SUBJECT: Compliance Evaluation Inspection
Milton WWTP

Dear Mr. Jeffress:
On January 7, 2015, a Compliance Evaluation Inspection was conducted at the Town of Milton's Wastewater
Treatment Plant. Mr. Matthew Smith, ORC, was present during the inspection. The inspection consisted of two parts:
an on -site inspection of the treatment facility and a review of facility files. The following is a summary of findings
during the inspection.
A) Facility records were available for review and were complete. Raw field parameter data and lab data correctly
correlated with data submitted on monthly Discharge Monitoring Reports.
B) The permit expires on May 31, 2017. A renewal application is due 180 days prior to expiration.
C) One of 2 blowers for the aeration basin was inoperable during the inspection. The ORC usually cycles
between the two blowers. It is essential to have the other blower repaired as soon as possible so that the ORC has a
back-up.
D) The back-up generator is waiting for repairs, as the ORC was unable to get it started since approximately
two weeks prior to the inspection. Records indicated that the repairs had been requested by the ORC but not yet made.
Overall, Mr. Smith appears to be operating the plant sufficiently. It is important that preventative maintenance be
performed regularly and repairs made promptly in order. to support Mr. Smith's efforts. Please refer to the

On January 16, 2014, Ms. Jenifer Carter of the Winston-Salem Regional Office conducted a Compliance Evaluation
Inspection at the Town of Milton's Wastewater Treatment Plant. Mr. Matthew Smith, ORC, was present during the
inspection. The inspection consisted of two parts: an on -site inspection of the treatment facility and a review of
facility files. The following is a summary of findings during the inspection.
A) Facility records were available for review and were complete. Raw field parameter data and lab data correctly
correlated with data submitted on Discharge Monitoring Reports.
B) The permit expires on May 31, 2017.
C) The digital display for the UV system's intensity reading does not work. Mr. Smith cleans the bulbs weekly and
changes them out after approximately 8,000 hours of use. Based on fecal coliform data, this appears to provide
adequate disinfection. One bulb was not working during the inspection. Mr. Smith was aware of it needing to be
replaced.
Overall, the plant seemed to be maintained and operated sufficiently. Please refer to the attached report for more

On December 6, 2012, Ms. Jenifer Carter of the Winston-Salem Regional Office conducted a Compliance
Evaluation Inspection at the Town of Milton's Wastewater Treatment Plant. Mr. Matthew Smith, ORC, was
present during the inspection. The inspection consisted of two parts: an on -site inspection of the treatment facility
and a review of facility files. The following is a summary of findings during the inspection.
A) Mr. Smith took over as ORC in July 2012, obtaining the required Grade Il Biological Wastewater
certification soon thereafter, in September 2012.
B) The permit expires on May 31, 2017.
C) Facility records were available for review. Raw data for Dissolved Oxygen (DO) and Total Residual Chlorine
(TRC) could not be found to correlate with August 20, 2012 data submitted on the August 2012 Discharge
Monitoring Report. Since there were calibration records for that day, it is possible that data was written on the
unsigned DMR without being entered in the daily log book. Steps should be taken to prevent this from
happening in the future.
D) The digital display for the UV system's intensity reading was malfunctioning during the inspection, though a
reading was finally obtained. Although Mr. Smith said the bulbs were cleaned weekly and changed annually, an
accurate intensity reading is necessary to ensure proper disinfection is taking place at any given time. Please
make the necessary repairs to the UV system are made to allow for reliable readings.
Overall, the plant seemed to be maintained and operated sufficiently. Please refer to the attached report for more

aeration basin with suspended media. The DO for these types of systems is typically

Milton WWTP, Caswell County
Dear Mr. Jeffress:
Jenny Graznak of this office performed a Compliance Evaluation Inspection at the Mllton
WWTP in Caswell County on September 27, 2011. Ron Wall, Operator in Responsible Charge
(ORC) was present for the inspection. This type of inspection consists of two basic parts: an in -
office review of facility files and self -monitoring data and an on -site inspection of the facility. This
particular inspection evaluated nine (9) areas, and observations are addressed below:
1. Permit
The current permit became effective September 1, 2007 and expires May 31, 2012. A copy of the
permit was available during the inspection.
2. Self -Monitoring Program
The plant began discharging in January 2010. A review of the self -monitoring data for the months of
July 2010 through June 2011 showed that all parameters were sampled and analyzed per the
requirements of the permit. There were no violations of effluent limitations or monitoring frequency
during this time period.
3. Laboratory
The operator measures temperature, dissolved oxygen and pH at the plant, and R & A Laboratories
analyzes all other parameters at the laboratory. R & A Labs holds field parameter certification #
5483. Calibration records for the operator's field meters are maintained with his meters.
4. Flow Measurement
Effluent flow is measured daily. BW Allen Company calibrated the meter on September 23, 2010,
and is scheduled to re -calibrate the meter this week. Weekly flow charts are maintained at the plant.
5. Facility Site Review
The plant consists of a bar screen, equalization tank, aeration tank, clarifier, UV disinfection, and
sludge holding tanks. There is also an emergency generator that runs automatically for 20 minutes
every Thursday, under the supervision of Town of Milton staff. Each of the two blowers in the
aeration tank runs 6 hours per day. Since the plant began discharging in 2010, the operators
recognized several issues with regard to plant:
-- The sludge holding tanks are underground and covered and are not aerated. Operators
are worried about gas buildup and are requesting the addition of aeratiop in the tanks.

-- Operators feel that the return sludge pipe is not placed far enough down in the clarifier to
pull an appropriate amount of sludge. Operators are requesting that the pipe be adjusted
lower in the clarifier tank.
-- The clarifier weirs are not level and need to be adjusted.
-- The UV system appears to be calibrated for drinking water instead of wastewater. The
system runs continuously.
6. Operations & Maintenance
Ron Wall (Grade IV) is the ORC and Matt Smith (Grade 1) is the Back -Up ORC for the plant. All
maintenance is handled by Town of Milton, however no record was available of maintenance
activities at the plant. A maintenance log should be kept at the plant to record regular maintenance
activities.
It was noted during the file review that during the first half of 2011 (January —June), the ORC
has only visited the plant 62% of the system visitations while the Back -Up ORC has visited
the plant 38% of the system visitations. You should be aware that according to NC
Administrative Code Title 15A 08G .0205, "the Back -Up ORC may act as surrogate for the
ORC ... for a period not to exceed 40% of the system visitations required per calendar year."
At this current rate of visitation, you may have trouble meeting this requirement for the
calendar year.
7. Records/Reports
The Discharge Monitoring Reports (DMRs), files, and lab data are maintained according to the
permit. A daily operator log book is used to record activities at the plant. The Annual Report for
2010 or 2011 was not available for review during the inspection. Please forward a copy of the 2010
Annual Report to our office as soon as possible.
8. Effluent/Receiving Waters
The effluent is discharged into Country Line Creek. The path to the effluent pipe has been cleared
and maintained by the operations staff. The effluent appeared clear with no traceable solids on the
day of the inspection.
9. Sludge Disposal
No sludge wasting has occurred at the plant since it began operating in January 2010. However, a
contract exists with Jenkins Waste Management to remove sludge from the sludge holding tanks as
necessary.

Jenny Graznak of this office performed a Compliance Evaluation Inspection at the Milton
WWTP in Caswell County on April 22, 2010. Ron Wall, Operator in Responsible Charge (ORC) was
present for the inspection. This type of inspection consists of two basic parts: an in -office review of
facility files and self -monitoring data and an on -site inspection of the facility. This particular
inspection evaluated nine (9) areas, and observations from each area are addressed below:
1. Permit
The current permit became effective September 1, 2007 and expires May 31, 2012. A copy of the
permit was available during the inspection.
2. Self -Monitoring Program
The plant began discharging in January 2010. A review of the self -monitoring data submitted since
its official start showed that all parameters were sampled and analyzed per the requirements of the
permit. There were no violations of effluent limitations during this time period.
3. Laboratory
The operator measures Total Residual Chlorine, temperature, dissolved oxygen and pH at the plant,
and R & A Laboratories analyzes all other parameters at the laboratory. R & A Labs holds field
parameter certification # 5483. Calibration records for the operator's field meters are maintained
with his meters. Please note that you do not need to monitor Total Residual Chlorine since you are
not adding chlorine as`a disinfectant in _the �'treatmenx process: Effluent disinfection is attained
through a UV system.
4. Flow Measurement
Effluent flow is measured daily. BW Allen Company calibrated the meter on August 28, 2009.
Weekly flow charts are maintained at the plant.
5. Facility Site Review
The plant consists of a bar screen, equalization tank, aeration tank, clarifier, UV disinfection, and
sludge holding tanks. There is also an emergency generator that runs automatically for 20 minutes
every Tuesday evening, under the supervision of Town of Milton staff. Each of the two blowers in
the aeration tank runs 6 hours per day. Since this is a brand new facility, the operators have
recognized several issues with regard to the plant itself:

-- The sludge holding tanks are underground and are not aerated. Operators are worried
about odor concerns and are requesting the addition of aeration in the tanks.
— Operators feel that the return sludge pipe is not placed far enough down in the clarifier to
pull an appropriate amount of sludge. This may be the reason for some floating sludge that
was observed on the surface of the clarifier during the inspection. Operators are requesting
that the pipe be adjusted lower in the clarifier tank.
-- The clarifier weirs are not level and need to be adjusted.
— The UV system requires daily maintenance so the operators speculate that the system
was calibrated on freshwater instead of wastewater.
-- The plant should be equipped with a dumpster for the disposal of bar screenings.
Currently, the operator is hauling a bucket with bar screenings in his truck to the next site.
6. Operations & Maintenance
Ron Wall (Grade III) is the ORC and Neal McDuffie (Grade 1) is the Back -Up ORC for the plant. No
issues were noted with regard to maintenance during the inspection. The entrance gate to the plant
lists emergency contact numbers.
7. Records/Reports
The Discharge Monitoring Reports (DMRs), files, and lab data are maintained according to the
permit. A daily operator log book is used to record activities at the plant. Maintenance activities at
the plant are handled entirely by staff from R & A Labs.
8. Effluent/Receiving Waters
The effluent is discharged into Country Line Creek. The path to the effluent pipe has been cleared
and maintained by the operations staff. The effluent appeared clear with no traceable solids on the
day of the inspection.
9. Sludge Disposal
Since the plant has only been discharging for four months, no sludge wasting has occurred yet.
However, a contract exists with Jenkins Waste Management to remove sludge from the sludge
holding tanks as necessary.
Other than the few highlighted issues in this letter, there were no major problems found
during the inspection. Please refer to the attached report for specific details from the inspection. If

above to the North Carolina Attorney General's Office for collection through the courts unless

The purpose of this letter is to provide the Engineer's Certification for the above reference project. Per
our telephone conversation on September 29, 2009 it is understood that this certification will cover the
facilities that have been constructed and changes from the Authorization to Construct dated May 30, 2007
will be noted. This wastewater treatment plant is permitted for a flow of 25,000 gpd under NPDES
Permit No. NC0087645.
The Authorization to Construct resulted in a design and permitted capacity of 25,000 gpd, and was
awarded for the construction of the following specific facilities:
"A 7,500 gallon equalization tank with influent bar screen and dual 80 gpm equalization pumps; a
10,000 gallon aeration tank using a fixed -film rotating aerator; a back-up 100 cfm air blower; a
144 square foot clarifier with air-lift sludge return pumps; a UV disinfection system rated at
25,000 gpd; an effluent flow meter; an 8 inch gravity outfall pipe; dual 3,000 gallon sludge
holding tanks; a 25 kW emergency generator; and associated yard piping, in conformity with the
project plans, specifications, and other supporting data filed and approved by the Department of
Environment and Natural Resources."
In order to comply with North Carolina General Statute 133-3, a performance specification was
developed and alternate equipment was approved during the bidding and submittal phases of the project.
It was determined that this equipment would meet the performance specification requirements.
The Engineer's Certification is attached to this letter and all changes have been included in the description

emergency generator; and associated yard piping, in conformity with the project plans,

The discharge pipe is built up with rip rap and will discharge to Country Line Creek. Approximately 120-125
houses will be hooked up to the collection pressure system. Each house has a septic tank to collect solids
and a pump that directs to wastewater plant. At first, R & A Labs will be responsible for all operations.
However, eventually Elmer Stewart will take over as ORC for the colelction system aspect of the facility.
The system has been deemed, with permit number WQCSD0422.

the Contractor has a few more items to complete before ready.

A letter of request for Authorization to Construct was received March 28, 2007, by the
Division, and final plans and specifications for the subject project have been reviewed and found
to be satisfactory. Authorization is hereby granted for the construction of a new 25,000 gpd
Town of Milton Wastewater Treatment Plant, with discharge of treated wastewater into County
Line Creek in the Roanoke River Basin.
This authorization results in a design and permitted capacity of 25,000 gpd, and is
awarded for the construction of the following specific facilities:
A 7,500 gallon equalization tank with influent bar screen and dual 80 gpm equalization
pumps; a 10,000 gallon aeration tank using a fixed -film rotating aerator; a back-up 100 cfm
air blower; a 144 square foot clarifier with air-lift sludge return pumps; a UV disinfection
system rated at 25,000 GPD; an effluent flow meter; an 8-inch gravity outfall pipe; dual
3,000 gallon sludge holding tanks; a 25 kW emergency generator; and associated yard
piping, in conformity with the project plans, specifications, and other supporting data
subsequently filed and approved by the Department of Environment and Natural Resources.
This Authorization to Construct is issued in accordance with Part III, Paragraph A of
NPDES Permit No. NCO087645 issued March 16, 2004, and shall be subject to revocation unless
the wastewater treatment facilities are constructed in accordance with the conditions and
limitations specified in Permit No. NC0087645.
The sludge generated from these treatment facilities must be disposed of in accordance
with G.S. 143-215.1 and in a manner approved by the North Carolina Division of Water Quality.
In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may
be required by this Division, such as the construction of additional or replacement wastewater
treatment or disposal facilities.

The Winston-Salem Regional Office, telephone number (336) 771-5000, shall be notified
at least forty-eight (48) hours in advance of operation of the installed facilities so that an on site
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted facility
has been installed in accordance with the NPDES Permit, this Authorization to Construct and the
approved plans and specifications. Mail the Certification to Cecil G. Madden, Jr., P.E.,
Construction Grants & Loans, DWQ / DENR, 1633 Mail Service Center, Raleigh, NC 27699-
1633.
Upon classification of the wastewater treatment facilities by the Water Pollution Control
System Operators Certification Commission (WPCSOCC), the Permittee shall designate and
employ a certified operator to be in responsible charge (ORC) and one or more certified
operator(s) to be back-up ORC(s) of the facilities in accordance with 15A NCAC 8G .0201. The
ORC shall visit the facilities in accordance with 15A NCAC 8G .0204 or as specified in this
permit and shall comply with all other conditions specified in these rules.
A copy of the approved plans and specifications shall be maintained on file by the
Permittee for the life of the facility.
During the construction of the proposed additions/modifications, the permittee shall
continue to properly maintain and operate the existing wastewater treatment facilities at all times,
and in such a manner, as necessary to comply with the effluent limits specified in the NPDES
Permit.
You are reminded that it is mandatory for the project to be constructed in accordance
with the North Carolina Sedimentation Pollution Control Act, and, when applicable, the North
Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractor's
responsibilities shall be in complying with these Acts.
Prior to entering into any contract(s) for construction, the recipient must have obtained all
applicable permits from the State.
Failure to abide by the requirements contained in this Authorization to Construct may
subject the Permittee to an enforcement action by the Division of Water Quality in accordance
with North Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from
complying with any and all statutes, rules, regulations, or ordinances which may be imposed by
other government agencies (local, state, and federal) which have jurisdiction.

Part II -. DESCRIPTION OF DISCHARGE AND TREATMENT WORKS

The Alternatives Analysis Report concludes that a constructed wetlands
wastewater treatment facility is the best alternative based on the net present cost
standpoint. The net present cost of the NPDES option (constructed wetland -
$2,299,045) is 30 % cheaper than the nondischarge option (spray irrigation -
$2,990,996).
The Preliminary Engineering Report proposes a 0.05 MGD constructed wetlands
wastewater treatment facility consisting of the following: influent metering station,
bar screen, facultative lagoon (with floating aerator), dual shallow rooted plant
cells (bullrush and cattails) and post -treatment (sandfilter, UV disinfection,
aeration unit and metering manhole). In addition, a collection system with 13,858
feet of gravity line, 4,409 feet of force main and three pump stations is proposed.

g. Possible toxic impacts to surface waters: A speculative limit of 5.9 mg/1 is
proposed to protect for ammonia toxicity.

PART III - OTHER PERTINENT INFORMATION
1. Is this facility being constructed with Construction Grant Funds or public monies
(municipals only)? Funding has been secured for the project. The Town of Milton
received a $1,307,000 grant from the Rural Center through the Unsewered Communities
Grant Program. Additional funding will come from the USDA Rural Development office.
In addition, the Town has committed $20,000 to the project.
2. Special monitoring or limitations (including toxicity) requests: NA.
3. Important SOC, JOC or Compliance Schedule dates: NA.
4. Alternative Analysis Evaluation:
Spray Irrigation: The Alternatives Analysis report indicates that a spray or drip irrigation
system is not feasible because "the availability of that much consecutive land is a concern
for the Town of Milton". It should be noted that a detailed soils analysis report was not
provided as required by DWQ's EEA Guidance Document. In addition, it was not
determined if any additional land could be acquired, nor was there any documentation
from adjacent owners stating that additional land was unavailable.
Connection to Regional Sewer System: Not feasible due to distance (approximately 9
miles of force main needed to reach the City of Danville, Va).
Subsurface: Not feasible due to volume of water to be disposed.

PART IV - EVALUATION AND RECOMMENDATIONS
The Town of Milton (population 236 or 107 connections) is currently served by septic
tanks. Due to concerns over failing systems, the town is proposing the construction of a
collection system and NPDES treatment facility. The proposed Constructed Wetland Wastewater
Treatment Facility (CWWTF) would be a good match for the Town of Milton's situation (lower
maintenance costs and domestic wastes only).
