However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on February 17, 2020. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through F, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Please reference the subject application number when providing the requested information. All revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper copies and one electronic copy submitted to my attention at the address below. Cover Letter: 1. No Comments B. Application: 1. No Comments C. Property Ownership Documentation: 1. The previous permit issuance notes that there is an approved reduced setback from a property line under separate ownership allowing wastewater to be irrigated within 200 feet of the property. Please provide a copy of this setback waiver or provide the deed book and page that the document is registered in the Wake County Register of Deeds. E. Site Map: 1. If possible, provide a site map showing the property lines, irrigation zones, and all monitoring wells showing the entire site on one sheet. F. Current Permit: 1. The division has approved installation of monitoring wells to identify background contamination concentrations for this site for arsenic, iron, and manganese. Please specify if these wells have been constructed solely for use in determining background concentrations for these parameters and will be abandoned after the study, or if the new monitoring wells will be used for the entirety of this permit and will be sampled for all parameters listed in Attachment C. 2. The following schedules were included in the previous permit issuance and the Division does not have documentation showing they have been completed: i. Plans and specifications for a flow measuring device pursuant to Condition IV.3. shall be prepared by a licensed North Carolina Professional Engineer and submitted to the Raleigh Regional Office 30 days prior to installation. The flow measuring device shall be installed within one year of the date of this permit. ii. Prior to the distribution of DAF residuals to the City of Raleigh Wrenn Road Facility from DE Benton WTP, the Permittee shall submit a completed Residuals Source Certification for the DAF residual waste stream and all associated analyses to the Division demonstrating similar characteristics to the sedimentation blowdown waste stream and that the combined effluent qualities are suitable for the treatment at the City of Raleigh Wrenn Road Facility. Once the Division has approved, the Wrenn Road Wastewater Irrigation System shall be permitted to treat the DAF residual waste stream.
