The assistance and cooperation of Mr. Bobby Joyce, Operator in Responsible Charge (ORC), were greatly appreciated. Inspection findings are summarized below and an inspection report is attached for your records. The facility is located on Reid Lake Road, in Reidsville, Rockingham County, North Carolina. The treated wastewater is discharged from the WTP to an unnamed tributary of Troublesome Creek, which is currently classified as Water Supply II- Nutrient Sensitive waters in the Cape Fear River Basin. Site Review The facility was clean and well secured. The treatment system was inspected and found to be operational. No discrepancies from the permit were noted. The outfall was not inspected as there needs to be a clear path to the outfall point. There was no flow at the time as we checked the outgoing discharge box. Documentation Review Mr. Lon Snider evaluated laboratory data during the inspection. Comparison of lab reports and field monitoring data with discharge monitoring reports (DMRs) showed no concerns or errors. Chain of custody records were available and complete. Reidsville Lake WTP discharges into an upper tributary of Troublesome Creek. Whole effluent samples were collected on April 1 and April 3 by Jenifer Carter and facility representative C. Scott Jewell for use in a chronic Ceriodaphnia dubia pass/fail toxicity test. The test using these samples resulted in a fail. Toxicity information follows. The problem area of the feed line is located 15 inches underground at the front entrance to the chemical feed building and is used to transfer caustic from the day tank to the various feed points in the water treatment plant. This caustic transfer chemical pipe contains caustic and transfer water. It is believed that the feed line was broken due to heavy rainfall over the past month which caused some shifting of the soil and extra pressure on the pipe. It is believed that the caustic followed the path of less resistance and flowed underneath the chemical building, up and out from the back of the building. The caustic then flowed down an embankment to a heavily vegetative area which is located approximately 60 yards away from the chemical building. Just beyond this area is the Little Troublesome Creek. Samples of the liquid leak was collected and tested for pH. The pH levels ranged from 11.0 SU to 13.0 SU. The chemical feed line was repaired on 12/30/2013. A follow-up inspection was completed on 12/31/2013 and the area where the caustic was following under the building had stopped. A visual inspection of the Creek was made and not signs of impact were observed. We estimated the caustic leak to be approximately 1,300 gallons over an estimated 45-day period. This is based on a 25% loss of the chemical feed rate. Thank you so much for talking with your supervisor about this. It is my understanding that there is nothing in our NPDES Permit that would prevent the disposal of the fluoride in the treatment basin although the State would prefer that it not be. Currently, the sludge is being removed from the treatment lagoons and they are segregated so that water is discharged from only one of the treatment basins. The fluoride will be metered into the off-line sludge lagoon. There should be very little fluoride discharged under the NPDES Permit. We will increase the monitoring frequency for fluoride to insure the water quality standard for fluoride is not exceeded. If you have any questions or concerns, please do not hesitate to let me know. The assistance and cooperation of Mr. Gary Moore, Operator in Responsible Charge (ORC) was greatly appreciated. Inspection findings are summarized below. The facility is located on Reid Lake Road, in Reidsville, in Rockingham County. Wastewater produced at the facility is discharged to an Unnamed Tributary of Troublesome Creek, which is currently classified as Waters Supply II - Nutrient Sensitive Waters in the Cape Fear River Basin. Site Review The facility is very well operated and maintained. All treatment units were online and functioning properly at the time of inspection with the exception of the de -chlorination unit. Mr. Moore informed Mr. Thomas that the de - chlorination unit had been struck by lightning recently. Staff at the facility are adding de -chlorination tablets manually while repairs are being made Documentation Review Mr. Thomas evaluated laboratory data during the inspection. Operation logs were reviewed and found to meet all requirements. Laboratory records for both the WWTP lab and the contract lab were reviewed and found to be accurate and complete. Merritech and Environment 1 process the samples not run by the facility. Laboratory chains of custody were complete and well organized. Operation and maintenance records are complete and well organized. In addition, the operator visitation log was reviewed and found to be complete and current. Reidsville, NC 27320 (336) 349-1042 I. Description of Collection System or Treatment Process: The wastewater discharged by the Reidsville Water Plant is generated by the filter backwash operation. There are three treatment lagoons used for settling. The lagoon that discharges through Pipe 001 is an emergency outfall for settling water before it gets back to the lake. Currently, this lagoon and outfall is not used but is still included in the NPDES Permit as a permitted outfall. Currently, the filter backwash water is settled in two lagoons and discharges through Pipe 002. The settled water is discharged from either one or both lagoons by use of telescopic valves located in each of the two lagoons. The wastewater travels through the valves and empties into a wet well where it is pumped out by one of two submersible pumps. The wastewater is dechlorinated by the addition of sodium bisulfite prior to discharing to Little Troublesome Creek. Sludge which has settled from the wastewater is pumped out of the lagoons by a floating dredge. While the slurry is being pumped through the dredge, polymer is injected in the line to the sludge slurry and the dewatered sludge is deposited in the sand drying bed located just above the sludge lagoons. The sludge is dried to a dense cake and hauled away. II. Compliance Performance The North Carolina Department of Environment and Natural Resources (NCDENR) regulates the City of Reidsville's effluent discharge under the National Pollutant Discharge Elimination System (NPDES). The NCDENR issued to the City a NPDES Permit, which includes water quality limits and sampling and monitoring requirements. The NPDES Permit requires the City to test more than 6 different constituents in the treated water. The monitoring frequency for these constituents are set at various intervals such as continuous, daily, weekly, monthly, and quarterly. During the previous year, the City conducted 340 tests of the treated water before it was discharged to the creek. The WTP achieved a compliance level of 100% with the NPDES Permit requirements. Following is a summary of the testing for the parameters, which are assigned Water Quality standards by the NPDES Permit. Please note the summary does not include parameters, which incorporates a monitoring requirement only, nor does it include daily process control testing. The unit processes in the treatment scheme include preliminary treatment, activated sludge biological treatment with the chemical addition of Powdered Activated Carbon, and secondary clarification followed by tertiary filtration. The treated effluent is chlorinated/dechlorinated and is discharged to the Haw River in the Cape Fear River Basin. Aerobic digestion is provided to biologically stabilize the excess sludge. The stabilized liquid sludge is land applied to approved application sites or may be dewatered on sand drying beds. II. The system consists of approximately 168 miles of gravity pipe ranging from size from 4 to 30 inches in diameter with 8" lines making up about 80% of the total. The system also contains 16 sewer pump stations and 53,675 ft of force mains. The system collects sewage from throughout the City and transports it to the City's Wastewater Treatment Plant where it is treated and ultimately released back to the Haw River. III. Compliance Performance The wastewater collection system had 2 sewer spills in 2010. Following is a summary of those events. July 9, 2010 The City of Reidsville had an untreated wastewater discharge from its collection system in the vicinity of 709 Russell Avenue. The calculated discharge of approximately 600 gallons lasted approximately 4 hours and ended July 9, 2010.The spill was caused by rags/adult diapers that we believe are coming from the Annie Penn Hospital or Avante Nursing Home. Both facilities have been spoken to about this problem. Approximately 400 gallons of the discharge reached a ditch that leads to an unnamed tributary of Little Troublesome Creek in the Cape Fear River Basin. As much of the spill as possible was vacuumed up using the combination jet/vacuum truck. July 26, 2010 The City of Reidsville had an untreated wastewater discharge from its collection system in the vicinity of 1821 South Park Drive. The calculated discharge of approximately 600 gallons lasted approximately 1 hour and ended July 26, 2010. The spill was caused by grease. Approximately 400 gallons of the discharge reached Little Troublesome Creek in the Cape Fear River Basin. The creek was dammed to contain the spill. NC0046345 NARRATIVE DESCRIPTION OF DIAGRAM Raw water is pumped from Lake Reidsville to the sedimentation basin, where chemical addition occurs for solids settling. After sedimentation, the water is chlorinated and filtered. Following filtration, additional chemicals are added to the water and then stored in a clear well. The assistance and cooperation of Mr. Gary Moore, Operator in Responsible Charge (ORC), was greatly appreciated. Inspection findings are summarized below and an inspection report is attached for your records. There are currently two outfalls regulated by this permit for the Reidsville WTP. Outfall 001 discharges directly into Lake Reidsville, which is currently classified as WS-III (water supply), CA (Critical Area), NSW (Nutrient Sensitive Water), in the Cape Fear River Basin. Outfall 002 discharges to Troublesome Creek, which is currently classified as a WS-III, NSW in the Cape Fear River Basin. According to Mr. Moore, wastewater is rarely discharged from 001, as all wastewater produced at the facility drains to the lagoons to settle and is then discharged from outfall 002. Documentation Review Mr. Boone evaluated laboratory data and discharge monitoring reports from March and June 2010. Comparison of lab data and reports with discharge monitoring reports (DMR) submitted for those months revealed no mistakes or transcription errors. Chains of custody were complete. All lab records were well organized. The lab's certification for field laboratory tests is current. Mr. Moore's operation and visitation logs were complete and current. Records of plant operations and maintenance were also available. All records must be maintained for a period of three years, except laboratory records, which must be maintained for five years. Reidsville, NC 27320 (336) 349-1042 Description of Collection System or Treatment Process: The wastewater discharged by the Reidsville Water Plant is generated by the filter backwash operation. There are three treatment lagoons used for settling. The lagoon that discharges through Pipe 001 is an emergency outfall for settling water before it gets back to the lake. Currently, this lagoon and outfall is not used but is still included in the NPDES Permit as a permitted outfall. Currently, the filter backwash water is settled in two lagoons and discharges through Pipe 002. The settled water is discharged from either one or both lagoons by use of telescopic valves located in each of the two lagoons. The wastewater travels through the valves and empties into a wet well where it is pumped out by one of two submersible pumps. The wastewater is dechlorinated by the addition of sodium thiosulfate (BSP Captor) prior to discharing to Little Troublesome Creek. Sludge which has settled from the wastewater is pumped out of the lagoons by a floating dredge. While the slurry is being pumped through the dredge, polymer is injected in the line to the sludge slurry and the dewatered sludge is deposited in the sand drying bed located just above the sludge lagoons. The sludge is dried to a dense cake and hauled away. II. Compliance Performance The North Carolina Department of Environment and Natural Resources (NCDENR) regulates the City of Reidsville's effluent discharge under the National Pollutant Discharge Elimination System (NPDES). The NCDENR issued to the City a NPDES Permit, which includes water quality limits and sampling and monitoring requirements. The NPDES Permit requires the City to test more than 6 different constituents in the treated water. The monitoring ppppppp- frequency for these constituents are set at various intervals such as continuous, daily, weekly, monthly, and quarterly. During the previous year, the City conducted 346 tests of the treated water before it was discharged to the creek. The WTP achieved a compliance level of 100% with the NPDES Permit requirements. Following is a summary of the testing for the parameters, which are assigned Water Quality standards by the NPDES Permit. Please note the summary does not include parameters, which incorporates a monitoring requirement only, nor does it include daily process control testing. CONCLUSIONS: Correcting the above -cited findings will help this lab to produce quality data and meet certification requirements. DENR - WATER QUALITY POINT SOURCE BRANCH prepared: February 2009 2008 Annual Performance Report for the City of Reidsville Water Treatment Facility I. General Information Facility/System Name: City of Reidsville Water Treatment Responsible Entity: City of Reidsville Person in Charge/Contact: Charlie Hamilton Public Works Director 230 W. Reidsville, NC 27320 (336) 349-1042 Description of Collection System or Treatment Process: The wastewater discharged by the Reidsville Water Plant is generated by the filter backwash operation. The filter backwash is deposited into the lagoon labeled 001 and located between the front of the water filtration plant and the raw water source (Lake Reidsville). The suspended contaminants are allowed to settle out and are removed physically from the bottom of the lagoon with the aid of a track hoe or some other type of digging device. The sedimentation is transported to the City's landfill. Also the rewash from the filters are diverted to this outfall and treated in the same manner. We have a second outfall on site. This outfall (002) will be the only remaining outfall at the plant. It is located at the back of the plant. It consists of two 100 feet by 40 feet lagoons that are interconnected via one forty-eight inch cement pipe. This lagoon collects water and solids that are washed out of the hopper valves located in each of the three sedimentation basins. During routine cleaning of the hoppers and quarterly cleaning of the entire basins, water and suspended solids are emitted to the lagoons. The solids are settled and clear water is drained off the top of the lagoon on occasion. During the fall, spring, and summer months, sludge is processed and pumped on the large sand bed or one of the two wedgewater beds. Drying takes place and the sludge is hauled away. The sludge must be of a consistency so that the solid will not seep out of cracks in the beds of the vehicle transporting the processed sludge. The supernated flow is calculated by physically measuring the amount of time a gallon container is filled and multiplying the amount of time the flow is allowed to continue. F_ II. Compliance Performance The North Carolina Department of Environment and Natural Resources (NCDENR) regulates the City of Reidsville's effluent discharge under the National Pollutant Discharge Elimination System (NPDES). The NCDENR issued to the City a NPDES Permit, which includes water quality limits and sampling and monitoring requirements. The NPDES Permit requires the City to test more than 6 different constituents in the treated water. The monitoring frequency for these constituents are set at various intervals such as continuous, daily, weekly, monthly, and quarterly. During the previous year, the City conducted 330 tests of the treated water before it was discharged to the creek. The WTP achieved a compliance level of 100% with the NPDES Permit requirements. Sullins, Director Division of Water Quality City of Reidsville Attn: Kelly Almond, City Manager 230 West Morehead Street Reidsville, NC 27320 September 3rd, 2008 Subject: Compliance Evaluation Inspection NPDES Individual Wastewater Discharge Permit — NCO046345 Reidsville Water Treatment Plant Rockingham County Dear Mr. Almond: On August 27th, 2008, Mr. Ron Boone and Mr. Steve Mauney of the Winston-Salem Regional Office of the Division of Water Quality (DWQ) conducted a compliance evaluation inspection (CEI) of the City of Reidsville's water treatment plant (WTP), located on Reids Lake Road (NC state road 2445), just west of US Highway 29, in Reidsville, Rockingham County, North Carolina. The assistance and cooperation of Mr. Gary Moore, Operator in Responsible Charge (ORC), was greatly appreciated. Inspection findings are summarized below and an inspection report is attached for your records. There are currently two outfalls regulated by this permit for the Reidsville WTP. Outfall 001 discharges directly into Lake Reidsville, which is currently classified as WS-III (water supply), CA (Critical Area), NSW (Nutrient Sensitive Water), in the Cape Fear River Basin. Outfall 002 discharges to Troublesome Creek, which is currently classified as a WS-III, NSW in the Cape Fear river basin. According to Mr. Moore, wastewater is rarely discharged from 001, as all wastewater produced at the facility drains to the lagoons to settle and is then discharged from outfall 002. Documentation Review 3. Mr. Boone evaluated laboratory data from February and May 2008. Comparison of lab bench data and reports with discharge monitoring reports (DMR) submitted for those months revealed no mistakes or transcription errors. Chains of custody were complete. All lab records were well organized. The lab's certification for field laboratory tests is current. You are reminded that laboratory records must be maintained for a minimum of five years. The only discrepancy noted was the frequency of testing for Total Suspended Solids (TSS). The permit requires testing for TSS twice per month with at least 10 calendar days between sampling events. There were several occasions during which these samples were taken with less than 10 calendar days between sampling events. Mr. Moore is now aware of the requirement but please ensure that a span of at least 10 calendar days is maintained between the two monthly TSS samples. 4. Mr. Moore has daily sign -in logs that were complete and current. Records of plant operations and maintenance were also available, complete and current. All records aside from laboratory records must be maintained for a period of three years. Site Review The lagoons are currently being upgraded and a dechlorination system is being installed in order to meet the new permit Total Residual Chlorine (TRC) limit of 17 micrograms per liter, which became effective on September 1st, 2008. 6. During the inspection, a hose on the ground indicated that water was pumped from the polymer pond into an adjacent field. Reidsville, NC 27__320 (336) 349-1042 Description of Collection System or Treatment Process: The wastewater discharged by the Reidsville Water Plant is generated by the filter backwash operation. The filter backwash is deposited into the lagoon labeled 001 and located between the front of the water filtration plant and the raw water source (Lake Reidsville). The suspended contaminants are allowed to settle out and are removed physically from the bottom of.the lagoon with the aid of a track hoe or some other type of digging device. The sedimentation is transported to the City's landfill. Also the rewash from the filters are diverted to this outfall and treated in the same manner. We have a second outfall on site. This outfall (002) will be the only remaining outfall at the plant. It is located at the back of the plant. It consists of two 100 feet by 40 feet lagoons that are interconnected via one forty-eight inch cement pipe. This lagoon collects water and solids that are washed out of the hopper valves located in each of the three sedimentation basins. During routine cleaning of the hoppers and quarterly cleaning of the entire basins, water and suspended solids are emitted to the lagoons. The solids are settled and clear water is drained off the top of the lagoon on occasion. During the fall, spring, and summer months, sludge is processed and pumped on the large sand bed or one of the two wedgewater beds. Drying takes place and the sludge is hauled away. The sludge must be of a consistency so that the solid will not seep out of cracks in the beds of the vehicle transporting the processed sludge. The supernated flow is calculated by physically measuring the amount of time a gallon container is filled and multiplying the amount of time the flow is allowed to continue. Il. Compliance Performance The North Carolina Department of Environment and Natural Resources (NCDENR) regulates the City of Reidsville's effluent discharge under the National Pollutant Discharge Elimination System (NPDES). The NCDENR issued to the City a NPDES Permit, which includes water quality limits and sampling and monitoring requirements. The NPDES Permit requires the City to test more than 6 different constituents in the treated water. The monitoring frequency for these constituents are set at various intervals such as continuous, daily, weekly, monthly, and quarterly. During the previous year, the City conducted 388 tests of the treated water before it was discharged to the creek. The WTP achieved a compliance level of 100% with the NPDES Permit requirements. Following is a summary of the testing for the parameters, which are assigned Water Quality standards by the NPDES Permit. Please note the summary does not include parameters, which incorporates a monitoring requirement only, nor does it include daily process control testing.
