The Division received your revised map on May 4, 2020, for the subject project depicting the sewer lateral
lines. However, the revised map does not denote the distances from the proposed well locations to the building
foundation.
Please confirm the proposed wells will meet the 15-foot offset from building foundations as required in 15A
NCAC 02C .0222(e)(5)(A).

septic system or sewer lateral lines.
Per 15A NCAC 02C .0222(d)(4)(A), please provide a map that depicts the applicable feature for the subject
residence.
The NOI is deemed incomplete and cannot be processed until this information is received.
