A compliance Evaluation Inspection was performed at South Elementary School's well water treatment
system on December 15, 2014 by Jenifer Carter of the Winston-Salem Regional Office. Matthew Smith,
ORC, was present for the inspection. This type of inspection consists of two basic parts: a review of
facility files and self -monitoring data, and an on -site inspection.
There were no violations noted during the inspection. Lab data correlated correctly with data submitted on
the monthly Discharge Monitoring Reports. Maintenance and ORC visitation logs were available for
review and complete.

A compliance Evaluation Inspection was performed at South Elementary School's well water treatment system
on December 18, 2013 by Jenifer Carter of the Winston-Salem Regional Office. Matthew Smith, ORC, was
present for the inspection. This type of inspection consists of two basic parts: a review of facility files and self -
monitoring data, and an on -site inspection.
There were no violations noted during the inspection. Lab data correlated correctly with data submitted on the
monthly Discharge Monitoring Reports. Maintenance and ORC visitation logs were available for review and
complete.
Mr. Smith noted during the inspection that flow is measured with a flanged flow meter that cannot be calibrated
without dismantling the piping. It is not necessary to check the calibration since there no flow limit in the
permit.

The Division understands that you have made no significant alterations to your potable water waste -treatment
plant (WTP) facilities during the previous permit cycle except to add dechlorination (ATC May 4, 2009). For
renewal we updated your permit, applying the latest permitting strategy for WTPs statewide specific to green -
sand filter treatment technology (adopted September 2009). This strategy includes a revised list of monitoring
requirements, permit limits, and effluent parameters of concern [see permit Section A. (L)].
Following the mandatory 30-day public comment period, the Division will review all pertinent comments and
take appropriate action prior to issuing your NPDES permit final. If you have questions concerning this draft,

effluent page to reflect the Water Treatment Plant (WTP) Permitting procedures 2009
Total Iron, Total Zinc, and Total Manganese data are available for Reasonable Potential Analyses (RPA) — all
three show reasonable potential to exceed respective water -quality standards (see attached RPA). Total
Manganese monitoring is required for greensand technology, but it remains as "monitoring only, no limit."
Although Total Manganese past data do show RP to exceed the Human Health standard of 200µg/L (MAX
predicted = 4,032µg/L), this standard applies to Class WS waters only. There are no other parameters of
concern.
Toxici - Green sand filter systems do not require Whole Effluent Toxicity (WET) testing.
Compliance - This facility has an excellent compliance record (see attached).

A compliance Evaluation Inspection was performed at South Elementary School's well water treatment
system on January 25, 2012 by Jenifer Carter of the Winston-Salem Regional Office. Mr. Glen Price,
ORC, was present for the inspection. This type of inspection consists of two basic parts: a review of
facility files and self -monitoring data, and an on -site inspection.
There were no violations noted during the inspection. Lab data correlated correctly with data submitted on
the monthly Discharge Monitoring Reports. Maintenance and ORC visitation logs were available for
review and complete. The dechlorination system and discharge pipe seemed to be well -maintained and
functioning properly.

SUBJECT: Compliance Evaluation Inspection — NPDES Permit No. NCO086983
South Elementary School WTP — Caswell County
Dear Dr. Barker:
Jenny Graznak of this office performed a Compliance Evaluation Inspection at the South
Elementary School water treatment filter backwash discharge in Caswell County on February 3,
2011. Steven Yarbrough, Operator in Responsible Charge (ORC), was present for the
inspection. This type of inspection consists of two basic parts: an in -office review of facility files
and self -monitoring data and an on -site inspection of the facility. This particular inspection
evaluated eight (8) areas, and observations from each area are addressed below:
1. Permit
The permit became effective October 1, 2007 and expires March 31, 2012.
2. Self -Monitoring Program
A review of the monthly self -monitoring reports from January 2010 through December 2010
showed one violation of the permit limit for Total Residual Chlorine that occurred in February
2010. This violation was already addressed by a previous correspondence from our office.
3. Flow Measurement
The flow from the filter backwash system is measured by a Hershey flow meter. The operators
calibrate the meter on an annual basis. Verification (date, name, etc.) of the calibration and the
method used is maintained with plant records. The flow meter was calibrated in August 2009
before the students returned to school from the summer break.
4. Facility Site Review
All system components appeared to be operating properly. The system is designed to
backwash every 150,000 gallons of water filtered, and occurs typically two to three times per
week. Approximately 300-500 gallons are released during each backwash event.
A Backwash Dechlorinator Norweco Bio-Dynamic Tablet Feeder Model LF 1000 was installed in
May 2009 to comply with the new Total Residual Chlorine limit that took effect April 1, 2009.
Upon installation, the unit had some issues with leakage and improper flow design for the
system. Therefore, the operators enclosed the unit inside a casing to prevent these problems
from occurring. Since then, the unit has been working with no issues. Mr. Randall Thompson
from Simmons Engineering signed the Engineer's Certification for the unit.

5. Operations & Maintenance
The facility appeared to be functioning properly during the inspection. Daily Backwash Logs,
ORC Visitation Logs, and Monthly Preventive Maintenance Logs are kept in the operations
building. Mr. Steven Yarbrough is the ORC. Mr. Jerry Hatchett is Back -Up ORC.
6. Records/Reports
The Discharge Monitoring Reports (DMRs), files, and lab data are very well organized in one
binder and are maintained at the Caswell County Schools Maintenance Shop. All records were
available for review during the inspection.
7. Laboratory
Pace Analytical performs all sample analysis on a bi-monthly basis. The lab technician from
Pace performs Total Residual Chlorine and pH tests at the facility. All other samples are
collected as grab samples and transported to Pace Analytical's laboratory for analysis.
8. Effluent/Receiving Waters
The effluent pipe is located at the bottom of the hill below the well house. The effluent is
discharged into a dry drainage ditch that ultimately drains to Hyco Creek in the Roanoke River
Basin.

system did not operate properly once installed, and operators were forced to remove it. With
the help of an engineer, operators designed a workable dechlorination system that is currently in
place. Please note that you will need to secure the engineer's certification for the modified
dechlorination system and maintain this with the facility files.

noncompliance problem. Please be aware that violations of your NPDES permit could subject
you to enforcement action by this Division with the possible assessment of civil penalties of up

"If" the filter is part of a Discharging system, such as a "Well" system that treats for Iron and Manganese and has a back-
wash, they would be
required by regulation to have an NPDES permit and need to be classified to have an ORC and Back-up ORC. There was
some discussion at prior
Consultants meetings that these "small" water systems needed to be permitted and have operators. I disagreed, with
the stance that they
should be dealt with on a case -by -case basis depending on the environmental impact. The new "Ground Water Rule"
being enforced by PWSS
requires that a lot of the small water systems meet a "CT" time regarding their disinfection methods. To do this, some of
these small systems will have to use "everything" capable to meet the "CT" requirement. This may include "back
washing" hydropunematic pressure tanks to back -washing

I know that Mark made some noise about trying to get the operators of green sand filters to have a operators' license
requirement (physical/chemical). As far as I know, nothing ever came of that. I just received a call from a green sand
filter operator who was told by staff from Pace Analytical that he would need a Physical/Chemical certification to
continue to run the backwash system. I conferred with Gary and we both agreed that we had not seen/heard about a
new requirement. Do you know if this is still the case? Thanks.

Dear Mr. Yarbrough:
A fast track application for Authorization to Construct dechlorination facilities was
received on April 27, 2009, by the Division of Water Quality (Division). Authorization is hereby
granted for the construction of modifications to the existing South Elementary WTP, with
discharge of washwater effluent into Hyco Creek in the Roanoke River Basin.
This authorization results in no increase in design or permitted capacity and is awarded
for the construction of the following specific modifications:
Installation of a dechlorination system utilizing sodium thiosulfate pursuant to the
fast track application received on April 27, 2009, and in conformity with the
Minimum Design Criteria for Dechlorination Facilities.
This Authorization to Construct is issued in accordance with Part III, Paragraph A of
NPDES Permit No. NCO086983 issued September 7, 2007, and shall be subject to revocation
unless the wastewater treatment facilities are constructed in accordance with the conditions and
limitations specified in Permit No. NC0086983.
In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may
be required by this Division, such as the construction of additional or replacement wastewater
treatment or disposal facilities.

The Winston-Salem Regional Office, telephone number (336) 771-5000 shall be notified
at least forty-eight (48) hours in advance of operation of the installed facilities so that an on site
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Upon completion of construction and prior to operation of these permitted facilities, the
attached Engineer's Certification form must be received from a professional engineer certifying
that the permitted facility has been installed in accordance with the NPDES Permit and this
Authorization to Construct. Mail the Certification to: Construction Grants & Loans,
DWQ/DENR, 1633 Mail Service Center, Raleigh, NC 27699-1633.
Upon classification of the facility by the Certification Commission, the Permittee shall
employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the
wastewater treatment facilities.. The operator must hold a certificate of the type and grade at least
equivalent to or greater than the classification assigned to the wastewater treatment facilities by
the Certification Commission.
The Permittee must also employ a certified back-up operator of the appropriate type and
grade to comply with the conditions of T15A:8G.0202. The ORC of the facility must visit each
Class I facility at least weekly and each Class II, III and IV facility at least daily, excluding
weekends and holidays, must properly manage the facility, must document daily operation and
maintenance of the facility, and must comply with all other conditions of T15A:8G.0202.
A copy of the approved plans and specifications shall be maintained on file by the
Permittee for the life of the facility.
During the construction of the proposed additions/modifications, the permittee shall
continue to properly maintain and operate the existing wastewater treatment facilities at all times,
and in such a manner, as necessary to comply with the effluent limits specified in the NPDES
Permit.
You are reminded that it is mandatory for the project to be constructed in accordance
with the North Carolina Sedimentation Pollution Control Act, and, when applicable, the North
Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractor's
responsibilities shall be in complying with these Acts.
Prior to entering into any contract(s) for construction, the recipient must have obtained all
applicable permits from the State.
Failure to abide by the requirements contained in this Authorization to Construct may
subject the Permittee to an enforcement action by the Division in accordance with North
Carolina General Statute 143-215.6A to 143-215.6C.

complying with any and all statutes, rules, regulations, or ordinances which may be imposed by
other government agencies (local, state, and federal) which have jurisdiction.
