The Division of Water Resources(DWR or the Division)received your application to renew the
subject permit on March 4,2019,unsigned by you.Please resubmit this application with proper
signature and date as it is required for our records [You may email it to the undersigned].
Our review process will be held pending receipt of your properly signed application.
Please review the attached draft permit carefully to ensure your understanding of the limits and
monitoring conditions,and to correct errors, if any. Concurrent with this notification,the
Division will solicit public comment on this draft by publishing a notice in newspapers having
circulation in the general Catawba County area,as required by the NPDES Program. Please
submit your written comments(if any),to my attention, DEQ/DWR/NPDES no later than
June 6,2020(approximately 30 days after receiving this document).We welcome your written
comments,but they are not mandatory.
Renewal Changes: The Division understands that you have made no significant alterations to
treatment processes since last renewal.We have therefore made only minimal updates to your
permit including facility treatment system and site location map.
eDMR Submittals: Federal regulations require electronic submittal of all discharge monitoring
reports(DMRs)and program reports [See Special Condition A. (3.)]. If you have filled out and
submitted to DWQ an electronic signature agreement,you are no longer required to submit
DMR hardcopy as back-up to the eDMR.

A. (2.) SPECIAL CONDITION— Spill Notification
[G.S. 143-215.1(b)]
(a) Contacting Public Health Directors
The Permittee must notify the Catawba,Iredell,Lincoln and Mecklenburg County Public Health
Directors within 12 hours of first knowledge by the owner/operator of any discharge of untreated
wastewater to waters of the State or a discharge from the wastewater treatment plant that has not
received adequate disinfection due to a malfunctioning treatment unit.

(b) Public Notification of Spills
The Permittee must notify the ublic of un eated stewat"'spills. tewater facility owners or
operators must issue a press re se after a discharge to surface waters of 1,000 gallons within 24 hours
of first knowledge of the-spill by he own /operator. The press release must be issued to"all electronic
and print news med. ou ets that provide coverage in the counties(Catawba,Iredell,Lincoln
and Mecklenburg w e the di harg�\ooecurred." A copy Of the press release must be maintained for
one year by the Pe ' e. This p ss.release is required in addition to the permit requirement to
contact the North Car ling Division fWa r esources(DWR).
If a discharge of 15,000 pitons or mo taches surface waters, a public notice is required in addition
to the press release. The pub' notice inu t be placed in a newspaper having general circulation in the
County in which the discharge ccurred and the county immediately downstream. The owner or
operator shall publish the notice ' 10 days after the Secretary has determined the counties that are
significantly affected by the discharge and approved r ved the form and content of the notice and the
newspapers in which the notice is to be published.
At a minimum the notice shall be published in the newspaper of general circulation in Catawba,
Iredell,Lincoln and Mecklenburg Counties. If a discharge of 1,000,000 gallons of wastewater or more
reaches surface waters,the DWR regional office must be contacted to determine in what additional
counties, if any,a public notice must be published. A copy of these public notices and proof of
publication must be sent to the attention of DWR's"Non-Discharge Compliance Enforcement Unit"at
this permit's letterhead address within 30 days of publication.
NOTE: The Permittee, at minimum shall,locate the spill,estimate spill volume,identify the surface
waterbody affected,and detail the steps taken to prevent future spills.

A. (3.) ELECTRONIC REPORTING OF DISCHARGE MONITORING REPORTS
[G.S. 143-215.1(b)]
Federal regulations require electronic submittal of all discharge monitoring reports(DMRs)and
program reports. The final NPDES Electronic Reporting Rule was adopted and became effective on
December 21,2015.
NOTE: This special condition supplements or supersedes the following sections within Part II of this
permit(Standard Conditions for NPDES Permits):
• Section SignatoryB. 11. Requirements
• Section D. (2.) Reporting
• Section D. (6.) Records Retention
• Section E. (5.) Monitoring Reports
1. Reporting Requirements [Supersedes Section D. (2.) and Section E. (5.) (a)l
The Permittee shall report discharge monitoring d e ctronically using the NC DWR's
Electronic Discharge Monitoring Report(eDMR) ' e et appli tion.
Monitoring results obtained during the pr i month( 1 e summarized4 each month and

If a permittee is unable to se tl� eDMR system due to a demonstrated hardship or due to the
facility being physically loc ed ' an area where less than 10 percent of the households have
broadband access,then a temp waiver from the NPDES electronic reporting requirements
may be granted and discharge monitoring data may be submitted on paper DMR forms(MR 1, 1.1,
2, 3)or alternative forms approved by the Director. Duplicate signed copies shall be submitted to
the mailing address above. See"How to Request a Waiver from Electronic Reporting"section
below.
Regardless of the submission method,the first DMR is due on the last day of the month following
the issuance of the permit or in the case of a new facility,on the last day of the month following the
commencement of discharge.

Starting on December 21,2020,the Permittee must electronically report the following compliance
monitoring data and reports,when applicable:
• Sewer Overflow/Bypass Event Reports;
• Pretreatment Program Annual Reports;and
• Clean Water Act(CWA) Section 316(b)Annual Reports.
The permittee may seek an electronic reporting waiver from the Division (see"How to Request a
Waiver from Electronic Reporting" section below).
2. Electronic Submissions
In accordance with 40 CFR 122.41(1)(9),the Permittee must identify the initial recipient at the time
of each electronic submission. The Permittee should use the EP ' bsite resources to identify
the initial recipient for the electronic submission.
Initial recipient of electronic NPDES information from I*P S-reg facilities means the
entity(EPA or the state authorized by EPA to implemenx the NPDES am)that is the
designated entity for receiving electronic NPDES data [see 40 CFR 127.

3. How to Rep st,a Waiver tronic Reporting
The permittee may s k a tempo lect p inic reporting waiver from the Division. To obtain an
electronic reporting waver, a perm tt e must fast submit an electronic reporting waiver request to
the Division. Requests f tempo electronic reporting waivers must be submitted in writing to
the Division for written approval at least sixty(60)days prior to the date the facility would be
required under this permit to begin submitting monitoring data and reports. The duration of a
temporary waiver shall not exceed 5 years and shall thereupon expire. At such time,monitoring
data and reports shall be submitted electronically to the Division unless the Permittee re-applies for
and is granted a new temporary electronic reporting waiver by the Division. Approved electronic
reporting waivers are not transferrable. Only permittees with an approved reporting waiver request
may submit monitoring data and reports on paper to the Division for the period that the approved
reporting waiver request is effective.

4. Signatory Requirements [Supplements Section B. (11.) (b); Supersedes Section B. (11.) (d)1
All eDMRs submitted to the permit issuing authority shall be signed by a person described in Part
II, Section B. (11.)(a)or by a duly authorized representative of that person as described in Part II,
Section B. (11.)(b). A person,and not a position,must be delegated signatory authority for eDMR
reporting purposes.
For eDMR submissions,the person signing and submitting the DMR must obtain an eDMR user
account and login credentials to access the eDMR system. For more information on North
Carolina's eDMR system, registering for eDMR and obtaining an eDMR user account, please visit
