Nowicki March 18, 2020 Page 2 of 2 A. Operation & Maintenance Plan: All renewal application packages must include an Operation and Maintenance Plan as per the instructions of Form DCAR 06-16. The Operation and Maintenance Plan must be in accordance with 15A NCAC 02T .1110 and include the following as minimum requirements: a. Operational functions: please describe the operation of the program and by whom the functions are to be conducted. b. Maintenance schedules: please include equipment calibration, maintenance of signs, etc. c. Safety measures: please include safety training program, manuals, signs, etc. d. Spill response plan please describe control, containment and remediation measures and emergency contact information. e. Names and titles of personnel responsible for conducting inspections. f. Frequency and location of inspections, and procedures to assure that the selected location(s) and inspection frequency are representative of the residuals management program. g. Detailed description of inspection procedures including record keeping and actions to be taken by the inspector in the event that noncompliance is observed. h. Sampling and monitoring plan including the following information; i. Names and titles of personnel responsible for conducting the sampling and monitoring. j. Detailed description of monitoring procedures for compliance. This Operation and Maintenance Plan is required for all residuals management programs permitted under North Carolina General Statue Chapter 143 Article 21.
