In accordance with a written request received March 3, 2016 from John Ciolino, Secretary, Gold
Hill Airpark Homeowner's Association, Inc., we are forwarding herewith corrected Page 1 of Permit No.
WQ0003687 issued March 9, 2015.
The correction to the subject permit is as follows:
1. The facility description has been changed from twenty-eight (28) residential lots in the Gold
Hill Airpark to thirty-four (34) residential lots in the Gold Hill Airpark. Note that all other
elements of the facility description remain the same, including flow of 10, 080 gallons per day.
2. The name of the permit issuer has been changed/updated from the Department of Environment
and Natural Resources to the Department of Environmental Quality.
This request required significant investigation and discussions among several Central Office Departments
and the Regional Office. A flow reduction was evaluated and considered to be inappropriate for the
situation. The actual measured flow contribution from the existing 26 homes indicated a total of 34 homes
could be served without exceeding the flow limitation of 10,080 gallons per day. A minor permit
modification was also considered. Because, however, the change did not involve modification of a facility
component or process and did not involve a change in facility design, operation, or flow, a permit correction
rather than a permit minor modification was deemed most appropriate.
Please replace Page 1 of Permit No. WQ0003687 issued March 9, 2015 with the enclosed
corrected Page 1.
