You are required to submit this form, with items 1 through 10 completed, to the appropriate Division of Environmental Management Regional Water Quality Supervisor (see attached listing). At a minimum, you must include an 8.5" by I V copy of the portion of a 7.5 minute USGS Topographic Map which shows the subject surface waters. You must identify the location of the facility and the closest downslope surface waters (waters for which you are requesting the classification) on the submitted map copy. Three sets of detailed plans and specifications signed and sealed by a North Carolina Professional Engineer. The plans must include a general location map, a plan view of the sewer extension, a profile of the sewer extension, details on the pump station, and must show the proximity of the sewer extension to other utilities and natural features. Specifications may be omitted for delegated authorities. Each sheet of the plans and the first page of the specifications must be signed and sealed Three copies of the existing permit if a renewal or modification. C. Three copies of all calculations, including pump selection, friction calculations, cycle time, pump curves (including system curves applicable with one pump running, two pumps running, three pumps running, etc.), and evaluation of downstream pump stations. These items must be submitted under the signature and seal of the North Carolina Professional Engineer. d. The appropriate permit processing fee, in accordance with 15A NCAC 2H .0205(c)(5). e. If the owner/authority of the wastewater treatment facility (WW F) that will be accepting the wastewater flow from this project is different from the applicant of the project, then a letter must be provided from the owner/authority of the WW TF specifying the volume of flow that will be accepted. The letter should be a recent letter and should refer to the project by the same name as that identified on the application and the plans/specifications. f. OFFICE Subject: WQ0013456 Additional Information Union Pines Middle School Wastewater Collection and Pump Station Moore County The Permits and Engineering Unit has completed a preliminary engineering review of the subject application. The following items must be addressed before we can complete our review: The subject application was previously returned as incomplete because a flow acceptance letter from the owner of the receiving wastewater treatment plant was not provided. However, the resubmitted application (with treatment plant flow acceptance letter) indicates the subject wastewater flow will travel through the Town of Carthage's sewer collection system. Please confirm the Town's system will be used, and if so, please provide an acceptance letter from the Town. As the Town will be held responsible any future problems created by this new flow. 2. The submitted application indicates a stand by emergency generator is provided. However, the application received does not appear to have any information nor does it show on the submitted plans where and how this generator is provided. Is it an on site generator? If so, please show it on plans. Is it to be brought to the pump station in an emergency? If so, please provide calculations and confirmation that it can be brought to the site before a pump station overflow occurs (measured at average gallon per minute flow and available wet well storage above the alarm float level). 3. The submitted application states the new pump station will have 105 GPM pumps, however the submitted plans and specifications (which are also under a P.E. seal) show only 80 GPM pumps are used. Please confirm the size of the pumps to be used. 4. Two different sets of plans for the subject project have been submitted. One set shows only 6 inch gravity line from the School to the new pump station. P.O. One set of plans shows two force mains coming from the new pump station but both sets of plans show the pump station's wet well details with only one force main leaving the wet well. Please confirm and provide a set of revised plans if necessary of what gravity line sizes and force main numbers will be used will be used. 5. The submitted application states the highest point of the subject force main is at elevation 610 feet. However the submitted plans show the highest point elevation is near 205 feet. Please confirm and supply revised plans as to what the highest point of the force main is. 6. The submitted detail plans of the new pump station wet well shows venting for the pump's housing shelter, but the plans do not appear to show any venting of the wet well itself. A wet well vent is required by regulation, to prevent the build-up of explosive and/or corrosive gasses in the wet well itself. Refer to the subject permit application number when providing the requested information. Also, please note that failure to provide this additional information on or before April 11, 1997, will subject your application to being returned as incomplete in accordance with 15 NCAC 2H .0208.
