Dear Mr. Tankard;
We are in receipt of the Notice of Violations dated March 30, 2020. The following steps have been
taken to remediate the issues found in each violation.
NOV-2020-PC-0181
October 2019 Late/Missing NDMR
Operator will provide monitoring reports to be signed by Slash Creek designated agent by the 15th of
each month. The designee will send original and two copies to the mail center by US mail,
registered/return receipt as well as submit electronically.
NOV-2020-PC-0182
October Late/Missing NDAR-1
Operator will provide monitoring reports to be signed by Slash Creek designated agent by the 151h of
each month. The designee will send original and two copies to the mail center by US mail,
registered/return receipt as well as submit electronically.
NOV-2020-PC-0178
November Late/Missing NDAR-1
Operator will provide monitoring reports to be signed by Slash Creek designated agent by the 15th of
each month. The designee will send original and two copies to the mail center by US mail,
registered/return receipt as well as submit electronically.
NOV-2020-MV-0042
November NDMR - Parameters input on PPI 002 not PPI 001

The NDMR reporting requirements were reported on PPI 2,3,4 not PPI 1,2,3. PPI 001 page failed to
allow input and the PPI 002 was copied and used in its place. An amended report is being filed, a copy is
attached. The operator has requested a new formatted excel spread sheet to replace corrupted file in
use. In the future, if there is a failure in the excel format program, the input will be made manually on
the correct PPI page.
