Mr. Lon Snider of the Winston-Salem Regional Office of the North Carolina Division of Water
Resources (DWR) conducted a compliance evaluation inspection of the Betsy -Jeff Penn 4-H Educational
Center waste water treatment plant (WWTP) on April 9, 2015. The assistance and cooperation of Mr. Jeffery
Johnson, operator in responsible charge (ORC), were greatly appreciated. Inspection findings are
summarized below and an inspection report is attached for your records.
The facility is located at 804 Cedar Lane in Reidsville, Rockingham County, North Carolina. The plant
consists of two septic tanks that operate in series, a filter chamber, recirculation and dosing tanks with a
pumping system, a recirculating fixed -media "BZ-Treat" system, ultraviolet disinfection, and effluent flow
measurement. Treated wastewater is discharged from the plant to Carroll Creek, which is currently classified
as Class C waters in the Roanoke River Basin.
Site Review
The facility was clean and secured. The treatment system was inspected and found to be well -
maintained and operational. Septic tanks are pumped about once per year by Billingsley. The tanks had just
been pumped a few days before the inspection. Records are kept in the maintenance office. No discrepancies
from the permit were noted.
The outfall was also inspected, no discharge was observed at the time of inspection due to the tanks
just being pumped. The effluent and receiving waters appeared clear and free of solids and foam. Access to
the discharge point was very well -maintained.
Documentation Review
Visitation and operation and maintenance (O&M) logs were reviewed and found to be complete and
current. Mr. Johnson keeps his O&M log on the computer and keeps an electronic backup copy of the file. A
printed copy was also available. All logs and records are kept for 20 years due to a requirement by the federal
government for summer camps for children.

Mr. Snider evaluated laboratory data during the inspection. Comparison of available lab reports and
field monitoring data with discharge monitoring reports (DMRs) showed no concerns or errors. Chain of
custody records were available and complete. All required non -field testing for the facility is conducted by
Pace Analytical Laboratories. Mr. Johnson is certified to perform field lab testing through the DWQ's
Laboratory Certification Branch.

Ms. Aana Taylor -Smith of the Winston-Salem Regional Office of the North Carolina Division of Water
Quality (DWQ) conducted a compliance evaluation inspection of the Betsy -Jeff Penn 4-H Educational Center
waste water treatment plant (WWTP) on 4 March 2013. The assistance and cooperation of Mr. Jeffery Johnson,
operator in responsible charge (ORC), were greatly appreciated. Inspection findings are summarized below and
an inspection report is attached for your records.

The purpose of this letter is to retract the January 8, 2013 Notice of Violation sent to you concerning the
October 2012 DMR for the Betsy -Jeff Penn 4-H Educational Center WWTP. The letter cited the failure to perform
twice -monthly monitoring for total nitrogen. Following the January 15, 2013 correspondence with Jeffrey Johnson,
it was discovered that Betsy -Jeff Penn 4-H Educational Center WWTP is not required to perform monitoring for
total nitrogen, but for ammonia nitrogen, which was performed as required in October 2012. No further action is
required on your part.
Our office apologizes for any inconvenience caused by the previous letter. If you have questions concerning

Added the NOV retracted event

Ran into a bit of a snag with an NOV I sent out (NOV-2013-MV-0002). For the October 2012 DMR, BIMS generated a
violation for Betsey Jeff Penn 4H Center for a parameter that they are not required to sample for. The violation in
question was a frequency violation for Total Nitrogen. As the ORC pointed out, their current permit does not require BJP
to sample for Total N, but for Ammonia Nitrogen. They did complete the required sampling for Ammonia Nitrogen in
October, so no violations should have been generated. If you wouldn't mind helping us out with that, I'd greatly
appreciate it! The permit number is NC0078271.

System Description

The influent enters the plant at a 13,000 gallon septic tank were most of the solids settle out.
From there it passes through a baffle wall into a filter chamber, there are two Polylok PL-525
effluent filters in the chamber. From there it flows into the first of two 8100 gallon
septic/recirculation tanks. Each tank has a pair of Polylok PL-525 effluent filters. After exiting
the second 8100 gallon Septic/recirculation tank it flows into a 8100 gallon filter dosing pump
tank. Up to this point every thing has been gravity feed.
The dossing pump tank has 3 sets of submersible duplex turbine pumps rated @ 52 gallons per
minute. Each set of pumps operates a zone of E-Z treat Pods, and each set is controlled by a
float level switches and a timer that controls the dossing time for each E-Z treat pod. The
pumps in each set alternate after each cycle of 4 minutes on 2 minutes off. There is a low limit
float that shuts off the pumps in case of low level conditions, and high level float that sets off
the alarm and flashing lights at the control panel.
The E-Z treat pods are 7feet 9inch square and 34 in deep, they contain a lightweight, non -
biodegradable synthetic sphere in three different diameters encapsulated in a mattress 7 % feet
square by 2 feet thick. Each pod has a spray manifold system containing 12 - .0625 diameter
brass nozzles that maintains a consistent misting spray. The effluent flows through the mattress
coming in contact with the media as it exits the mattress. It flows through 7 large 4 inch under
drains, and then flows to the recirculation tank to start the cycle over again.
There are three zones of pods. Zone one and two have eight, E-Z treat treatment pods, and
zone three has four E-Z treat treatment pods. 2/3 of the influent from each zone is returned to
the first septic/recirculation tank were it returns through the system again. The other 3rd is
sent back to the pump tank through a cross over ball float valve at which time it returns to the
pump tank for recirculation through the system. When the effluent level in the pump tank
reaches an elevation of 671.07' the ball valve closes and the effluent is sent to the discharge
tank.
The control panel contains three set of Ohio Electric electronic panels. This is a 120/240 volt
200 amp system with a heavy duty transfer switch for back up generation. There is a 15000
watt Guardian Ultra Source generator on site for emergency stand by. This is all protected by a
weather proof shelter.

The discharge tank is a1350 gallon tank with one 60 gallon per minute pump controlled by a
float switch and timer. This tank also has a high level float that if activated will set off a alarm
and flashing light at the control panel. The flow meter for the system is in this tank. It is a 2"
brass ABB T3000 cold water turbine meter numerical display.
Once the float is activated in the discharge tank the effluent goes through a SIMITECH STF-
104/600 micron poly -knit filter sock, from here it enters the Trojan UV2000 ultraviolet
disinfection system. The discharge from the UV system goes into a 2'X2' catch basin and down a
4" schedule 40 PVC pipe to Carroll Creek.
The Betsy -Jeff Penn 4-H Educational Center has a NPDES permit that allows treated wastewater
from the camp to be discharged into Carroll Creek. The current permit, number NC0078271,
allows a maximum discharge of 13100 gallons per day and has the following limits on the
quality of the discharge.

Quality conducted a compliance evaluation inspection of the Betsy -Jeff Penn 4-H Education Center
Waste Water Treatment Plant (WWTP) on 29 March 2011. Your assistance and cooperation was
greatly appreciated. Inspection findings are summarized below.
The facility is located at 804 Cedar Lane, in Reidsville, in Rockingham County, North Carolina.
The plant currently consists of two septic tanks that operate in series, a pump tank with a pumping
system, a proprietary recirculating fixed -media filter known as an "EZ Treat" system, ultraviolet
disinfection and effluent flow measurement. Treated wastewater is discharged from the plant to
Carroll Creek, which is currently classified as Class C waters in the Roanoke River basin.
Site Review
The facility was clean and well secured. The treatment system was inspected and found to be
operational. No discrepancies from the permit were noted. The outfall was also inspected and there
was only a small amount of discharge at the time of inspection. The discharge point appeared free of
pollutant indicators and was in very good condition. Access to the discharge point was well
maintained.
Documentation Review
Mr. Thomas evaluated laboratory data during the inspection. Operation logs were reviewed and
found to meet all requirements. Comparison of available lab reports and field monitoring data with
discharge monitoring reports (DMR) showed no concerns or errors. Chain of custody records are
available and complete. The visitation and operation/maintenance logs were reviewed and found to be

In accordance with the provisions of the North Carolina Administrative Code 15A NCAC 2H .0805 (g) (7), all
field parameter certified laboratories are required to participate in a Performance Evaluation (PE) study at least
annually to maintain certification. Field parameter laboratories must also analyze follow-up PE samples for any
parameters graded unacceptable. Failure to complete the required PE studies and/or follow-up PE results within
the allotted time frames may result in enforcement action in accordance with 15A NCAC 02H .0805 (g) (8).
The following information is provided to enable your facility to complete the North Carolina
Wastewater/Groundwater Laboratory Certification (NC WW/GW LC) performance evaluation for the year 2009
in the required timeframe.
PE (also known as Proficiency Testine or PT) Schedule
The 2009 calendar year PE schedule for the NC WW/GW LC program will end on October 31, 2009. NOTE:
We must receive the graded results from the vendor in this office by the October 31, 2009 deadline. Be
cautioned against using the October studies because the results may not be in our office before the October
31, 2009 deadline. If not performing a quick turnaround type sample, September studies will likely be the
latest monthly study that will meet the deadline.
PE Study Guidance
Attached is a list of parameters with the corresponding method codes and technologies. It is the facility's
responsibility to verify that a PE sample, for all certified parameters where available, is analyzed during the time
frame outlined. There are no PE samples available for dissolved oxygen (DO) and temperature. Late results
cannot be used for certification purposes. A PE sample must be analyzed by each technology for a particular
parameter for which your laboratory holds certification. Example: total residual chlorine by colorimetric (DPD)
has five different method codes included for the colorimetric technology; only one result is required for
colorimetric analysis. Double-check the method codes listed for each analysis; the entire method reference should
be listed to ensure proper credit.
The PE samples that you order must be "blind" or "unknown" values, and the results must be submitted to an
accredited vendor for grading purposes. Graded results must be sent directly to NC WW/GW LC by the
accredited vendor. PE samples received from vendors providing "sealed envelopes" with the correct results
enclosed (often called "Corrective Action", "QA" or "QC" samples) may not to be used for certification purposes.
The NC WW/GW LC program will accept quick turn -around type PE samples. Accredited vendors can be found
at the following website: http://h2o.enr.state.nc.us/lab/cert.htm or you may go directly to the A2LA website at
http://www.a2la.org[Rt/ProficienaTesting.c . The list of accredited vendors may change and should be checked
each time performance evaluation samples are ordered.
Any PE sample results analyzed outside the DMR-QA package must be reported by the accredited vendor to the
NC WW/GW LC Program on or before October 31, 2009. Many vendors run concurrent DMRQA/WPCHEM
studies each year. Follow US EPA and vendor instructions for DMR-QA Study 29 PE sample results deadlines.
Failure to report the required PE studies by October 31, 2009 or by the DAM-QA Study 29 deadline will be
considered as a first unacceptable result for certification purposes.
The NC WW/GW LC address is 1623 Mail Service Center, Raleigh, NC 27699-1623. When completing the
reporting forms, please ensure that the correct address is clearly identified for results to be reported to this office.
The laboratory's EPA Lab Code and the State ID number (this may be the lab certification number) must also be
included to ensure proper credit.
Additional parameter -specific PE requirements:
For colorimetric (DPD) procedures, Total Residual Chlorine (TRC) PE samples must be analyzed on the same
spectrophotometric program (i.e., the same wavelength) using the same procedure that is used for sample analysis.
There are two options for achieving this:
If a regular level TRC performance evaluation sample is analyzed, it must be diluted to the
verified range of the low level curve. The reported result must then be calculated using the
dilution factor and the TRC value obtained.
2. Since the dilution factor in option 1 may introduce error, it is recommended that a low level TRC
sample be analyzed. This sample should be within the range of your verified curve on the low
level program.
Some vendors include a low-level TRC PE in a Water Pollution (WP) study that can be used for the DMR-QA
study.
DMR-OA Corrective Actions
US EPA's DMRQA Study 29 for selected NPDES permittees is anticipated to begin May 1, 2009 with ungraded
data packets due in before August 28, 2009. Follow the instructions you received in the US EPA DMRQA Study
29 instruction package for participating NPDES permit holders. Corrective action plans for DMR-QA studies are
to be submitted to the NC WW/GW LC Program State Coordinator, Mr. Pat Donnelly, by the date indicated by
the US EPA DMR-QA 29 instructions (probably December 4, 2009). Selected DMRQA study 29 NPDES
permittees should receive packages from EPA in May..
Follow-up PE samples to unacceptable results
NC WW/GW LC will allow the facilities 60 DAYS from the date the results are issued by the accredited
vendors to complete and have vendors submit to NC WW/GW LC all follow-up PE results for any unacceptable
results.
Unacceptable results are defined by 15A NCAC 2H .0803 (15) as follows:
"Unacceptable results" means those results on performance evaluation samples that exceed the specified
acceptable range as indicated by a US EPA accredited vendor.

Mr. Ron Boone of the Winston-Salem Regional Office (WSRO) of the NC Division of Water Quality (DWQ
or Division) conducted a compliance evaluation inspection (CEI) of the Betsy Jeff Penn 4-H Educational
Center wastewater treatment plant (WWTP) on 24 February 2009. The assistance and cooperation of Mr.
Jeff Johnson, Operator in Responsible Charge (ORC), was greatly appreciated. Inspection findings are
summarized below and an inspection report is attached for your records.
The facility is located at 804 Cedar Lane, in Reidsville, Rockingham County, North Carolina. The plant
currently consists of two septic tanks that operate in series, a pump or dosing tank with pumping system, a
proprietary recirculating fixed -media filter known as an "EZ Treat" system, ultraviolet disinfection and
effluent flow .measurement. Treated wastewater is discharged from the plant to Carroll Creek, which is
currently classified as Class C waters in the Roanoke River basin. Please be sure to update the permit's
plant description during the next permit renewal cycle; the description currently only indicates one septic
tank at the plant and does not list the two pump stations, i.e. the EZ Treat dosing station and the effluent
station.
SITE REVIEW
3. The entire system appears to be very well maintained. No operational or maintenance issues were noted
during the inspection. Mr. Johnson stated that the septic tanks were being pumped about once per year,
but that recent monitoring of sludge levels in the tanks revealed very little sludge. Records of sludge
pumping are kept at the main offices of NC State University.
4. The plant was not discharging during the inspection. The outfall and receiving stream were evaluated.
There was no foam, floating solids or other signs of detrimental impact of the discharge to the receiving
stream. The right-of-way to the stream is well maintained.
DOCUMENTATION REVIEW
Mr. Johnson's operator visitation records and operation/maintenance log were reviewed and found to be
complete and current. Mr. Johnson keeps his O&M log on the computer and also keeps an electronic
backup of the file. It was suggested that he also print the O&M log as a second backup and just to have a
hard copy of it available. Please note that, although this is the ORC's responsibility, the University is
ultimately responsible for all such records. Should the operation of the facility change to another operator
at any time in the future, the University must retain all O&M records.

6. Discharge monitoring reports (DMR) were reviewed and compared with laboratory reports. No mistakes or
transcription errors were noted; however the electronic spreadsheet that Mr. Johnson is using for his DMRs
is set to automatically round off some of the numbers that he enters on the spreadsheet. The analytical
results must be reported to the level required by the permit. Mr. Johnson stated that he would seek out
assistance from someone else at the center to set up the spreadsheet correctly so entries are not rounded
off and lab results are reported at the required level.
Pace Analytical Laboratories performs non -field tests and Mr. Johnson performs all other field lab tests, i.e.
currently pH and temperature. All lab data, including chains of custody, was complete and current.
However, neither Mr. Johnson nor the center is currently certified to perform the field lab testing. You must
obtain the necessary certification from DWQ's Laboratory Certification Branch. Please visit their website at
http://h2o.enr.state.nc.us/labs.html to download the appropriate application and contact the personnel
necessary to initiate the certification process.
8. There is currently no backup ORC (BORC) listed for the facility. Mr. Johnson stated that his assistant, Mr.
Pete Pullian, is testing for his grade I license in March and if he receives his certification, will be assigned
as the BORC. To do so, please download the Water Pollution Control System ORC Designation Form from
the DWQ Technical Assistance and Certification Unit (TACU) website at:
http://h2o.enr.state.nc.us/tacu/index.htm1. Be sure to follow all instructions on the form. When you submit
the form, please also copy our office so that we are aware of your submission. Please note that the
BORC position should already be assigned a qualified operator.

This is to inform you that the Division of Water Quality received your monthly monitoring report
for April 2008 on July 2, 2008. This is in violation of Part II, Condition D(2) of the NPDES permit, as
well as 15A NCAC 2B .506(a), which requires the submittal of Discharge Monitoring Reports no later
than the thirtieth (3ffh) day following the reporting period. Failure to submit reports as required will
subject the violator to the assessment of a civil penalty of up to $25,000 per violation.
The Division must take these steps because timely submittal of discharge monitoring reports is
essential to the efficient operation of our water quality programs. We appreciate your assistance in this

On behalf of Betsy -Jeff Penn 4-H Center, we have reviewed the Notice of Violation issued on
June 3, 2008. Measured pH values on March 19 and 26 were below the permitted minimum of 6
standard units. These values were likely due to the low flows experienced during this time
(0.0039 MGD and 0.0038 MGD). Low flows through the system will provide greater hydraulic
retention time of the wastewater. The increased retention time in the septic tanks will allow for
additional decomposition, which will allow for the release of amino acids and a lowering of the
pH of the wastewater. When the plant is seeing flows closer to the design flow, pH should not be
a problem.
To raise the pH, a burlap sack filled with soda ash was hung in the second septic/recirculation
tank (tank prior to the filter bed dosing tank). Through the month of April the pH has remained
within the permitted parameters. As another option to soda ash, limestone chips can be placed in
the treatment pods (in net bags) to raise pH.
After reviewing the monitoring reports for January through April of 2008, we believe that the
reported value of 600 #/100 mL for fecal coliform is a sampling or lab error. All of the other
reported values for fecal coliform from January through April are less than one.
