The application form (Preconstruction Form) that you obtained from our website must be filled out in its entirety (we only received a copy of the first page of the application), seven copies made of the application and accompanying maps and pay a fee if you are applying for a 401 Water Quality Certification (fee of $200 for less than one acre of wetland impact or less than 150 feet of stream impact or $475 if impacts exceed those thresholds).
