revised impact maps and check # 4308 for the above referenced project. Modifications of already issued
projects require submission of a whole new application package and associated fee. The USACE would
dictate to you the permit type to request for 401 Water Quality Certification.
Please complete and submit five (5) application sets, complete with maps, associated project information
and the appropriate fee in order to initiate the review process for this latest version.
