handlina and Disposal Solids are land applied by Synagro under non -discharge permit # W00006242. V. Operations and Maintenance No problems or concerns were noted with the operation and maintenance of the facility. VI. Facility Site Review The 0.6 MGD facility consists of the following: manual bar screen, influent pump station, flow splitter with 50% of the influent routed to an equalization basin followed by a SBR with floating decanter and 50% routed to a circular aeration basin and secondary clarifier. Flow recombines prior to gas chlorination, contact chamber, dechlorination with sulfur dioxide gas and an aerated sludge holding basin. However, a receiving WWTP may have to get their Attachment A amended in their permit for an additional source. Otherwise, we would have to issue them some kind of a letter of approval if Sherri agrees and we get the OK from Raleigh. If I recall we have something similar to this before. The landfill would be the easiest scenario. They need to keep records for the land fill to turn in with their annual report so we know what happened to the sludge. WWTP owners . must reconcile their flow records to the best of their ability so it is known.how much flow has been obligated, but not vet made tributary to the WWTP Submit FTSE- 10/07 to the appropriate Regional. Office with Section I (a)-(e) completed by January 15, 2008." The instructions for Part I.d. of the FTS&10/07 form indicates that facilities that have obligated less that 60% of their permitted flow shall submit annual updates to the appropriate Rep-ional Office by January 15 of each year. Semi- annual updates are required if the obligated flow reaches 60% and quarterly updates when the obligated flow reaches 80%. Your submittal of 6-6-08 showed only 476/o obligated flow. As you prepare this document please recognize that we will require a description of the method used to determine the paper obligated flows which are not yet tributary (item d. on the FTSE 10/07 form). The existing WWTP consists of: bar screen, influent pump station, and flow splitter. The flow is then split equally between the equalization basin and SBR w/floating decanter, and the aeration basin w/o EQ) and secondary clarifier. Flow then recombines prior to gas chlorination unit, contact chamber, sulfur dioxide gas dechlorination unit and aerated sludge holding. Mike Mickey, from the Winston-Salem Regional office; conducted a site visit on 10-11-05 and found the facility in compliance at the time of inspection. The WSRO recommends permit renewal and states the facility is consistently in compliance. The facility has a 160,000-gallon digestor where sludge is wasted on a daily basis and is held for stabilization for a period of twelve months. It is then applied to farmland in conjunction with land application permit #WQ0006242 by Synagro Corporation. PRETREATMENT The facility has an active modified pretreatment program with a short term monitoring plan and no permitted SIU's since 2002 but wants to maintain their program as active. Metals values reported by pretreatment STMP and DMR submittals shows no values over permit limits. RECEIVING STREAM This facility discharges to Little River in the New River Basin, classified C waters in Subbasin 05-07-03. The Little River, from source to the New River (32.8 miles), is Supporting in the aquatic life category. The Little River is Not Rated for recreational use due to elevated fecal coliform bacteria. Potential sources of elevated bacteria levels include failing septic systems, straight pipes, and nonpoint source runoff from pasturelands. DWQ will continue to monitor water quality in Little River and work with local agencies to identify possible sources of the elevated fecal bacteria levels. INSTREAM MONITORING: Not required in the existing permit, will not be required in permit renewal. Toxicant Analysis: A reasonable potential analysis (RPA) was not conducted because all values were below detect for the toxicants currently monitored in the NPDES permit and the Pretreatment STMP. The data evaluated was reported between from July 2003 - July 2006. The DMR reported values below detection for a three-year period. Monitoring will continue through the NPDES permit and the STMP. Aquatic Toxicity Testing Testing results from (2002-2006) reflects passing the test with no failures. Proposed Changes: • Addition of Weekly Average NH3 limits of 27.9 mg/ L for Summer Months. • Addition of pH limits on effluent page. • Addition of Pollutant Analysis on 0.60 Effluent Page. • Deletion of Cadmium, Selenium and Mercury limits. • Change in monitoring frequency for Cadmium, Selenium and Mercury from weekly to 2/month.
