Smith/Camp Spring 76 — Groundwater Remediation Site Caswell County Dear Mr. Sparks: Donald R. van der Vaart Secretary A compliance Evaluation Inspection was performed of the groundwater remediation system at J.W. Smith/ Camp Spring 76 site in Caswell County on January 21, 2015 by Jenifer Carter of the Winston-Salem Regional Office. Mr. Danny Oldham, ORC, was present for the inspection. This type of inspection consists of two basic parts: a review of facility files and an on -site inspection of the system. Maintenance Records are updated regularly and were available for review. The remediation system was started during the visit and appeared to function properly. There was no discharge from the system during the inspection. Overall, the system appeared to be well -maintained and operated. Please review the enclosed inspection form for more detailed information. Phenol was identified at a concentration of 160 ppb in the remediation system effluent, which is slightly above the NPDES limit of 150 ppb. None of the other targeted compounds has been detected in the system effluent in May 2104. The remediation system will be shut down today and the carbon/aggregate media is scheduled to be replaced on May 29, 2014. The remediation system will be resampled during the week of June 1", 2014 and it will remain shut down until we receive the resampling analytical results and a confirmation that the phenol issue has been resolved. The NOV Letter was prepared based on a minor discharge permit exceedance which occurred in February 2012 at the JW Smith/Camp Springs `76 remediation system. Chronology of Events: The individual NPDES Permit was approved for the site to remediate the petroleum groundwater contamination required by the NCDENR UST Section for this high priority site. The remediation system pumps groundwater from three recovery wells and treats the water through an oil -water separator, air -stripper, and then carbon filtration. The following presents a bullet point list with dates of the activities surrounding the discharge exceedance. • On February 15, 2012, the system influent, midfluent, and effluent were sampled as part of normal operation & maintenance. Within the next week, the remediation system experienced and equipment fault (air stripper pressure switch failure) and the system automatically shut down. • On February 28, 2012, the laboratory results of the system discharge samples indicated a concentration of MTBE above the NPDES discharge limit. At that time the remediation system was not operational due to the broken air stripper pressure switch, and there was no discharge from the system. It was determined P.O. Mr. Guerra approved the request on March 14, 2012 in an e-mail. A copy of the email containing the request and Mr. Guerra's reply is enclosed. • In March 2012, a pre -approval for the conversion of ion exchange units, the exchange of carbon in two existing carbon vessels, and the remediation system repair was sent to the NCDENR UST Trust Fund. • The signed pre -approval was received in April 2012. • On May 3, 2012, the remediation system was repaired. • On May 8, 2012, the de -ionization cylinders were converted to carbon treatment vessels and the carbon vessels were exchanged with fresh carbon. These changes effectively doubled the carbon treatment capacity of the remediation system. • There was no discharge from the remediation system between February 28 and May 8, 2012. A copy of the March and April 2012 monthly DMRs are enclosed. Since the increase of the number of carbon vessels, the system discharge was sampled on May 24 and June 27, 2012. The May and June 2012 analytical results detected no presence of MTBE in the system discharge. Only phenol was identified at concentrations of 23 ppb and 7.5 ppb, which is well below the NPDES limit of 150 ppb. Copies of the May and June 2012 monthly NPDES reports are also attached. Pyramid/Sparks responded as quickly as humanly possible to halt the discharge and correct the system problems prior to any additional discharge. The corrective actions were effective and increased the system's capacity to keep this type of breakthrough from occurring in the future. We respectfully request that the Department consider the matter closed and that no permit fines be levied on Sparks Oil Company. Oil Water Separator and Product Storage Drum Free product from the OWS discharges by gravity to a 65 gallon polyethylene product storage drum installed outside of the system building. The product storage drum is equipped with a high- level switch to shut down operation of the remediation system. Residuals from the OWS and product storage drum are collected and handled by a contractor in accordance with all applicable regulations on an as needed basis. After the collection of the residuals, the contractor immediately hauls the residuals off the site. The contractor is also responsible for the proper disposal or storage/processing of residuals. Bag Filter The bag filter is changed every month during scheduled monthly maintenance events. The used bag filters are removed from the bag filter housing using disposable gloves and placed in a sealed plastic container. The used bags are immediately shipped as a non -hazardous waste for proper disposal. Pyramid is responsible for the collection, handling, and disposal of used bag filters. Aggregate Filter and Carbon Vessels The aggregate filter and carbon vessels are exchanged every six months by a contractor in accordance with all applicable regulation. Used aggregate filter and carbon vessels are immediately hauled off the site. The contractor is responsible for the residuals handling and proper disposal or storage/processing. Ion Exchange Vessels Ion exchange vessels are backwashed on an as needed basis. The backwash from the units are collected in a truck -mounted 320-gallon holding tank and transported off the site for proper disposal. Pyramid is responsible for the backwash collection, handling, and disposal. The used resin from ion exchange units is changed on an as needed basis by a contractor in accordance with all applicable regulation. The resin is immediately placed in sealed containers and removed from the site for processing and/or disposal. The contractor is responsible for proper disposal or storage/processing of the ion exchange residuals. Backwash Filter The used backwash filter media is collected and handled by a contractor in accordance with all applicable regulations on an as needed basis. After the collection of the used media, the contractor immediately removes the filter media from the site for processing and/or disposal. The groundwater remediation system was installed and completed in December 2009. The initial operation began on December 16, 2010. The initial discharge analyses showed that the acute toxicity testing passes, and that the chronic toxicity was within 15% of passing. Due to the failure of this test, the permit required re -analysis. In the second test, all organisms died, and the chronic test failed. In the third test, the organisms survived, but reproduction was very poor and the chronic test failed. After the three failing toxicity tests, the discussion was again opened up that toxicity testing may not be appropriate for this groundwater system. The laboratory personnel indicated that naturally occurring metals in groundwater appeared to be the reason for the toxicity test failures, not the contaminants of concern (petroleum hydrocarbons). Since this facility discharges to HQW, I'm assuming that's why we went with individual, and why the permit limits for toxicants were halved. I see potential problems trying to convert to GP coverage at this time. The NCG51 GW remediation GP expires 9/2011, and we can take another look at exclusions next year. I'd definitely be ok with removing the acute tox test, and leave the decision on chronic tox test to Matt/Cindy (leave in as limit, leave in as monitor -only, or remove). I know Cindy had concerns about the test conditions and reliability of tox test results for this facility. To remove a limit will require public notice. This permit expires May 2012. So far don't think they have talked. Actually not real sure a strategy is necessary. I think that I'm correct in that this is the ONLY petroleum remediation that even has a tox limit (much less two species). All others are GPs and have NO tox limit. I don't think the tox folks have a clue as to why the fails but in this particular situation it really does not matter. Bottom line the limit should never have gone into the permit. Just because it's a WSII (by definition only) was not reason to deviate from the GP scenario. Plus, in this situation the discharge is 1000' feet from a dry ditch and a mile to nearest stream. Bottom line there is no environmental concerns here. Not really a reason to spend lots of time on a strategy. This one could be handled by reissuing their permit as a GP as it should have been originally and rescinding the current individual permit and move on to bigger and better issues. Just an opinion. I would also suggest that no CPAs be issued relating to tox failures as long as all other parameters compliant. The permitting decision to include not one but two species for tox limits was flawed in my opinion and we need to clean this up. They are meeting other limits and as I said before I really do not think it's appropriate to put these Tox limits in a Ground water source of discharge. With his background in Tox and the fact he is the Section Chief and over permitting ....... that's where this needs to be resolved. Remember we are talking a discharge of 4 gallons per minute ......... to a ditch. Like any violation ........ you guys have the option to BPJ a violation or NOV. You do not HAVE to do CPA. The facility failed their chronic test in January, February and we just found out that March failed. Cindy suggested that they contact the WSRO for guidance. As of right now, I don't know if we should give them a CPA (next month's compliance), and want to get you involved in that decision. For some reason, Sergei Chernikov issued the permit with 2 tests each quarter. A fathead 24 hr P/F AND a Cerio 7 day P/F chronic. Susie Meadows questioned the need for 2 tests when the permit was reviewed. Matt agreed that 2 tests were not necessary, but Sergei felt that there was a need for 2 tests. Would you agree that 2 tests are not needed? Maybe drop the fathead test? Can you possibly advise the consultants as to which direction they should go to help identify their problem? In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement wastewater treatment or disposal facilities. The Winston-Salem Regional Office, telephone number (336) 771-5000, shall be notified at least forty-eight (48) hours in advance of operation of the installed facilities so that an on site inspection can be made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the NPDES Permit, this Authorization to Construct and the approved plans and specifications. Upon classification of the facility by the Certification Commission, the Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to or greater than the classification assigned to the wastewater treatment facilities by the Certification Commission. The Permittee must also employ a certified back-up operator of the appropriate type and grade to comply with the conditions of T15A:8G.0202. The ORC of the facility must visit each Class I facility at least weekly and each Class II, III and IV facility at least daily, excluding weekends and holidays, must properly manage the facility, must document daily operation and maintenance of the facility, and must comply with all other conditions of T1 SA:8G.0202. A copy of the approved plans and specifications shall be maintained on file by the Permittee for the life of the facility. During the construction of the proposed additions/modifications, the permittee shall continue to properly maintain and operate the existing wastewater treatment facilities at all times, and in such a manner, as necessary to comply with the effluent limits specified in the NPDES Permit. You are reminded that it is mandatory for the project to be constructed in accordance with the North Carolina Sedimentation Pollution Control Act, and when applicable, the North Carolina Dam Safety Act.
