Three sets of detailed plans and specifications signed and sealed by a North Carolina Professional Engineer. The plans must include a general location map, a plan view of the sewer extension, a profile of the sewer extension, and must show the proximity of the sewer extension to other utilities and natural features. Specifications may be omitted for delegated authorities. Each sheet of the plans and the first page of the specifications must be -signed and sealed. Three copies of the existing permit if a renewal or modification. c. The appropriate permit processing fee, in accordance with 15A NCAC 2H .0205(c)(5) d. If the owner/authority of the wastewater treatment facility (WWTF) that will be accepting the wastewater flow from this project is different from the applicant of the project, then a letter must be provided from the owner/authority of the WWTF specifying the volume of flow that will be accepted. The letter should be a recent letter and should refer to the project by the same name as that identified on the application and the plans/specifications. Moore Regional Hospital Sewer Extension Moore County Dear Mr. Gattis: The Permits and Engineering Unit has completed a preliminary engineering review of the subject application. The following items must be addressed before we can complete our review: 1) On February 1, 1993, the Division adopted new regulations that require that the infiltration/exfiltration rate must not exceed 100 GPD per inch of pipe diameter per mile of pipe. All sewer extension permit applications received on or after February 1, 1993, must conform to these new regulations. Please revise page 02660-5, section 3.4.A.1., of your specifications to comply with the new value. Please be advised that the Division is in the process of revising the application forms to reflect the new regulations. 2) Please include in your specifications a section addressing the required minimum separations (horizontally and vertically) of sanitary sewers from other utilities (i.e. storm sewers and water lines). Note: Only pages which have been revised need to be forwarded. Please ensure that 3 signed and sealed copies of each, with perforations along the left-hand edge, are submitted to my attention.
