Mr. Gale D. Howell, ORC, Grade IV, and Mr. John Turner, Back-up, Grade I, were present for the inspection. The inspection is comprised of an overall evaluation in order to document compliance and noncompliance of. facilities equipment, capability to meet effluent limits, and NPDES permit terms & conditions. Permit Your NPDES permit became effective on January 1, 2012 and expires on March 31, 2016. The permit identifies the following discharge pipes: (001) Non -contact cooling water, A/C condensate, boiler blow -down, de -ionizer backwash, assembled product wash water & process wastewater. (003) A/C condensate. (006) Domestic wastewater. (007) Recycled non -contact cooling water, and filtered surface water during maintenance periods only. Records & Reports Mr. Howell maintains a log. Mr. Howell documents phone conversations, calibrations, process control, and all are done in an excellent manner. Facility Site Review The effluent from the Imhoff tank was clear with no visible solids. The sandfilters were raked and appeared to be clean and in good condition. There was no evidence of ponding on the surface of the sandfilters. The dosing lines and concrete pads are intact. 450 W. Hanes Mill Road, Winston-Salem, North Carolina 27105 DKnn 11P 77P WN) \ In#ornc}• nnrionr nnv "Im Flow Measurement The instantaneous flow at Outfall 006 is obtained by measuring the height of the water in a 1-inch parshall flume then converting it to flow (MGD). The Stevens, Model 61R flow meter at Outfall 001 was calibrated by B.W. Allen on June 25, 2015. Laboratory Mr. Howell performs pH and Temperature; and these tests are recorded on the daily monitoring report. A field certification for pH and Temperature was issued to Mr. Butch Howell for United Chemi-Con (lab #5099). All remaining tests are performed by TriTest (Certified Lab #103). Calibration for pH is performed using Fisher standards of pH 4 and 10, and a check at pH 7. The temperature is recorded against a NIST traceable thermometer. Effluent/Receiving Waters The effluent from Pipe 006 to the North Fork New River, Class C HQW waters of the State was clear, no color, no solids, or no odor. The effluent from pipes 001 and 006 do not impact the North Fork New River. Pipe 001 is discharging clear water with no color. The manhole with the parshall flume for Pipe 006 has some solids from the river backing -up into the flume. Effluent pipes 003 and 007 were not discharging. In conclusion, none of the discharge pipes appear to be impacting the river. Operations & Maintenance Mr. Butch Howell, Grade IV is the current ORC. The back-up operator is Mr. John Turner, Grade I. The facility (pipe 006) with the Imhoff tank is considered a Class I facility. The facility was well maintained by the staff, and they are considered to perform their jobs in an outstanding manner. The sandbed is well maintained. The Imhoff tank appears to be well operated & maintained. Process control tests of pH, ammonia, alkalinity, oil & grease are performed on the influent and Imhoff tank. These process control tests appear to be successfully used in measuring plant performance. rSeif-Monitoring Program A review of the monthly self -monitoring reports from June 2014 through March 2015 did not show any limit violations. Grab samples are taken from the end of the pipe 006 at the bank of the river. The composite sampler at Pipe 001 is time composite at 300 mL every 30 minutes. This time composite sample is acceptable because the flow only varies related to non -contact cooling water, which is tied to plant production rates. There are no inflow & infiltration issues. The chain -of -custody reports were accurate and in good order. Mr. Howell continues to perform in an excellent manner. The compliance evaluation inspection is considered satisfactory. Mr. Gale D. Howell, ORC, Grade IV, was present for the inspection. The inspection is comprised of an overall evaluation in order to document compliance and noncompliance of. facilities equipment, capability to meet effluent limits, and NPDES permit terms & conditions. Permit Your NPDES permit became effective on January 1, 2012 and expires on March 31, 2016. The permit identifies the following discharge pipes: (001) Non -contact cooling water, A/C condensate, boiler blow -down, de -ionizer backwash, assembled product wash water & process wastewater. (003) A/C condensate. (006) Domestic wastewater. (007) Recycled non -contact cooling water, and filtered surface water during maintenance periods only. Records & Reports Mr. Howell maintains a log. Mr. Howell documents phone conversations, calibrations, process control, and all are done in an excellent manner. Facility Site Review The effluent from the Imhoff tank was clear with no visible solids. There was no evidence of ponding on the surface of the sandfilters. The dosing lines and concrete pads are intact. Flow Measurement The instantaneous flow at Outfall 006 is obtained by measuring the height of the water in a 1-inch parshall flume then converting it to flow (MGD). The Stevens, Model 61R flow meter at Outfall 001 was calibrated by B.W. Allen last year and is scheduled for calibration June 5, 2013. Laboratory Mr. Howell performs pH and Temperature; and these tests are recorded on the daily monitoring report. A field certification for pH and Temperature was issued to Mr. Butch Howell for United Chemi-Con (lab #5099). All remaining tests are performed by TriTest (Certified Lab #103). Calibration for pH is performed using Fisher standards of pH 4, 7, 10. The temperature is recorded against a NIST traceable thermometer. Effluent/Receiving Waters The effluent from Pipe 006 to the North Fork New River, Class C HQW waters of the State was clear, no color, no solids, or no odor. The effluent from pipes 001, 003, 006, and 007 do not impact the North Fork New River. The manhole with the parshall flume for Pipe 006 is free of debris Effluent pipes 003 and 007 were not discharging. Operations & Maintenance Mr. Butch Howell, Grade IV is the current ORC. The back-up operator is Mr. Allan Miller is in training. The facility (pipe 006) with the Imhoff tank is considered a Class H facility. The facility was well maintained by the staff, and they are considered to perform their jobs in an outstanding manner. The sandbed is well maintained. The Imhoff tank appears to be well operated & maintained. Process control tests of pH, ammonia, alkalinity, oil & grease are performed on the influent and Imhoff tank. Process control tests appear to be successfully used in measuring plant performance. rPageof3 3 Self -Monitoring Program A review of the monthly self -monitoring reports from June 2012 through February 2013 did not show any limit violations. Grab samples are taken from the end of the pipe 006 at the bank of the river. The composite sampler at Pipe 001 is time composite at 200 mL every 30 minutes. The chain -of -custody reports were accurate and in good order. Mr. Howell continues to perform in an excellent manner. The compliance evaluation inspection is considered satisfactory. Mr. Gale D. Howell, ORC, Grade IV, was present for the inspection. The inspection is comprised of an overall evaluation in order to document compliance and noncompliance of. facilities equipment, capability to meet effluent limits, and NPDES permit terms & conditions. Permit Your NPDES permit became effective on January 1, 2012 and expires on March 31, 2016. This permit is currently effective until the new permit is issued. The permit identifies the following discharge pipes: (001) Non -contact cooling water, A/C condensate, boiler blow -down, de -ionizer backwash, assembled product wash water & process wastewater. (003) A/C condensate. (006) Domestic wastewater. (007) Recycled non -contact cooling water, and filtered surface water during maintenance periods only. Records & Reports Mr. Howell maintains a log. Mr. Howell documents phone conversations, calibrations, process control, and all are done in an excellent manner. Facility Site Review The effluent from the Imhoff tank was clear with no visible solids. There was no evidence of ponding on the surface of the sandfilters. The dosing lines and concrete pads are intact. Flow Measurement The instantaneous flow at Outfall 006 is obtained by measuring the height of the water in a 1-inch parshall flume then converting it to flow (MGD). The Stevens, Model 6 1 R flow meter at Outfall 001 was calibrated by B.W. Allen on March 23, 2012. Laboratory Mr. Howell performs pH and Temperature; and these tests are recorded on the daily monitoring report. A field certification for pH and Temperature was issued to Mr. Butch Howell for United Chemi-Con (lab #5099). All remaining tests are performed by TriTest (Certified Lab #103). Calibration for pH is performed using Fisher standards of pH 4, 7, 10. The temperature is recorded against a NIST traceable thermometer. Effluent/Receiving Waters The effluent from Pipe 006 to the North Fork New River, Class C HQW waters of the State was clear, no color, no solids, or no odor. The effluent from pipes 001, 003, 006, and 007 do not impact the North Fork New River. The manhole with the parshall flume for Pipe 006 was cleaned. It contained mud and algae which may have affected the sample integrity. Effluent pipes 003 and 007 were not discharging. Operations & Maintenance Mr. Butch Howell, Grade IV is the current ORC. The back-up operator is Mr. Allan Miller is in training. The facility (pipe 006) with the Imhoff tank is considered a Class II facility. The facility was well maintained by the staff, and they are considered to perform their jobs in an outstanding manner. The sandbed is well maintained. The Imhoff tank appears to be well operated & maintained. FJCEl une 11, 2012 Page 3 of 3 Process control tests of pH, ammonia, alkalinity, oil & grease are performed on the influent and Imhoff tank. Process control tests appear to be successfully used in measuring plant performance. Self -Monitoring Program A review of the monthly self -monitoring reports from April 2011 through February 2012 did not show any limit violations. Specifically, your concern regarding the "... stream of foam..." being discharged from Outfall 001 during Mr. George Smith's inspection. As per Mr. Smith's request, an immediate investigation was conducted to determine the excessive foam mentioned above. This investigation was inconclusive, but revealed the probable source of the foam to be from the cleaning of air conditioning coils on the roof of our facility. As the coils were being cleaned, the detergent was washed into the rainwater collection system and carried to Outfall 001, causing the excessive foaming. The material being used was Hydro Balance Non -Acid Hydro-Kleen Concentrate, and was being used in accordance with instructions on the container. Mr. Gale D. Howell, ORC, Grade IV, was present for the inspection. Mr. Allan Miller is currently training as a Back-up operator. The inspection is comprised of an overall evaluation in order to document compliance and noncompliance of: facilities equipment, capability to meet effluent limits, and NPDES permit terms & conditions. Permit Your NPDES permit became effective on January 1, 2007 and expires on March 31, 2011. This permit is currently effective until the new permit is issued. The permit identifies the following discharge pipes: (001) Non -contact cooling water, A/C condensate, boiler blow -down, de -ionizer backwash, assembled product wash water & process wastewater. (003) A/C condensate. (006) Domestic wastewater. (007) Recycled non -contact cooling water, and filtered surface water during maintenance periods only. Records & Reports Mr. Howell maintains a log. Mr. Howell documents phone conversations, calibrations, process control, and all are done in an excellent manner. Facility Site Review The effluent from the Imhoff tank was clear with no visible solids. There was no evidence of ponding on the surface of the sandfilters. The dosing lines and concrete pads are intact. Flow Measurement The instantaneous flow at Outfall 006 is obtained by measuring the height of the water in a 1-inch parshall flume then converting it to flow (MGD). The Stevens, Model 61R flow meter at Outfall 001 was calibrated by B.W. Allen on March 23, 2011. Laboratory Mr. Howell performs pH and Temperature; and these tests are recorded on the daily monitoring report. A field certification for pH and Temperature was issued to Mr. Butch Howell for United Chemi-Con (lab #5099). All remaining tests are performed by TriTest (Certified Lab #103). Calibration for pH is performed using Fisher standards of pH 4, 7, 10. The temperature is recorded against a NIST traceable thermometer. Effluent/Receiving Waters The effluent from Pipe 001 to the North Fork New River, Class C + waters of the State showed a continuous stream of foam entering the river and then dissipated. There are no obvious impacts. The permit requires that no visible foam should be discharged other than trace amounts. The effluent from Pipe 006 to the North Fork New River, Class C + waters of the State was clear, no color, no solids, or no odor. The effluent from pipes 001, 003, 006, and 007 do not impact the North Fork New River. Effluent pipes 003 and 007 were not discharging. Operations & Maintenance Mr. Butch Howell, Grade IV is the current ORC. The back-up operator is Mr. Allan Miller is in training. The facility (pipe 006) with the Imhoff tank is considered a Class II facility. CEI June 7, 2011 Page 3 of 3 The facility was well maintained by the staff, and they are considered to perform their jobs in an outstanding manner. The sandbed is well maintained. The Imhoff tank appears to be well operated & maintained. Process control tests of pH, ammonia, alkalinity, oil & grease are performed on the influent and Imhoff tank. Process control tests appear to be successfully used in measuring plant performance. Self -Monitoring Pro ram A review of the monthly self -monitoring reports from April 2010 through February 2011 did not show any limit violations. Please note that monitoring must be performed according to the permit except for weekends and State holidays. (please see attached memo) Grab samples are taken from the end of the pipe 006 at the bank of the river. The composite sampler at Pipe 001 is time composite at 200 mL every 30 minutes. The chain -of -custody reports were accurate and in good order. Action Item(s) During the previous report it was noted that process wastewater was hauled away from the site. During the inspection of Outfall 001 a continuous stream of foam was discharged to the river and then dissipated. There are no obvious impacts to the river. Your permit does allow the discharge of assembled product wash water and process wastewater. Please investigateprocess water that is discharged from Outfal1001, and the pollutants that are present and provide a resort to this office. Please respond to this office, in writing, in ten (10) days of receipt of this letter. Mr. Howell continues to perform in an outstanding manner. Mr. Gale D. Howell, ORC, Grade IV, was present for the inspection. Mr. Gale Wyatt is the Grade I Back-up operator. The inspection is comprised of an overall evaluation in order to document compliance and noncompliance of: facilities equipment, capability to meet effluent limits, and NPDES permit terms & conditions. Permit Your NPDES permit became effective on January 1, 2007 and expires on March 31, 2011. The permit identifies the following discharge pipes: (001) Non -contact cooling water, A/C condensate, boiler blow -down, de -ionizer backwash, assembled product wash water & process wastewater. Process wastewater is not discharged, it is hauled away. (003) A/C condensate. (006) Domestic wastewater. (007) Recycled non -contact cooling water, and filtered surface water during maintenance periods only. Records & Reports Mr. Howell maintains a log. Mr. Howell documents phone conversations, calibrations, process control, and all are done in an excellent manner. Facility Site Review The effluent from the Imhoff tank was clear with no visible solids. There was no evidence of ponding on the surface of the sandfilters. The dosing lines were discharging onto the concrete pads. They worked good. Flow Measurement The instantaneous flow at Outfall 006 is obtained by measuring the height of the water in a 1-inch parshall flume then converting it to flow (MGD). The Stevens, Model 6 1 R flow meter at Outfall 001 was calibrated by B.W. Allen on March 23 2010. Laboratory Mr. Howell performs pH and Temperature; and these tests are recorded on the daily monitoring report. A field certification for pH and Temperature was issued to Mr. Butch Howell for United Chemi-Con (lab #5099). All remaining tests are performed by Tritest (Certified Lab #103). Calibration for pH is performed using Fisher standards of pH 4, 7, 10. The temperature is recorded against a NIST traceable thermometer. Effluent/Receivine Waters The effluent from Pipes 001 and 006 to the North Fork New River, Class C + waters of the State was clear, no color, no solids, or no odor. The effluent from pipes 001, 003, 006, and 007 do not impact the North Fork New River. Effluent pipes 003 and 007 were not discharging. Operations & Maintenance Mr. Butch Howell, Grade IV is the current ORC. The back-up operator is Mr. Gale Wyatt, Grade I. The facility (pipe 006) with the Imhoff tank is considered a Class II facility. The facility was well maintained by the staff, and they are considered to perform their jobs in an outstanding manner. The sandbed is well maintained. The Imhoff tank appears to be well operated & maintained. Process control tests of pH, ammonia, alkalinity, oil & grease are performed on the influent and Imhoff tank. Process control tests appear to be successfully used in measuring plant performance. FX4r. Howell April 29, 2010 Page 3 of 3 Self -Monitoring Program A review of the monthly self -monitoring reports from March 2009 through December 2009 did not show any monitoring or limit violations. Grab samples are taken from the end of the pipe 006 at the bank of the river. The composite sampler at Pipe 001 is time composite at 200 mL every 30 minutes. The chain -of -custody reports were accurate and in good order. Mr. Gale Wyatt performs the plant Operations & Maintenance in an outstanding manner. Permit Your NPDES permit became effective on January 1, 2007 and expires on March 31, 2011. Flow Measurement The instantaneous flow at Outfall 006 is obtained by measuring the height of the water in a 1-inch parshall flume then converting it to flow (MGD). The Stevens, Model 6 1 R flow meter at Outfall 001 was calibrated by B.W. Allen on March 11, 2009. Laboratory Mr. Howell performs pH and Temperature; and these tests are recorded on the daily monitoring report. A field certification for pH and Temperature was issued to Mr. Butch Howell for United Chemi-Con (lab #5099). All remaining tests are performed by Tritest (Certified Lab #103). Calibration for pH is performed using Fisher standards of pH 4, 7, 10. The temperature is recorded against a NIST traceable thermometer. Effluent/Receiving Waters The effluent from Pipes 001 and 006 to the North Fork New River, Class C + waters of the State was clear, no color, no solids, or no odor. The effluent from pipes 001, 003, 006, and 007 do not impact the North Fork New River. Effluent pipes 003 and 007 were not discharging. Operations & Maintenance Mr. Butch Howell, Grade IV is the current ORC. The back-up operator is Mr. Gale Wyatt, Grade I. The facility (pipe 006) with the Imhoff tank is considered a Class II facility. The facility was well maintained by the staff, and they are considered to perform their jobs in an outstanding manner. The sandbed is well maintained. The Imhoff tank appears to be well operated & maintained. Process control tests of pH, ammonia, alkalinity, oil & grease are performed on the influent and Imhoff tank. Director Division of Water Quality A Compliance Evaluation Inspection was performed by George Smith and Sue White on May 10, 2007. Mr. Gale D. Howell, ORC, Grade IV, was present for the inspection. Mr. Gale Wyatt is the Grade I Back-up operator. The inspection was comprised of an overall evaluation in order to document compliance and noncompliance of: facilities equipment, capability to meet effluent limits, and NPDES permit terms & conditions. The attached inspection form evaluates nine (9) areas as noted in Section C of the attached EPA inspection report, and the observations and comments during the inspection are addressed below: Permit Your NPDES permit became effective on January 1, 2007 and expires on March 31, 2011. The permit identifies the following discharge pipes: (001) Non -contact cooling water, A/C condensate, boiler blowdown, de -ionizer backwash, assembled product wash water & process wastewater. Process wastewater is not discharged, it is hauled away. (003) A/C condensate. (006) Domestic wastewater. (007) Recycled non -contact cooling water, and filtered surface water during maintenance periods only. Records & Reports Mr. Howell maintains a log. Mr. Howell documents phone conversations, calibrations, process control, and all are done in an excellent manner. The sandfilters were raked and appeared to be clean and in good condition. There was no evidence of ponding on the surface of the sandfilters. The dosing lines appear to be functioning properly. Flow Measurement The instantaneous flow at Outfall 006 is measured with a 1-inch parshall flume by measuring the head and converting it to flow (MGD). The Stevens, Model 61 R flow meter at Outfall 001 was calibrated by B.W. Allen on March 14, 2007. The instantaneous flow from the chart read 0.002 MGD at the time of the inspection. Laboratory Mr. Howell performs pH and Temperature; and these tests are recorded on the daily monitoring report. A field certification for pH and Temperature was issued to Mr. Butch Howell for United Chemi-Con (lab #5099). All remaining tests are performed by Tritest (Lab #103), which is,a certified lab. The pH calibration is performed using a two -point calibration method (pH 4 & 10), and a mid -range check (pH 7). The calibrations were documented. The temperature is recorded against a NIST traceable thermometer. Effluent/Receiving Waters The effluent from Pipes 001 and 006 to the North Fork New River, Class C + waters of the State was clear, no color, no solids, or no odor. The effluent from pipes 001, 003, 006, and 007 did not appear to impact the North Fork New River. In fact, pipes 003 and 007 were not discharging. Operations & Maintenance Mr. Butch Howell, Grade IV is the current ORC. The back-up operator is Mr. Gale Wyatt, Grade I. The facility (pipe 006) with the Imhoff tank is considered a Class II facility. JMr. Howell June 11, 2007 Page 3 of 3 The facility was well maintained by the staff, and they are considered to perform their jobs in an outstanding manner. The equipment appeared to be in good operating order. The Imhoff tank showed vigorous degassing at the sidewalls, which indicate that the system is well operated & maintained. Process control tests of pH, ammonia, alkalinity, oil & grease are performed on the influent and Imhoff tank. Process control tests appear to be successfully used in measuring plant performance. Self -Monitoring Program A review of the monthly self -monitoring reports from April 2006 through March 2007 did not show any monitoring or limit violations. Data verification was performed on November 2006 data. The lab data and the monitoring report data were identical. The grab samples, if required, are taken from the end of the pipe at all outfalls. The composite sampler at Pipe 001 is time composite at 200 mL every 30 minutes. The chain -of -custody reports were accurate and in good order. Sludge Disposal Sludge is removed from the plant by Cox Septic Service. Mr. Howell continues to perform his job in an outstanding manner. Mr. Gale Wyatt performs the plant Operations & Maintenance in an outstanding manner. The compliance evaluation inspection is considered excellent. This review process is referred to as the "triennial review." Public hearings were held to solicit comments for the 2000-2003 Triennial Review of surface cater quality standards during the summer of 2002. The Hearing Officer's recommendations were presented before the state's Environmental Management Commission on October 10, 2002. The Hearing Officer's recommended changes included modifications to the existing standards for Arsenic, Cyanide, Methylene Blue Active Substances (MBAS) and Total Residual Chlorine (TRC). The Hearing Officer also recommended the addition of a standard for Methyl Tert-Butyl Ether (MTBE). All of the Hearing Officer's proposed changes were adopted with the exception of NME. The changes took effect on April 1, 2003. This memo documents the changes to the NPDES permitting policy for TRC limits. Total Residual Chlorine (TRC) Previously, TRC had an Action Level Standard, except in designated Trout waters. The Triennial Review changes modified the TRC Action Level Standard to an Aquatic Life Standard of 17 µg/L The Aquatic Life Standard will remain for designated Trout waters. Aquatic Life Standard — 17 µg/L (freshwater only) Trout Waters — 17 µg/L Revised TRC Policy ➢ TRC limits will be assigned to permit renewals and all new permits issued after April 1, 2003. ➢ Facilities that do not use chlorine will not receive a TRC limit; however, the presence of a chlorine back-up system to augment UV or other disinfection requires a TRC Emit. ➢ Facilities discharging to streams with a 7Q10 <0.05 cfs (zero -flow streams) will receive a limit of 17 µg/L. ➢ Limits will be capped at 28 µg/L to protect against acute impacts. It is likely that many facilities will need to add dechlorination (or use another means of disinfection) to comply with the new TRC standard. Therefore, a compliance schedule of 18 months from the effective date of the permit may be added to any TRC limit added during the current permit renewal. This time period is allowed in order for the facility to budget and design/construct the dechlorination and/or alternative disinfection system(s). Permit NC0000019 CHRONIC TOXICITY PERMIT LIMIT (QRTRLY) The effluent discharge shall at no time exhibit observable inhibition of reproduction or significant mortality to Ceriodaphnia dubia at an effluent concentration of 0.75%% The permit holder shall perform at a minimum, quarterly monitoring using test procedures outlined in the "North Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or subsequent versions or "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or subsequent versions. The tests will be performed during the months of January, April, July and October. Effluent sampling for this testing shall be performed at the NPDES permitted final effluent discharge below all treatment processes. If the test procedure performed as the first test of any single quarter results in a failure or ChV below the permit limit, then multiple -concentration testing shall be performed at a minimum, in each of the two following months as described in "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or subsequent versions. The chronic value for multiple concentration tests will be determined using the geometric mean of the highest concentration having no detectable impairment of reproduction or survival and the lowest concentration that does have a detectable impairment of reproduction or survival. The definition of "detectable impairment," collection methods, exposure regimes, and further statistical methods are specified in the "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or subsequent versions. All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge Monitoring Form (MR-1) for the months in which tests were performed, using the parameter code TGP3B for the pass/fail results and THP3B for the Chronic Value. There was no difference between values reported at upstream and downstream sites. RPA Analysis RPAs were performed for copper, zinc, aluminum, and butrolactone. Aluminum monitoring will be removed due to the lack of reasonable potential for the levels present to cause an exceedance of EPA criteria (there are no NC standards for Al). Butrolactone monitoring will also be removed since no detections were noted during the entire review period. Quarterly monitoring for copper and zinc will remain. Reported levels were higher than the acute criteria, however monitoring will not be increased since the facility consistently passes its WET tests. WET Test Results United Chemic Con has a chronic whole effluent toxicity (WET) testing requirement at 0.75% effluent concentration. Klimek, P.E. Director Division of Water Quality June 15, 2006 A Compliance Evaluation Inspection was performed by George Smith on June 8, 2006. Mr. Gale D. Howell, ORC, Grade IV, was present for the inspection. Mr. Everett Adams back-up operator, Grade I, was not present. The inspection was comprised of an overall evaluation in order to document compliance and noncompliance of: facilities equipment, capability to meet effluent limits, and NPDES permit terms & conditions. The attached inspection form evaluates nine (9) areas as noted in Section C of the attached EPA inspection report, and the observations and comments during the inspection are addressed below: Permit Your NPDES permit became effective on April 1, 2001 and expires on March 31, 2006. The permit renewal application was sent to the NPDES Unit. The new permit was not yet issued. The permit identifies the following discharge pipes: (001) Non -contact cooling water, A/C condensate, boiler blowdown, de -ionizer backwash, assembled product wash water & process wastewater. Process wastewater is not discharged, it is hauled away. (003) A/C condensate. (006) Domestic wastewater. (007) Recycled non -contact cooling water, and filtered surface water during maintenance periods only. Records & Reports Mr. Howell maintains a log. The sandfilters were raked and appeared to be clean and in good condition. There was no evidence of ponding on the surface of the sandfilters. The dosing lines appear to be functioning properly. Flow Measurement The instantaneous flow at Outfall 006 is measured with a 1-inch parshall flume by measuring the head and converting it to flow (MGD). There was a thin film of mud noticed all over the flume since it is sometime submerged when the river is high. This was noticed during the previous inspection. The Stevens, Model 6 1 R flow meter at Outfall 001 was calibrated by B.W. Allen on April 6, 2006. The instantaneous flow from the chart read 0.10 MGD at the time of the inspection. Laboratory Mr. Howell performs pH and Temperature; and these tests are recorded on the daily monitoring report. A field certification for pH and Temperature was issued to Mr. Butch Howell for United Chemi-Con (lab #5099). All remaining tests are performed by Tritest (Lab 4103), which is a certified lab. The pH calibration is performed using a two -point calibration method (pH 4 & 10), and a mid -range check (pH 7). The calibrations were documented. The temperature is recorded against a NIST traceable thermometer. Effluent/Receiving Waters The effluent from Pipes 001 and 006 to the North Fork New River, Class C + waters of the State was clear, no color, no solids, or no odor. The effluent from pipes 001, 003, 006, and 007 did not appear to impact the North Fork New River. In fact, pipes 003 and 007 were not discharging. Operations & Maintenance Mr. Butch Howell, Grade IV is the current ORC. The back-up operator is Mr. Everett Adams, Grade I. The facility (pipe 006) with the Imhoff tank is considered a Class II facility. Mr. Howell June 15, 2006 Page 3 of 3 The facility was well maintained by the staff, and they are considered to perform their jobs in an outstanding manner. The equipment appeared to be in good operating order. The Imhoff tank showed vigorous degassing at the sidewalls, which indicate that the system is well operated & maintained. Process control tests of pH, ammonia, alkalinity, oil & grease are performed on the influent and Imhoff tank. Process control tests appear to be successfully used in measuring plant performance. The operator adds 50 pounds of Soda Ash approximately once a month to keep the alkalinity at a good level. Self -Monitoring Program A review of the monthly self -monitoring reports from April 2005 through March 2006 did not show any monitoring or limit violations. The grab samples, if required, are taken from the end of the pipe at all outfalls. The composite sampler at Pipe 001 is time composite at 200 mL every 30 minutes. The chain -of -custody reports were accurate and in good order. Sludge Disposal Sludge is removed from the plant by Cox Septic Service. The sludge drying beds are never used. Mr. Howell continues to perform his job in an outstanding manner. The compliance evaluation inspection is considered excellent. In order to insure the proper operation and maintenance of these systems, this Rule requires that all systems permitted for a physical / chemical process to treat wastewater be classified as physical / chemical systems. If the subject physical / chemical system consists of systems designed for (1) the remediation of contaminated groundwater, or (2) that utilizes a primarily physical process to treat wastewaters, (with the exception of reverse osmosis, electrodialysis, and utrafiltration systems), then that system shall be classified as a Grade I Physical / Chemical Water Pollution Control System.
