A Compliance Evaluation Inspection was performed on the subject facility on October 9, 2015 by George
Smith. Mr. Tim Church, ORC, was present for the inspection. The inspection is comprised of an overall
evaluation of the facilities equipment, capability to meet effluent limits, and compliance with the NPDES
permit.
Permit
Your NPDES permit became effective on April 1, 2011 and expires on March 31, 2016.
It is recommended that the permittee register for eDMR. It will be a permit requirement in your next permit
cycle.

Facility Site Review
The equipment, grounds, and circular sandfilter appeared to be in good condition. The area inside the fence
was overgrown with weeds.

The facility was not being operated at the time of the inspection, nor, did it appear to have been operated
within the last year.
Flow Measurement
There is no flow. In the event the facility will be restarted, you will be required to verify and calibrate the
flow measuring device within 10% accuracy.
Laboratory
There are no tests performed.
Effluent/Receiving Waters
There was no effluent discharge to Dog creek.
Operations & Maintenance
There is minimal operation & maintenance required since the facility is not in operation. In the event of a
plant restart, a back-up ORC of the appropriate grade will be required.
Self -Monitoring Program
A review of the monthly self -monitoring reports from January 2015 through August 2015 reported no flow.
The monitoring reports are correctly submitted.
Sludge Disposal
There is no sludge generated because the plant receives no wastewater.

A Compliance Evaluation Inspection was performed on the subject facility on December 2, 2010 by George Smith.
Mr. Tim Church, ORC, was present for the inspection. The inspection is comprised of an overall evaluation of the
facilities equipment, capability to meet effluent limits, and compliance with the NPDES permit.
Permit
Your NPDES permit became effective on April 1, 2006 and expires on March 31, 2011.
Records & Reports
The monthly monitoring reports are properly submitted.
Facility Site Review
There was clear water in the aeration basin at the time of the inspection.
The equipment, grounds, and circular sandfilter were in good condition.
The facility was not being operated at the time of the inspection, nor, did it appear to have been operated within the
last year.
Flow Measurement
There is no flow. In the event the facility will be restarted, you will be required to verify and calibrate the flow
measuring device within 10% accuracy.

Effluent/Receiving Waters
There was no effluent discharge to Dog creek.
Operations & Maintenance
There is minimal operation & maintenance required since the facility is not in operation. In the event of a plant
restart, a back-up ORC of the appropriate grade will be required.
Self -Monitoring Program
A review of the monthly self -monitoring reports from October 2009 through September 2010 reported no flow. The
monitoring reports are correctly submitted.
Sludge Disposal
There is no sludge generated because the plant receives no wastewater.

Sludge Management Plan
When the facility is operational, sludge is withdrawn from the aeration basin as needed to
maintain suitable levels of mixed liquor suspended solids. This work is contracted to a
local vacuum truck operator. The sludge is disposed at the Town of Jefferson WWTP.
