Attached please find a copy of the Permit Contacts Report for NCG210399.

Please review for accuracy, specifically the Billing Contact.

For billing contact changes, email Deborah Reese. For other changes, such as address corrections or updates, email
Suzanne McCoy. Be sure to include the individual permit number or Certificate of Coverage (COC) number with these
requests.

Also, please note there are 3 years of outstanding permit fees. I have requested that Deborah Reese send you copies of
the invoices.
