On December 16, 2014, Jenifer Carter of the Winston-Salem Regional Office conducted an NPDES Wastewater
Compliance Evaluation Inspection at the Town of Yanceyville's Water Treatment Plant. Brian Merritt, ORC, was
present during the inspection. The inspection consisted of two parts: an on -site inspection of the treatment facility and a
review of facility files and self -monitoring data. The following is a summary of findings during the inspection:
A) Sludge levels in the settling ponds continue to better -maintained under Mr. Merritt. Although Mr. Merritt is able
to stave off seepage from the dam of the larger lagoon by inspecting the dechlorinator tubes daily (to keep them
from clogging), the Town needs to begin planning for repairing the concrete liner in order to properly
maintain the treatment unit as required by the permit.
B) Lab data corresponded correctly with data entered on the submitted Discharge Monitoring Reports.
C) If not already done, please have an Annual Performance Report submitted to the Division, and provide a copy
to users/customers of the Town's Water Treatment Plant, as required by Part II Section E-12 of your permit.
Please refer to the enclosed report for more inspection details. The Division appreciates Mr. Merritt's efforts to
maintain and operate the aging treatment system and hopes that the Town continues its good working relationship with

Mr. Merritt has done well operating the system, considering the need for repairs to the liner in the lower

which was out of holding time. Per 15A NCAC 2H .0805 (a) (7) (N) any time a sample is received
which does not meet sample collection, holding time, or preservation requirements, the laboratory
must notify the sample collector or client and secure another sample if possible. If another sample

Environment 1, Inc on behalf of Yanceyville WTP (Permit # NC0007323). At your request the
sample was analyzed out of holding time.
Please be advised that under provisions of North Carolina Regulation, 15A NCAC .0805 (a) (7) (N),
any time a laboratory receives samples that do not meet sample collection, holding time, or
preservation requirements, the laboratory must notify the sample collector or client and secure
another sample if possible. If another sample cannot be secured, the original sample may be
analyzed but the results reported (on both the client report and the Discharge Monitoring Report or
other regulatory compliance report submitted to the state of North Carolina) must be qualified with

pond was being bypassed, and has since put it back into proper service.
B) Sludge levels in the settling ponds are better maintained under Mr. Merritt, who said that sludge is due to be removed
again in June or July of this year. Retaining walls around the lagoons are still in need of attention, but maintaining lower
sludge levels has almost eliminated the permit limits violations that were a consistent problem in the past.
C) Lab data corresponded correctly with data entered on the submitted Discharge Monitoring Reports. Overall record
keeping is much improved.
Mr. Merritt has made many improvements since the last inspection, including a vast improvement in record keeping, and
returning the settling ponds to proper working condition. The Division greatly appreciates his efforts and hopes that the

over operations of wastewater treatment at the plant, he discovered that the first pond
was being bypassed. He arranged to have sludge removed after the last inspection and

carefully to ensure thorough understanding of the conditions and requirements it contains.
The draft permit includes a number of changes in the monitoring requirements from those found in your
current permit. These changes were made as part of the implementation of a modified NPDES strategy
for wastewater discharges from conventional water treatment plants, which are being implemented
throughout the state as permits are issued and/or renewed. A summary of these changes follows:
• The monitoring requirement for Settleable Solids has been removed from the permit.
• Monitoring for Turbidity has been changed from instream monitoring to monitoring of the effluent.
• Monitoring requirements for Aluminum, Manganese, and Copper have been added based upon
studies of discharges from conventional water treatment facilities.
• Whole Effluent Toxicity monitoring has been added to the permit. Please note that while the
result of the required test will be "pass" or "fail," at present this is a monitoring requirement
only, with no associated compliance limit.
• The monitoring frequency for most constituent parameters has been reduced to 2/month or quarterly.
Quarterly monitoring should be performed to coincide with whole effluent toxicity testing.
• A footnote has been added regarding the reporting and compliance determination of Total Residual
Chlorine values.

A. (2.) CHRONIC TOXICITY MONITORING (QUARTERLY)
The permittee shall conduct quarterly chronic toxicity tests using test procedures outlined in the "North
Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or subsequent
versions.
The effluent concentration defined as treatment two in the procedure document is 90 %. The testing
shall be performed as a Ceriodaphnia dubia 7day pass/fail test. The tests will be performed during
the months of January, April, July and October. Effluent sampling for this testing shall be
performed at the NPDES permitted final effluent discharge below all treatment processes.
All toxicity testing results required as part of this permit condition will be entered on the Effluent
Discharge Monitoring Form (MR-1) for the month in which it was performed, using the parameter
code TGP3B. Additionally, DWQ Form AT-1 (original) is to be sent to the following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Section no
later than 30 days after the end of the reporting period for which the report is made.
Test data shall be complete and accurate and include all supporting chemical/physical measurements
performed in association with the toxicity tests, as well as all dose/response data. Total residual
chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for
disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is
required, the permittee will complete the information located at the top of the aquatic toxicity (AT) test
form indicating the facility name, permit number, pipe number, county, and the month/year of the
report with the notation of "No Flow" in the comment area of the form. The report shall be submitted
to the Environmental Sciences Section at the address cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, then
monthly monitoring will begin immediately. Upon submission of a valid test, this monthly test
requirement will revert to quarterly in the months specified above.
Should any test data from this monitoring requirement or tests performed by the North Carolina
Division of Water Quality indicate potential impacts to the receiving stream, this permit may be re-
opened and modified to include alternate monitoring requirements or limits.
If the Permittee monitors any pollutant more frequently then required by this permit, the results of such
monitoring shall be included in the calculation & reporting of the data submitted on the DMR & all AT
Forms submitted.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control
organism survival, minimum control organism reproduction, and appropriate environmental controls,
shall constitute an invalid test and will require immediate follow-up testing to be completed no later
than the last day of the month following the month of the initial monitoring.

Dear Mr. Basinger
On behalf of the Town of Yanceyville, the Town and Envirolink, Inc. appreciates the opportunity to
respond to the Notice of Deficiency issued on May 17, 2012 for failure to report a settable solids value on
the January 2012 DMR for the week of 1/14/12. After investigating the above mentioned event by
reviewing the plant logs and the laboratory chain of custody file, it has been concluded that that the
Operator failed to collect a settable solid sample during the week ending 1/14/12.
During the period of July 2011 through February 2012, Envirolink was using R&A Laboratories to
analyze the compliance samples, including the settable solids parameter. Due to the issue surrounding
this and other events at the wastewater treatment plant, Envirolink has changed its Operator in
Responsible Charge and our method of analyzing settable solids. Since Envirolink is Field Certified for
Settable Solids, the current ORC began analyzing the settable solids in house, March 1, 2012, to ensure
proper monitoring and reporting per the NPDES Permit. Envirolink has also established Standard
Operating Procedures to include a weekly compliance sampling schedule that will ensure the Operator is
sampling the required parameters in accordance with the Permit.

On January 25, 2012, Ms. Jenifer Carter of the Winston-Salem Regional Office conducted a NPDES Wastewater
Compliance Evaluation Inspection at the Town of Yanceyville's Water Treatment Plant. Mr. Terry Klutz, ORC at
the time, was present during the inspection. The inspection consisted of two parts: an on -site inspection of the
treatment facility and a review of facility files and self -monitoring data.
A review of the submitted self -monitoring data since the last inspection revealed several violations of Total
Suspended Solids (TSS) and Settleable Solids (SS) limits during the July, August and September 2011. During the
same period of time, there were several parameters that were not monitored at the frequency required. These
violations have been addressed separately through the Division's monthly enforcement program.
The following is a summary of findings during the inspection:
A) EnviroLink took over operations of the plant in July 2011. Prior to that, R&A Labs had been paying plant
staff to conduct field parameter tests. It appears that R&A did not provide proper oversight to ensure record
keeping and maintenance of field equipment met permit requirements. It has been stated that R&A continued to
pay staff to monitor just Settleable Solids at the time of the inspection. Mr. Klutz, of EnviroLink, also did not
appear to have a good grasp on the record keeping, or lack thereof, during the inspection. It is my understanding
that Mr. Klutz has been replaced since the inspection. This information has been shared with the Division Lab
Certification Unit, which will handle any related compliance issues.
B) As noted during at least the last five (5) inspections, the retaining walls around the two sludge lagoons are in
need of repair. TSS and SS violations also suggest that sludge needs to be removed from the lagoons, as there
was little remaining freeboard. If these items are not addressed before the next inspection, enforcement
action for failure to maintain wastewater treatment facilities may be considered.
C) Facility records were available for review with the exception of lab data/bench sheets to support pH data
entered on the Discharge Monitoring Report for August 21, 2011. Lab data that was available corresponded
correctly with data entered on the submitted Discharge Monitoring Reports.
D) The permit expires on May 31, 2012. A renewal application has been submitted.

indicating you submitted a discharge monitoring report for November 2012 and provide a certified mail
receipt confirming delivery of the missing report.

the violator to the assessment of a civil penalty of up to $25,000 per violation.

Yanceyville Water Treatment Plant — Caswell County
Dear Mr. Brigman:
Jenny Graznak of this office performed a Compliance Evaluation Inspection of the filter
backwash wastewater at the Yanceyville Water Treatment Plant in Caswell County on February
3, 2011. Matt Smith, Back -Up ORC, was present for the inspection. Neal McDuffie is the ORC.
This type of inspection consists of two basic parts: an in -office review of facility files and self -
monitoring data and an on -site inspection of the facility. This particular inspection evaluated
. nine (9) areas, and observations from each area are addressed below:
1. Permit
The permit became effective December 1, 2007 and will expire May 31, 2012.
2. Self -Monitoring Program
A review of the monthly self -monitoring reports from January 2010 through December 2010
showed no limit violations.
3. Laboratory
Research & Analytical Laboratories of Kernersville analyzes all samples for the facility except
the following field samples: temperature, total residual chlorine, and pH. Documentation of
thermometer verification was available during the inspection. R & A Labs last verified the
thermometer on September 21, 2010. Meter calibration records are maintained according to the
permit. The pH meter is calibrated everyday.
4. Flow Measurement
Effluent flow is estimated based on filter backwash volumes. Approximately 40,000 gallons is
discharged each day. This figure originated from the engineering specifications for the
backwash system.
5. Sludge Handling & Disposal
The Town of Yanceyville has a land application permit with DWQ Aquifer Protection Section for
the removal of sludge from the plant's two lagoons. Synagro has not removed sludge from the
lagoons since June 2008, but will likely re -visit the plant in the Spring of 2011. Please
remember that records of sludge removal should be maintained at the facility.

6. Operations & Maintenance
Town of Yanceyville performs all maintenance on the lagoons. However at the time of
the inspection, both lagoons appeared as if very little maintenance has been performed
since the last inspection. The lagoon furthest from the gate entrance had cattail growth
and has very little freeboard remaining. In addition, the concrete retaining wall for this
lagoon is crumbling in a state of disrepair. This observation has been made repeatedly
in compliance evaluation inspection reports from our office. Please remember that these
lagoons are part of the wastewater treatment process and should be maintained as
necessary.
7. Facility Site Review
The water treatment plant discharges decanted filter backwash water through a dual settlement
lagoon system. Filters are washed approximately every other day during cooler months and
every 3-4 days during the summer. A tablet dechlorination system was installed in 2009.
Dechlorination tablets are added to the four -tube system approximately every 2 weeks.
8. Records/Reports
The Discharge Monitoring Reports (DMRs), files, and lab data are maintained according to the
permit. A copy of this data was available for review during the inspection.
9. Effluent/Receiving Waters
The effluent is discharged into the lake below the settling basins. Ultimately, the discharge goes
to Fullers Creek (WS-III, CA) in the Roanoke River Basin. The effluent appeared clear with no
traceable solids.

maintain the integrity of the lagoons in order to keep all wastewater and sludge contained.
These problems will ultimately need to be addressed.

for a period of at least 3 years. Any records pertaining to sludge use and disposal shall be
retained for a period of at least 5 years.

This is to inform you that the Division of Water Quality has not received your monthly monitoring report
for July 2011. This is in violation of Part II, condition D(2) of the NPDES permit, as well as 15A
NCAC 2B .506(a), which requires the submittal of Discharge Monitoring Reports no later than the
last calendar day followingtporting period. Failure to submit reports as required will subject the
violator to the assessment of a civil penalty of up to $25,000 per violation.
To prevent further action, please submit said report within fifteen (15) days of receipt of this notice.
Additionally, this letter provides notice that this office will recommend the assessment of civil
penalties if future reports are not received within the required time frame during the next twelve (12)

Yanceyville WTP — Caswell County
Dear Mr. Parrish:
Jenny Graznak of this office performed a Compliance Evaluation Inspection of the filter
backwash wastewater at the Yanceyville WTP in Caswell County on May 13, 2009. Neal
McDuffie, ORC, and Matt Smith, Back Up ORC, were present for the inspection. This type of
inspection consists of two basic parts: an in -office review of facility files and self -monitoring data
and an on -site inspection of the facility. This particular inspection evaluated nine (9) areas, and
observations from each area are addressed below:
1. Permit
The permit became effective December 1, 2007 and will expire May 31, 2012. This permit
includes a new Fluoride monitoring requirement and a limit for Total Residual Chlorine (TRC).
The new Fluoride requirement took effect on December 1, 2007. The new TRC limit is 17
microgramsAiter (0.017 milligrams/liter) and takes effect on June 1, 2009.
2. Self -Monitoring Program
A review of the monthly self -monitoring reports from March 2008 through February 2009
showed no limit violations.
3. Laboratory
Research & Analytical Laboratories of Kernersville analyzes all samples for the facility except
the following field samples: temperature, total residual chlorine, and pH. R & A Labs last
verified the thermometer on June 17, 2008. Documentation of thermometer verification was
available during the inspection. Meter calibration records are maintained according to the
permit.
4. Flow Measurement
Effluent flow is estimated based on filter backwash volumes. Approximately 40,000 gallons is
discharged each day. This figure originated from the engineering specifications for the
backwash system.
5. Sludge Handling & Disposal
The Town of Yanceyville has a land application permit with DWQ Aquifer Protection Section for
the removal of sludge from the plant's two lagoons. Synagro removed sludge in June 2008.

Records of sludge removal are maintained at the Town Hall. Please note that copies of
sludge removal records should be maintained at the plant and made available to DWQ
inspectors upon request.
6. Operations & Maintenance
The lagoons at the plant look much better since sludge has been removed and the vegetation
on the banks has been better maintained. However, it was noted that the grass clippings
from mowing the banks ended up inside the lagoons. This will affect the quality of your
effluent from the lagoons. Please see that the lawn maintenance staff is careful to avoid this
from happening in the future. In addition, the concrete retaining walls in both lagoons
continue to fail and are in need of repair. This observation has been made repeatedly in
compliance evaluation inspection reports from our office. It is important to maintain the integrity
of the lagoons in order to allow for appropriate settling and high quality effluent. These
problems need to be addressed.
7. Facility Site Review
The water treatment plant discharges decanted filter backwash water through a dual settlement
lagoon system. A tablet dechlorination system was installed several months ago in preparation
for the new Total Residual Chlorine (TRC) limit, which will become effective June 1, 2009.
8. Records/Reports
The organization of paperwork and records retention continues to be much improved at the
plant since previous inspections. Please remember that .the permit requires that you retain
records of all monitoring information, including all calibration and facility maintenance records
for a period of at least 3 years. Any records pertaining to sludge use and disposal shall be
retained for a period of at least 5 years.
9. Effluent/Receiving Waters
The effluent is discharged into the lake below the settling basins. Ultimately, the discharge goes
to Fullers Creek (WS-III, CA) in the Roanoke River Basin. The effluent appeared clear with no
traceable solids.

not available) for the new WTP. The old water supply lake
and creek should be reclassified (when the new discharge
