III. Certification Statement:
I W.T. Bass IV, PE certify to the best of my knowledge that the addition of
the volume of wastewater to be permitted in this project has been evaluated along the route to the receiving
wastewater treatment facility and that the flow from this project is not anticipated to cause any capacity
related sanitary sewer overflows or overburden any downstream pump station en route to the receiving
treatment plant under normal circumstances, given the implementation of the planned improvements
identified in the planning assessment where applicable. This analysis has been performed in accordance
with local established policies and procedures using the best available data. This certification applies to
those items listed above in Sections I and II plus all attached planning assessment addendums for which I
am the responsible party. Signature of this form certifies that the receiving collection system or treatment
works has adequate capacity to transport and treat the proposed new wastewater.

This project consists of 8", 12" & 16" DIP Gravity Public Sewer Extension to serve a proposed Multi -
Phase Apartment Complex located at 3761 Raleigh Road Parkway W. in Wilson, NC. Phase 1 of the
proposed complex will consist of 264 residential units, a clubhouse, pool house and a maintenance
building. This public outfall gravity sewer main extension is proposed to discharge into the existing
15" gravity main sewer line, located to the southwest of the project, within an existing 30' sewer
easement recorded in DB 2808 at Page 449. This proposed gravity sewer main extension will also
include some steel encased 16" DIP along with some 12" & 16" DIP installed upon piers.

The above referenced previously submitted application includes lines that are deemed "high
priority" as indicated on the construction plans included herewith and as follows:
1. 475 LF of 16" Restrained Joint DIP gravity sewer on piers from L1 station 0+00 to 4+75,
which includes:
a. 20+/- LF (0+00 to 0+20+/-) adjacent to streambank and subject to erosion.
b. 68 LF (0+25 to 0+88) inside a 30" steel encasement pipe for an aerial creek crossing.
c. 387 LF (0+88 to 4+75) aerial or no cover within a flood hazard area.
2. 525 LF of 12" Restrained Joint DIP gravity sewer on piers from L1 station 6+50 to 11+75,
which includes:
a. 100+/- LF (6+75+/- to 7+75+/-) of aerial pipe within a flood hazard area.
b. 425+/- LF with very little or no cover within a flood hazard area.
3. 237+/- LF of 12" Restrained Joint DiP gravity sewer on piers from L2 station 0+00 to 2+37+/-,
which Includes pipe with very little or no cover within a flood hazard area.
The above conditions are indicated on sheets CP1 and CP3 of the construction plans.
