The proposed property is moderately sloped with areas of open cut grass, thick undergrowth, and 10-15 year tree growth. Land use in the immediate vicinity is both residential and
commercial.

The purpose of the project is to construct a roadway with sidewalk and utilities (water and sewer service) for future proposed 9, 24'X40' residential homes. For this proposed roadway,
we will be installing three new48" concrete culverts.

The proposed project is to construct a roadway, and with this roadway will be the installation of three new48" culverts. These culverts are to impact unnamed tributaries of Horse Creek.
The project will require the utilization of equipment such as a bull dozer, backhoe, excavator, front end loader, and dump truck. Construction activities include grading, filling, installing
erosion control measures, and utility installation.

LIMIT OF MODERATE WAVE ACTION NOTES TO USERS: For some coastal flooding zones the AE Zonecategory has been divided by a Limit of Moderate Wave Action (LiMWA). The LiMWA represents the approximatelandward limit of the 1.5-foot breaking wave. The effects of wave hazards between the VE Zone and the LiMWA(or between the shoreline and the LiMWA for areas where VE Zones are not identified) will be similar to, but lesssevere than those in the VE Zone.

The State of North Carolina hasimplemented a long term approach to floodplain management to decrease thecosts associated with flooding. 1.Obtain erosion control plan approval and
other applicable permits from Army Corps of
Engineers and NCDWQ.
2.Install temporary creek diversion piping
snug against existing creek bank.
3.Install upstream coffer dam.
4.Install downstream coffer dam.
5.Excavate area for culvert as needed and
install compacted ABC stone.
6.Install headwalls and new culvert. Ensure
that inlet and outlet of the culvert is 2"
below existing creek bed.
7.Backfill area over culvert.
8.Bank must be stabilized with erosion and
sediment control measures in place between
bank area and creek
9.Remove downstream coffer dam.
10.Remove upstream coffer dam and temporary
diversion piping and redirect stream thru
new culvert.
