However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on June 1, 2020. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through H, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Walker May 1, 2020 Page 2 of 4 A. Cover Letter: The existing wastewater treatment facilities are designed to treat a maximum of 30,000 gallons per day (GPD) of domestic wastes. This facility requires system modifications to increase the domestic waste treatment capacity to 80,000 GPD. Please supplement your application with a cover letter describing the current state of Seagrove-Ulah Metropolitan Water District's facilities, the intent for construction of Lagoon #1 (the 1,009,563 gallon synthetically lined flow equalization lagoon), and any proposed plans for modification to increase design flow. 2. Please include a brief description of the Permittee's renewal intent, requested permitting action(s), and timeline in your cover letter. B. Application Fee: If domestic wastewater flow above 30,000 GPD is requested, this constitutes a major modification. Major modification of a permit (i.e. modification to allow an increase in the volume of effluent) requires an application fee of $395. If you choose to propose major modification, please submit payment by check or money order, made payable to the North Carolina Department of Environmental Quality, or electronically through the Water Quality Permitting Section's ePayment option. C. Application: 1. If the proposed permitting action involves modification of the facility as part of the renewal, then a modification application (Form WWIS 06-16) must be completed and submitted. Your submittal of WWIS 06-16 should include all plans and specifications as well as the application fee noted above. 2. The application states (NDSR 01-20, II.2.) that the facility has been constructed according to Division approved plans and specifications. However, both treatment trains previously had bypass lines installed from each of the clarifiers directly into the 588,000-gal effluent storage pond resulting in sludge bypassing the sludge storage pit and being wasted directly into the storage pond. Additionally, the industrial influent lines appear to bypass disinfection. Please explain these deviations from division -approved plans and describe plans for any necessary water treatment facility modifications. D. Specifications: Please provide an updated and accurate description of the wastewater treatment facilities along with an updated set of plans. Updated plans should be clearly labeled, show flow directions within the treatment facilities, show all existing and proposed modifications (i.e. current work on the sludge storage pit lines), connection of the 2,005,265 gallon synthetically lined wet weather storage lagoon (Lagoon #2), proposed expansion of the disinfection system to accommodate treatment of 80,000 gallons per day, the re -plumbing of the old industrial influent lines, etc. 2. Please submit specifications for proposed modifications to the industrial influent lines. These should include at minimum: i. Table of contents with each section/page numbered. Mr. Michael T. Detailed specifications for each treatment/storage/irrigation unit, as well as all piping, valves, equipment (i.e., pumps, blowers, mixers, diffusers, flow meters, etc.), nozzles/emitters, precipitation/soil moisture sensor (if applicable), audible/visual high water alarms, liner material, etc. iii. Site Work (i.e., earthwork, clearing, grubbing, excavation, trenching, backfilling, compacting, fencing, seeding, etc.) iv. Materials (i.e., concrete, masonry, steel, painting, method of construction, etc.) v. Electrical (i.e., control panels, transfer switches, automatically activated standby power source, etc.) vi. Means for ensuring quality and integrity of the finished product, including leakage, pressure and liner testing. vii. Specifications shall represent a completed design and not be labeled with preliminary phrases (e.g., FOR REVIEW ONLY, NOT FOR CONSTRUCTION, etc.) that indicate they are anything other than final specifications. However, the specifications may be labeled with the phrase: FINAL DESIGN - NOT RELEASED FOR CONSTRUCTION. 3. Please provide calculations demonstrating that treatment of 80,000 gallons per day is attainable with specified modifications. 4. Based on the average flow during the last calendar year, please submit an engineering evaluation of future wastewater treatment, utilization, and disposal needs. The evaluation should include all current and presently non -tributary, obligated flows. The evaluation must provide justification that wastewater treatment needs will be met based on current records and future growth projections. If necessary, outline plans for meeting future wastewater treatment needs either by expansion of the existing system, or conservation measures to achieve waste flow reductions. 5. If, based on the future wastewater treatment capacity evaluation, modifications are necessary, please describe the source of funding for facility improvements. E. Site May: The permitted irrigation area is 40 acres, however not all 40 acres are irrigated during each application. An irrigation event on a subset of acres reported based on a 40-acre application area does not accurately reflect actual loading in the NDAR data. Therefore, a revised irrigation map is necessary. Please submit an updated map that shows all 16 irrigation lanes/zones. F. Groundwater Monitoring Wells: 1. Total Dissolved Solids have consistently been in excess of the 15A NCAC 02L standards at MW- 2 and MW-3. Please explain the reason for elevated TDS downgradient of the facility. 2. Due to the proximity of MW-2 and MW-3 to the compliance boundary, there is significant potential for violation of the groundwater standard for TDS beyond the compliance boundary and significant potential for impacts on Bear Creek. Violation of groundwater or surface water standards beyond the compliance boundary will require corrective action. Mr. Michael T. Walker May 1, 2020 Page 4 of 4 Please provide a mitigation plan to address the elevated levels of TDS. You may choose to include a groundwater model as means to quantify and justify the plan. G. Operation & Maintenance Plan: 1. Please provide an updated operation and maintenance plan. 2. The operation and maintenance plan should include the following minimum requirements: i. Operation of the wastewater treatment, storage and irrigation systems in sufficient detail to show what operations are necessary for the system to function and by whom the functions are to be conducted. ii. Anticipated maintenance of the wastewater treatment, storage and irrigation systems. iii. Safety measures, including restriction of access to the site and equipment. iv. Spill prevention provisions such as response to upsets and bypasses, including how to control, contain and remediate. v. Contact information for plant personnel, emergency responders and regulatory agencies. H. Residuals Management Plan: 1. Please provide a detailed explanation of the how residuals are to be collected, processed, stored, treated and disposed of. 2. Provide evaluation of the sludge pit storage requirements. A minimum of 30 days of residuals storage is required. 3. Please provide an updated plan for disposal of oil and grease, that includes the new businesses utilizing the wastewater treatment facility lines.
