Dear Mr. Smith:
The Division of Water Resources (DWR) acknowledges receipt of your 2019 Annual Report for the subject permit. A
review of this report was conducted by DWR staff person Justin Henderson. While, review of the subject report
reflects compliance with Permit No. WQ0032723, the following item of concern warrants your attention.
1) The Field Summary Form (FSF) for Field NC-RC-126-02 only summarized application of water treatment
plant residuals authorized by the subject permit. However, the FSF for the same field submitted in the
2019 Annual Report for Permit No. WQ0002376, indicated that applications of both water treatment
plant and wastewater treatment plant residuals occurred at this site. Complete FSF forms shall included a
summary of all residual sources applied to a particular application site, within the same application year,
to summarize the total Plant Available Nitrogen (PAN), and annual and cumulative metal loading
application rates.
If future land application events are planned, please consider contacting DWR staff person Justin Henderson
([e-mail] or 336-776-9701) prior to the event so that an onsite compliance inspection may
be completed.
