However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on June 1, 2020. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Application (15A NCAC 02T .0105): 1. Application Item II.4. indicates the facility is a Privately -Owned Public Utility. As such, a copy of the Certificate of Public Convenience and Necessity from the NC Utilities Commission demonstrating the Permittee is authorized to hold the utility franchise is required with the renewal application. Provide. 2. Application Item V.2. indicates reclaimed water is utilized on property not controlled by the generator. Per 15A NCAC 02U .0501(6). provide copies of all necessary leases, easements and/or agreements. Easements (executed and filed in the Register of Deeds in the county in which the land is located) shall indicate the intended use of the property and meet the requirements of 15A NCAC 02L .0107(f). B. Setbacks: 1. Verify that all setbacks are met: a. The reclaimed water irrigation sites (Fields S-1, S-2, S-3, S-4, S-5, S-6, S-7, S-8, S-9, P-DR, P-1, P-2, P-3, P-4, P-5, P-6, P-7, P-8, and P-9) were originally permitted and modified May 2, 1997 and July 29, 2004. The setbacks for irrigation sites originally permitted or modified from June 1, 1996 to August 31, 2006 are as follows (all distances in feet): i. Surface waters classified SA: 100 ii. Surface waters not classified SA: 25 iii. Each water supply well: 100 iv. Each non -potable well: 10 V. Each swimming pool: 25 [15A NCAC 02H .0219(k)(1)(C)(i)] b. The storage and treatment units were originally permitted and modified May 2, 1997, July 29, 2004, and June 22, 2006. The setbacks for storage and treatment units originally permitted or modified from June 1, 1996 to August 31, 2006 are as follows (all distances in feet): i. The setbacks for irrigation sites originally permitted or modified from September 1, 2006 to June 17, 2011 are as follows (all distances in feet): i. Surface waters classified SA: 100 ii. Site May: Provide an updated site map in accordance with 15A NCAC 02T .0105(d) that shows the following: 1. Legend, north arrow, and scale 2. Distance measurements verifying all setbacks are being met 3. Stormwater drainage controls 4. Location and ownership of property boundaries within 500 feet of the disposal area (including road/rail right-of-ways) 5. All facility -related structures and fences within the treatment, storage, and disposal areas 6. All habitable residences and places of assembly within 500 feet of all treatment, storage, and disposal sites 7. Topographic contour intervals not exceeding 10 feet or 25 percent of total site relief 8. Soil mapping units on all disposal sites 9. Location of all wells, streams (ephemeral, intermittent, and perennial), springs, lakes, ponds, ditches, and other surface drainage features within 500 feet of all waste treatment, storage, and disposal site(s) 10. Location and identification of each monitoring well (identify any background/upgradient wells) 11. Latitude and longitude coordinates of all monitoring wells (decimal degrees to 6 digits) 12. Location and identification of major components of the waste disposal system 13. Identification of all storage or treatment earthen structures that are part of the permit 14. The wetted perimeter of all irrigation/infiltration fields with field names (named according to the approved permit) 15. The delineation of compliance and review boundaries 16. 100-year floodplain (if applicable) D. Facility 1. Attachment B for the previous permit was missing the latitude and longitude coordinates for all fields. Provide central coordinates in NAD83 to six decimal degrees for all fields. 2. Provide information regarding any storage structure liner(s): type, full or partial, year installed.
