Please submit an updated narrative. See attached -V1.8. "Other" is marked with the comment :Applying for a private permit concurrently that has the flow allocation for the school. However, there was flow requested in this application. Please explain. Please removed "other" checkmark and remove language. This was left over when sent to the Town for signature and was not removed. You will see this should line up with the private application not having an flow allocated. -IX. 6 High priority lines is marked 'N/A' the utility plan submitted appears to depict a the sewer line crossing a stream. Please verify that there are no high priority lines and then either check yes or no for this question. Please revise to "no". There is a stream crossing, however it will be bore and jack and will not disturb the stream and does not qualify as a high priority line as listed on the application. The applicant and the engineer who signed the application will need to agree with the changes (this can be done via email, and you do not need to mail in a new signature page). NCDWQ Fast -Track Application Attachment — Project Narrative Project: Herbert Akins Middle School Owner: Wake County Public School System -Private / Town of Fuquay-Varina - Public Engineer: CLH Design, PA Date: 3 April 2020 revised 12-May-2020 Wake County Public School System proposes to construct Herber Akins Middle School on the same parcel as the existing Herbert Akins Elementary School Campus. Proposed construction will include the new middle school building, associated parking and drop off areas for passenger & busses, as well as athletic fields. This site proposes private sewer which includes approximately 860-ft of 8-in public gravity sewer and 980- ft of 8-in private gravity sewer. Given this information, sewer permit applications packages for private and public sewer are provided with this submittal. FEMA FIRM # 3720065800J dated May 2, 2006 indicates that the site is located outside the limits of the 100-year flood plain. The estimated sewer discharges have been allocated in the offsite permit — WQ0032932. Jay, I wanted to follow up on my previous email regarding the public water permit for Herbert Akins Middle School. Can you take a look at the following comments from the state and let me know if the revisions (in red) are acceptable? The state is giving us slack and not requiring any new signatures, just email authorization of any revisions. If you could follow up at your earliest convenience that would be great, as the private permit also for this school is on hold until the public permit is revised. Thanks! I have a couple minor comments from the state regarding the public sewer permit for Herbert Akins Middle School. Erin with the state is letting me instruct her how I want to revise the public sewer applications without needing to send any new applications. She just asks that I receive written authorization from the engineer and the Town about the requested revisions. If you have no issues, would you mind replying to this with some language stating you approve of the below mentioned revisions? Just need to include the Town of Fuquay for the public portion in the header and fix a couple of typos in the narrative. V1.8. Please explain. 0 The town has requested that the flow be allocated on the public permit therefore, the application will be revised to uncheck the "other" option and remove language about flow allocation being on the private application.
