The Compliance Evaluation Inspection was conducted by Kevin Mitchell and Tim Fox of the Asheville Regional Office. The facility is in non-compliance with permit NCG530148. Effluent monitoring data is required to be kept onsite for a minimum of three years. I have attached a copy of the permit and technical bulletin for your review of permit requirements. Delmar Holder and Operations Manager Mike Halodsky were present during the inspection. The facility appeared well maintained and operated. However, no monitoring records were kept onsite. Mr. Holder and Mr. Haldosky were given a copy of the permit and informed of the monitoring requirements. The Division also recommends that the settling basin for solids be maintained more often. This includes tracing the pathway of a storm sewer to its discharge point. c) If this application is being submitted by a consulting engineer (or engineering firm), include documentation from the applicant showing that the engineer (Or firm) submitting the application has been designated an authorized Representative of the applicant. d) Final plans for the treatment system (if applicable). The plans must be signed and sealed by a North Carolina registered Professional Engineer and stamped -"Final Design -Not released for construction". e) Final specifications for all major treatment components (if applicable). The specifications must be signed and sealed by a North Carolina registered Professional Engineer and shall include a narrative description of the treatment system to be constructed. This includes tracing the pathway of a storm sewer to its discharge point. c) if this application is being submitted by a consulting engineer (or engineering firm), include documentation from the applicant showing that the engineer (Or firm) submitting the application has been designated an authorized Representative of the applicant. d) Final plans for the treatment system (if applicable). The plans must be signed and sealed by a North Carolina registered Professional Engineer and stamped -"Final Design -Not released for construction". e) Final specifications for all major treatment components (if applicable). The specifications must be signed and sealed by a North Carolina registered Professional Engineer and shall include a narrative description of the treatment system to be constructed.
