Dear Mr. Thomas:
The Town of Pembroke's Pretreatment Annual Report (PAR), covering January 1st through December
31s' 2019, has been reviewed by the Division.
The review indicates that more information is needed to determine if the PAR satisfies the requirements
of 15A NCAC 2H.0908(b) and the Comprehensive Guidance of North Carolina Pretreatment Programs
Please respond within 60 days of receiving this letter and provide information or documentation for the
following:
• The PAR did not contain an SNC report. If there were no SNC events in 2019, you can submit the
form with "NONE" written on it.
• The PAR did not contain a Historical SNC form. A copy of the form should have been sent by the
Division in December of 2019. Please check the accuracy of this form and return it to the Division.
• The PAR indicates that Steven Roberts had unspecified permit violations in 2019. However, the
PPS form indicates that no NOVs were issued in 2019. Please provide additional information as to
the type of violations that occurred and whether NOVs were issued.
