In compliance with permitting requirements all applications, reports, or
information submitted to the Director are to be signed by the City Manager or his duly
authorized representative. Additionally regulations state that if a duly authorized
representative is designated, it must be done in writing to the Director by the City
Manager.
By this letter, the City of Graham is notifying the Division of Water Quality that

RESIDUALS MANAGEMENT PLAN
GRAHAM-MEBANE WATER TREATMENT PLANT
NPDES PERMIT No. NCO045292
The Graham -Mebane Water Treatment Plant uses three 500,000
gallon lagoons to settle and store the periodic discharge of residuals from the
water treatment process. Two lagoons have earthen sides with concrete
bottoms and one has concrete sides and earthen bottom. The supernatant is
discharged to Back Creek in accordance with our current NPDES Permit.
The Ferric residuals are removed at approximate four to six month
intervals. The City has a contract with Synagro, Inc. to remove the residuals
from the lagoons. The residuals are pumped into 6500 gallon tanker trucks
and hauled off to be land applied. The City has its own farm and operates a
land application program in accordance with our Non -Discharge Permit,
WQ0003824. There are currently 58.2 acres permitted for land application.
The Ferric Residuals are surface applied and co -mingled on the fields with
the BioSolids generated from the City's Wastewater Treatment Facility in
accordance with the permit restrictions.
A fescue cover crop is maintained on the fields and is harvested and
used for cattle feed. Fields are maintained and crops harvested in
accordance with the permit requirements.
Nutrient and metal loadings and application rates are calculated and
reported in accordance with WQ0003824 requirements.
All land application activities are performed in accordance with the
WQ0003824 Non -Discharge Permit requirements and EPA 40 CFR Part 503
federal regulations.

Subject: Compliance Evaluation Inspection

Graham -Mebane Water Treatment Plant
Alamance County
Dear Mr. Quick:
Jenny Graznak of this office performed a Compliance Evaluation Inspection at the Graham -Mebane WTP
in Alamance County on May 27, 2015. Tonya Mann, Operator in Responsible Charge (ORC), and Mike
Champion, Lab Supervisor, were present during the inspection. This type of inspection consists of two
basic parts: an in -office review of facility files and self -monitoring data and an on -site inspection of the
facility. This particular inspection evaluated nine (9) areas, and observations from each area are
addressed below:
1. Permit
The permit became effective August 1, 2011 and expires May 31, 2016. A copy of the current permit was
available for review during the inspection.
2. Self -Monitoring Program
A review of the monthly self -monitoring reports from January 2014 through March 2015 showed no
violations of permit limits. All monitoring appears to have been performed per the frequencies specified
in the permit.
3. Laboratory
Mike Champion operates the lab for the facility. He monitors the following parameters on site: total
residual chlorine, pH, and temperature. The City of Graham WWTP Laboratory analyzes turbidity, total
phosphorus, and total suspended solids samples. Meritech, Inc. handles Total Iron, Total Fluoride, Total
Copper, Manganese, Total Nitrogen, and Whole Effluent Toxicity. Aluminum monitoring is only required
if alum is used as part of the treatment process, and the facility recently switched from alum to ferric
sulfate.
4. Flow Measurement
The operator calculates the effluent flow from the wastewater lagoons to be equal to the flow used
during backwashing. The backwashing flow is metered.

5. Sludge Handling Disposal .
Sludge disposal is handled by Synagro, Inc. Alum residuals are land applied on City of Graham property
under permit number WQ0003824. Synagro last removed 430,000 gallons of sludge on May 8, 2014.
Records of sludge removal are maintained with the facility records and reports. The lagoons are cleaned
out on a regular schedule: #1 every year, #2 every other year, and #3 every 5 years.
6. Operations & Maintenance
No issues were noted with respect to facility operation and maintenance. The daily operations and
maintenance information is well organized. Maintenance activities include the addition of caustic soda if
necessary, sludge removal from lagoons, and mowing around the lagoons. Back -Up ORC is James Aiken.
7. Facility Site Review
The plant discharges filter backwash water. Backwashing occurs approximately every 2 to 4 days. The
treatment process of the filter backwash water consists of three settling lagoons, arranged in series.
Effluent is discharged from Lagoon #3, after it passes through Lagoons #1 and #2 in series. The filter
backwash is dechlorinated with Captor prior to its discharge to the lagoons.
8. Records/Reports
The Discharge Monitoring Reports (DMRs) and lab data are maintained according to the permit. DMRs
include all permit parameters. A copy of this lab data was available for review during the inspection.
Lab data, chain of custody reports, equipment calibration sheets and Discharge Monitoring Reports are
very well organized and maintained.
9. Effluent/Receiving Waters
The effluent is discharged into Back Creek in the Cape Fear River Basin. At the time of the inspection,
the effluent appeared clear with no traceable solids.
The facility was found to be in compliance with NPDES Permit No. NC0045292. Please refer to the
attached report for specific details from the inspection. If you have any questions concerning this

Dear Mr. Quick:
Jenny Graznak of this office performed a Compliance Evaluation Inspection at the Graham -
Mebane WTP in Alamance County on March 13, 2014. Mike Carson, Operator in Responsible
Charge (ORC), was present during the inspection. This type of inspection consists of two basic
parts: an in -office review of facility files and self -monitoring data and an on -site inspection of the
facility. This particular inspection evaluated nine (9) areas, and observations from each area are
addressed below:
1. Permit
The permit became effective August 1, 2011 and expires May 31, 2016. A copy of the current permit
was available for review during the inspection.
2. Self -Monitoring Program
A review of the monthly self -monitoring reports from November 2012 through November 2013
showed one daily and one monthly violation of Total Suspended Solids limits in June. All monitoring
appears to have been performed per the frequencies specified in the permit.
3. Laboratory
Mike Champion is the Laboratory Supervisor for the facility. Water Treatment Plant staff analyzes
the following samples at the lab on site: Total Residual Chlorine, pH, and Temperature. The City of
Graham WWTP Laboratory analyzes Turbidity, Total Phosphorus, and Total Suspended Solids
samples for the facility. Meritech, Inc. analyzes Total Iron, Total Fluoride, Total Copper, Manganese,
Total Nitrogen, and Whole Effluent Toxicity. Aluminum monitoring is only required if alum is used as
part of the treatment process. The facility recently switched from alum to ferric sulfate.
4. Flow Measurement
The operator calculates the effluent flow from the wastewater lagoons to be equal to the flow used
during backwashing. The backwashing flow is metered.
5. Sludge Handling Disposal
Sludge disposal is handled by Synagro, Inc. Alum residuals are surface land applied on City of
Graham property under permit number WQ0003824. Synagro last removed 155,000 gallons of

reports. The lagoons are cleaned out on a regular schedule.
6. Operations & Maintenance
No issues were noted with respect to facility operation and maintenance. The daily operations and
maintenance information is well organized. Maintenance activities include the addition of caustic
soda, sludge removal from lagoons, and mowing around the lagoons. Back -Up ORC is Tonya Mann,
Grade I.
7. Facility Site Review
The plant discharges filter backwash water. Backwashing occurs approximately every 3 to 4 days.
The treatment process of the filter backwash water consists of three settling lagoons, arranged in
series. Effluent is discharged from Lagoon #3, after it passes through Lagoons #1 and #2 in series.
Filter backwash is dechlorinated with Captor.
8. Records/Reports
The Discharge Monitoring Reports (DMRs) and lab data are maintained according to the permit.
DMRs include all permit parameters. A copy of this lab data was available for review during the
inspection. Lab data, chain of custody reports, equipment calibration sheets and Discharge
Monitoring Reports are very well organized and maintained.
9. Effluent/Receiving Waters
The effluent is discharged into Back Creek in the Cape Fear River Basin. At the time of the
inspection, the effluent appeared clear with no traceable solids.
The facility was found to be in compliance with NPDES Permit No. NC0045292. Please refer

Sludge disposal is handled by Synagro, Inc. Alum residuals are surface land applied on City of
Graham property under permit number WQ0003824. Synagro last removed 248,000 gallons of
sludge on July 25. 2012. Receipts from this service are maintained with the facility's records.
Lagoon #1 is cleaned out approximately once per year. Lagoon #2 is cleaned out approximately
every 2-3 years. Lagoon #3 is cleaned out approximately every 5 years.

4 days. The backwash water treatment process consists of three settling lagoons, arranged in
series. Effluent is discharged from Lagoon #3, after it passes through Lagoons #1 and #2 in series.
Backwash is dechlorinated prior to discharge.
8. Records/Reports
The Discharge Monitoring Reports (DMRs) and lab data are maintained according to the permit.
DMRs include all permit parameters. A copy of this lab data was available for review during the
inspection. Lab data, chain of custody reports, equipment calibration sheets and Discharge
Monitoring Reports are very well organized and maintained.
9. Effluent/Receiving Waters
The effluent is discharged into Back Creek in the Cape Fear River Basin. At the time of the
inspection, the effluent appeared clear with no traceable solids.
The facility was found to be in compliance with NPDES Permit No. NC0045292. Please refer

Dear Mr. Quick:
Jenny Graznak of this office performed a Compliance Evaluation Inspection at the
Graham -Mebane WTP in Alamance County on January 18, 2012. Mike Carson, ORC, was
present for the inspection. This type of inspection consists of two basic parts: an in -office
review of facility files and self -monitoring data and an on -site inspection of the facility. This
particular inspection evaluated nine (9) areas, and observations from each area are addressed
below:
1. Permit
The permit became effective August 1, 2011 and expires May 31, 2016. Additional monitoring
requirements were added to the new permit: Iron, Copper, Manganese, and Whole Effluent
Toxicity. The Settleable Solids monitoring requirement was removed. Upstream and
downstream monitoring requirements were also removed.
2. Self -Monitoring Program
A review of the monthly self -monitoring reports from November 2010 through October 2011
showed no limit violations. All monitoring was performed per the frequencies specified in the
permit.
3. Laboratory
Mike Champion is the Laboratory Coordinator for the facility's on -site laboratory. Water
Treatment Plant staff analyzes the following samples at the lab on site: Total Residual Chlorine,
pH, and Temperature. The City of Graham WWTP analyzes Turbidity, Total Phosphorus, and
Total Suspended Solids samples for the facility. Meritech, Inc. analyzes Aluminum, Total Iron,
Total Fluoride, Total Copper, Manganese, Total Nitrogen, and Whole Effluent Toxicity.

Alamance County
Division (DWQ) personnel have reviewed and approved your application for renewal of the subject
permit. Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between North Carolina and the U.S. Environmental Protection Agency dated October 15,
2007 (or as subsequently amended).
Your April 12, 2011 letter to the DWQ requested the following changes to the draft permit:
1. "The Monitoring requirement for flow remains as they are in the existing permit."
a. The Measurement Frequency has been changed to 2/month and the Sample type is
changed to Instantaneous with the duration of the flow also required to be reported
(Footnote 1).
2. "A footnote to require Monitoring only if fluoride is being used as part of the treatment process."
a. Footnote #3 had been be added requiring monitoring only if the permittee backwashes with
fluoridated finish water.
3. "A footnote identical to the existing permit for Aluminum and Iron."
a. Footnote #4 has been added requiring monitoring only if alum is used as part of the
treatment process.
b. The Monitoring requirement for Iron is part of the revised "Water Treatment Strategy
(October 2009) and must remain in your permit.
4. "Remove the requirement to continue monitoring for total phosphorus."
a. The Quarterly Monitoring requirement for Total Nitrogen and Total Phosphorus is part of the
revised - Water Treatment Strategy (October 2009) - which indicates that facilities should
monitor for nutrients if discharging to Nutrient Sensitive waters (NSW). Your facility
discharges to Back Creek classified as C — NSW which requires these two monitoring
requirements in your permit.

Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to
obtain other permits which may be required by the Division of Water Quality or permits required by the Division
of Land Resources, the Coastal Area Management Act or any other Federal or Local governmental permit that

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are
hereby revoked. As of this permit issuance, any previously issued permit bearing this number
is no longer effective. Therefore, the exclusive authority to operate and discharge from this

1) For Instantaneous flow monitoring the duration of the discharges must be reported.
2) The Division shall consider all effluent TRC values reported below 50 ug/I to be in compliance with the permit.
However, the Permittee shall continue to record and submit all values reported by a North Carolina certified
laboratory (including field certified), even if these values fall below 50 ug/I.
3) Aluminum monitoring is required only if alum is used as part of the treatment process.
4) Parameter should be monitored in conjunction with toxicity test.
5) Fluoride shall be monitored if facility backwashes with fluoridated finish water
6) TN = TKN + NO3-N + NO2-N, where TN is Total Nitrogen, TKN is Total Kjeldahl Nitrogen, and NO3-N and NO2-N
are Nitrate and Nitrite Nitrogen, respectively.
7) Chronic Toxicity Monitoring P/F (Ceriodaphnia Dubia) @ 10.87 %, during the months of March, June, September,
and December. See Part I, Condition A. (2.). Further studies will be required to determine the cause of toxicity, if
any. The collection of samples shall coincide with the sampling of other parameters.
There shall be no discharge of floating solids or visible foam in other than trace amounts

A. (2.) CHRONIC TOXICITY MONITORING (QRTRLY)
The permittee shall conduct quarterly chronic toxicity tests using test procedures outlined in the "North
Carolina (Ceriodaphnia Dubia) Chronic Effluent Bioassay Procedure," Revised February 1998, or
subsequent versions.
The effluent concentration defined as treatment two in the procedure document is 10.87 %. The testing shall be
performed as a Ceriodaphnia dubia 7day pass/fail test. The tests will be performed during the months of
March, June, September and December. Effluent sampling for this testing shall be performed at the NPDES
Permitted final effluent discharge below all treatment processes.
All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge
Monitoring Form (MR-1) for the month in which it was performed, using the parameter code TGP3B.
Additionally, DWQ Form AT-1 (original) is to be sent to the following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Section no
later than 30 days after the end of the reporting period for which the report is made.
Test data shall be complete and accurate and include all supporting chemical/physical measurements performed
in association with the toxicity tests, as well as all dose/response data. Total residual chlorine of the effluent
toxicity sample must be measured and reported if chlorine is employed for disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required, the
permittee will complete the information located at the top of the aquatic toxicity (AT) test form indicating the
facility name, permit number, pipe number, county, and the month/year of the report with the notation of "No
Flow" in the comment area of the form. The report shall be submitted to the Environmental Sciences Section at
the address cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, then monthly
monitoring will begin immediately. Upon submission of a valid test, this monthly test requirement will revert to
quarterly in the months specified above.
Should any test data from this monitoring requirement or tests performed by the North Carolina Division of Water
Quality indicate potential impacts to the receiving stream, this permit may be re -opened and modified to include
alternate monitoring requirements or limits.
If the Permittee monitors any pollutant more frequently than required by this permit, the results of such
monitoring shall be included in the calculation & reporting of the data submitted on the DMR & all AT Forms
submitted.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control
organism survival, minimum control organism reproduction, and appropriate environmental controls,

Dear Mr. Quick:
Jenny Graznak of this office performed a Compliance Evaluation Inspection at the
Graham -Mebane WTP in Alamance County on October 12, 2010. Mike Champion, Laboratory
Coordinator, was present for the inspection since Mike Carson, ORC, had a family emergency.
This type of inspection consists of two basic parts: an in -office review of facility files and self -
monitoring data and an on -site inspection of the facility. This particular inspection evaluated
nine (9) areas, and observations from each area are addressed below:
1. Permit
The permit became effective March 1, 2007 and expires May 31, 2011.
2. Self -Monitoring Program
A review of the monthly self -monitoring reports from August 2009 through July 2010 showed no
limit violations. All monitoring is performed per the frequencies specified in the permit.
3. Laboratory
Mike Champion is the Laboratory Coordinator for the facility's on -site laboratory. WTP staff
analyzes the following samples: Settleable Solids, Total Residual Chlorine, pH, and
Temperature. Meritech, Inc. analyzes Aluminum, Total Fluoride, and Total Nitrogen. The City
of Graham WWTP analyzes Turbidity, Total Phosphorus, and Total Suspended Solids samples
for the facility. Iron monitoring is not required since iron is not used in the treatment process.
4. Flow Measurement
Plant staff estimates the effluent flow from the wastewater lagoons on a weekly basis by the
volume of backwash, taking into consideration the volume that is used for re -wash.
5. Sludge Handling Disposal
Sludge disposal is handled by Synagro, Inc. Alum residuals are surface land applied on City of
Graham property under permit number WQ0003824. Synagro last removed approximately one
million gallons of sludge from Lagoons #1 and #2 September 2010. Receipts from this service
are maintained with the facility records. Lagoon #1 is cleaned out approximately once per year.
Lagoon #2 is cleaned out approximately every 2-3 years. Lagoon #3 is cleaned out
approximately every 5 years.

6. Operations & Maintenance
No major problems were noted with respect to facility operation and maintenance. The daily
operations and maintenance logbook is very well organized. Maintenance activities include the
addition of lime, sludge removal from lagoons, and mowing. Buddy, the rescue dog from the
Alamance County Alamance Shelter, is still a valuable member of the WTP staff. His task is to
keep the lake and lagoons free of geese.
7. Facility Site Review
The plant discharges filter backwash water. Backwashing occurs on an as -needed basis.
Drinking water regulations mandate no more than 96 hours on a filter. The backwash water
treatment process consists of three settling lagoons, arranged in series. Effluent is discharged
from Lagoon #3, after it passes through Lagoons #1 and #2 in series.
8. Records/Reports
The Discharge Monitoring Reports (DMRs) and lab data are maintained according to the permit.
DMRs include all permit parameters. A copy of this lab data was available for review during the
inspection. Lab data, chain of custody reports, equipment calibration sheets and Discharge
Monitoring Reports are very well organized and maintained.
9. Effluent/Receiving Waters
The effluent is discharged into Back Creek in the Cape Fear River Basin. At the time of the
inspection, the effluent appeared clear with no traceable solids.
The facility was found to be in compliance with NPDES Permit No. NC0045292. Please
refer to the attached report for specific details from the inspection. If you have any questions

The Graham -Mebane Water Treatment Plant uses three 500,000
gallon lagoons to settle and store the periodic discharge of residuals from the
water treatment process. Two lagoons have earthen sides with cement
bottoms and one has cement sides and earthen bottom. The supernatant is
discharged to Back Creek in accordance with our current NPDES Permit.
The Alum residuals are removed at approximate four to six month
intervals. The City has a contract with Synagro, Inc. to remove the residuals
from the lagoons. The residuals are pumped into 6500 gallon tanker trucks
and hauled off to be land applied. The City has its own farm and operates a
land application program in accordance with our Non -Discharge Permit,
WQ0003824. There are currently 89.4 acres permitted for land application.
The Alum Residuals are surface applied and co -mingled on the fields with
the BioSolids generated from the City's Wastewater Treatment Facility in
accordance with the permit restrictions.
A fescue cover crop is maintained on the fields and is harvested and
used for cattle feed. Fields are maintained and crops harvested in
accordance with the permit requirements.
Nutrient and metal loadings and application rates are calculated and
reported in accordance with WQ0003824 requirements.
All land application activities are performed in accordance with the
WQ0003824 Non -Discharge Permit requirements and EPA 40 CFR Part 503
federal regulations.

5. Sludge Handling Disposal
Sludge disposal is handled by Synagro, Inc. Alum residuals are surface land applied on City of
Graham property under permit number WQ0003824. Synagro last removed approximately one
million gallons of sludge from Lagoons #1 and #3 on March 6, 2008. Receipts from this service
were available for review during the inspection. Lagoon #1 is cleaned out approximately once
per year. Lagoon #2 is cleaned out approximately every 2-3 years. Lagoon #3 is cleaned out
approximately every 5 years.

6. Operations & Maintenance
No major problems were noted with respect to facility operation and maintenance. The daily
operations and maintenance logbook is very well organized. Maintenance activities include the
addition of lime, sludge removal from lagoons, and mowing. Buddy, a dog from the Alamance
County Alamance Shelter, is a relatively new addition to the staff at the WTP. His task is to
keep the lake and lagoons free of geese.
7. Facility Site Review
The plant discharges filter backwash water. Backwashing occurs on an as -needed basis.
Drinking water regulations mandate no more than 96 hours on a filter. The backwash water
treatment process consists of three settling lagoons, arranged in series. Effluent is discharged
from Lagoon #3, after it passes through Lagoons #1 and #2.
8. Records/Reports
The Discharge Monitoring Reports (DMRs) and lab data are maintained according to the permit.
DMRs include all permit parameters. A copy of this lab data was available for review during the
inspection. Lab data, chain of custody reports, equipment calibration sheets and Discharge
Monitoring Reports are very well organized and maintained.
9. Effluent/Receiving Waters
The effluent is discharged into Back Creek in the Cape Fear River Basin. At the time of the
inspection, the effluent appeared clear with no traceable solids.
The facility was found to be in compliance with NPDES Permit No. NC0045292. Please
refer to the attached report for specific details from the inspection. If you have any questions

Graham -Mebane Water Treatment Plant, Alamance County
Dear Mr. Quick:
Jenny Freeman of this office performed a Compliance Evaluation Inspection at the
Graham -Mebane WTP in Alamance County on November 28, 2007. Mike Carson, Operator in
Responsible Charge (ORC), was present for the inspection. This type of inspection consists of
two basic parts: an in -office review of facility files and self -monitoring data and an on -site
inspection of the facility. This particular inspection evaluated nine (9) areas, and observations
from each area are addressed below:
1. Permit
The new permit became effective March 1, 2007 and expired May 31, 2011. The limit for
aluminum has been removed and the monitoring frequency changed to monthly, and the
monitoring requirement for ammonia -nitrogen has been removed from the permit. This permit
includes a Total Residual Chlorine limit of 17 ug/L that will take effect on September 1, 2008.
2. Self-Monitorinq Proqram
A review of the monthly self -monitoring reports from October 2006 through September 2007
showed no limit violations. All monitoring is performed per the frequencies specified in the
permit.
3. Laboratory
Mike Champion is the Laboratory Coordinator for the facility's on -site laboratory. WTP staff
analyzes the following samples: Settleable Solids, Total Residual Chlorine, pH, Turbidity and
Temperature. Meritech, Inc. analyzes Aluminum, Total Fluoride, Total Nitrogen, Total
Phosphorus, and Total Suspended Solids samples. Iron monitoring is not required since iron is
not used in the treatment process.

Plant staff estimates the effluent flow from the wastewater lagoons on a weekly basis by the
volume of backwash, taking into consideration the volume that is used for re -wash.

carefully to ensure thorough understanding o the conditions and requirements it contains.
An analysis of your discharge monitoring data indicated there is a reasonable potential to exceed
the water quality standard for aluminum. Therefore, a weekly average effluent limit for aluminum
has been added to the permit. Additionally, a total residual chlorine ("TRC") limit has been
added to the permit. Please see the attached TRC policy memo for details. According to
information provided on the permit renewal application form, fluoride and ammonia are
added to the system; therefore, monitoring for total fluoride and ammonia -nitrogen has been
added to the permit. Additionally, monitoring for total nitrogen and total phosphorus has been
added to the permit.
Submit any comments to me no later than thirty (30) days following your receipt of the draft.

A. (2.) PERMIT RE -OPENER: SUPPLEMENTARY NUTRIENT MONITORING
Pursuant to N.C. General Statutes Section 143-215.1 and the implementing rules found in
Title 15A of the North Carolina Administrative Code, Subchapter 02H, specifically 15A
NCAC 02H.0 1 12(b)(1) and 02H.0114(a), and Part II, Sections B.12. and B.13. of this Permit,
the Director of DWQ may reopen this permit to require supplemental nutrient monitoring of
the discharge. The additional monitoring will be to support water quality modeling efforts
within the Cape Fear River Basin, and shall be consistent with a monitoring plan developed

SUBJECT: Total Residual Chlorine Policy for NPDES permits
Every three years the State is required by the Clean Water Act to review its surface water quality standards and
classifications, determine if changes are needed and to make any necessary changes. This review process is referred to
as the "triennial review." Public hearings were held to solicit comments for the 2000-2003 Triennial Review of surface
water quality standards during the summer of 2002. The Hearing Officer's recommendations were presented before
the state's Environmental Management Commission on October 10, 2002.
The Hearing Officer's recommended changes included modifications to the existing standards for Arsenic, Cyanide,
Methylene Blue Active Substances (MBAS) and Total Residual Chlorine (TRC). The Hearing Officer also
recommended the addition of a standard for Methyl Tert-Butyl Ether (MTBE).
All of the Hearing Officer's proposed changes were adopted with the exception of MTBE. Th-t changes took effect
on April 1, 2003. This memo documents the changes to the NPDES permitting policy for TRC limits.
Total Residual Chlorine (TRC)
Previously, TRC had an Action Level Standard, except in designated Trout waters. The Triennial Review changes
modified the TRC Action Level Standard to an Aquatic Life Standard of 17 jig/L. The Aquatic Life Standard will
remain for designated Trout waters.

Revised TRC Policy
➢ TRC limits will be assigned to permit renewals and all new permits issued after April 1, 2003.
➢ Facilities that do not use chlorine will not receive a TRC limit; however, the presence of a chlorine back-up
system to augment LTV or other disinfection requires a TRC limit.
➢ Facilities discharging to streams with a 7Q10 <0.05 cfs (zero -flow streams) will receive a limit of 17 µg/L.
➢ Limits will be capped at 28 µg/L to protect against acute impacts.
It is likely that many facilities will need to add dechlorination (or use another means of disinfection) to comply with
the new TRC standard. Therefore, a compliance schedule of 18 months from the effective date of the permit may
be added to any TRC limit added during the current permit renewal. This time period is allowed in order for the
facility to budget and design/construct the dechlorination and/or alternative disinfection system(s). The NPDES
Unit has promulgated minimum design criteria in order to expedite permitting of dechlorination equipment.

Your NPDES permit expires on May 31, 2006. Federal (40 CFR 122.41) and North Carolina (15A NCAC
2H.0105(e)) regulations require that permit renewal applications must be filed at least 180 days prior to expiration of
the current permit. If you have already mailed your renewal application, you may disregard this notice.
To satisfy this requirement, your renewal package must be sent to the Division postmarked no later than
December 2, 2005. Failure to request renewal by this date may result in a civil assessment of at least $500.00.
Larger penalties may be assessed depending upon the delinquency of the request.
If any wastewater discharge will occur after May 31, 2006, the current permit must be renewed. Discharge of
wastewater without a valid permit would violate North Carolina General Statute 143-215.1; unpermitted discharges of
wastewater may be assessed civil penalties of up to $25,000 per day.

Jenny Freeman of this office performed a Compliance Evaluation Inspection at the
Graham -Mebane WTP in Alamance County on August 30, 2005. Mike Carson, Operator in
Responsible Charge (ORC), was present for the inspection. This type of inspection consists of
two basic parts: an in -office review of facility files and self -monitoring data and an on -site
inspection of the facility. This particular inspection evaluated nine (9) areas, and observations
from each area are addressed below:
1. Permit
The permit became effective June 1, 2001 and expires May 31, 2006. A copy of the permit was
available during the inspection.
2. Self -Monitoring Program
A review of the monthly self -monitoring reports from June 2004 through June 2005 showed no
limit violations. All monitoring is performed per the frequencies specified in the permit.
3. Laboratory
Sandra Adkins is the Laboratory Technician for the facility's on -site laboratory. WTP staff
analyzes the following samples: settleable solids, chlorine residual, pH, and temperature. Total
Suspended Residue and turbidity are analyzed at the wastewater treatment plant in Graham.
Meritech, Inc. analyzes aluminum samples.
4. Flow Measurement
Currently, plant staff is estimating the influent flow to the wastewater lagoons. Please note that
the permit requires a weekly estimate of effluent flow from the wastewater lagoons. You
should take necessary steps to meet this monitoring requirement.
5. Sludge Handling Disposal
Sludge disposal is handled by Synagro, Inc. Alum residuals are surface land applied on City of
Graham property under permit number WQ0003824.
6. Operations & Maintenance
No major problems were noted with respect to facility operation and maintenance.

The plant discharges filter backwash water. Backwashing occurs on an as -needed basis,
occurring almost everyday in July 2005 but only 8 out 31 days in May 2005. The treatment
process consists of three settling lagoons in series. Effluent is discharged from Lagoon #3. At
the time of the inspection, there were some floating solids on the surface of Lagoon #3.
However, the surface water appeared reasonably clear.
8. Records/Reports
The Discharge Monitoring Reports (DMRs) and lab data are maintained according to the permit.
DMRs include all permit parameters. A copy of this lab data was available for review during the
inspection.
9. Effluent/Receiving Waters
The effluent is discharged into Back Creek in the Cape Fear River Basin. At the time of the
inspection, the effluent appeared clear with no traceable solids.

A Compliance Evaluation Inspection was performed on the filter backwash discharge from
the Graham -Mebane Water Treatment Plant in Alamance County by Jenny Freeman of this office
on July 13, 2004. Mike Carson, ORC, was present for the inspection. This type of inspection
consists of two basic parts: an in -office review of facility files and self -monitoring data; and an on -
site inspection of the facility.
A review of the self -monitoring data submitted for the period June 2003 through May 2004
showed that all parameters were sampled and analyzed per the requirements of the permit. In
regards to compliance with effluent limits, an evaluation of the effluent data for the same time
period revealed complete compliance. Please refer to the attached report for specific details from
