Division of Water Resources' Central and Regional staff has reviewed the application package
received January 23, 2005. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on May 18,
2020.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items,
or failure to provide the additional information on or before the above requested date may result in your
application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic cony.

A. Application (15A NCAC 02T .0105):
Complete the table for groundwater monitoring wells. The coordinates must have 6 decimal degrees and
include the gradient and location in relation to the compliance/review boundary.
Per 15A NCAC 02T .0110, the facility must be constructed in accordance with the permit and Division -
approved plans and specifications. The current configuration of the irrigation lagoon and Field 9 varies
from the plans attached to the permit dated December 12, 2005. The change means that Field 9 has a
smaller acreage than what it permitted. Additional fields also appear different than what was permitted.
Address.
B. ORC:
1. The backup ORC, Joseph Sutton, has an invalid certification. Please contact Beth Buffingion with
the Operator Certification Program to correct.
C. Site Map:
Provide an updated site map in accordance with 15A NCAC 02T .0105(d) that shows the following:
1. Legend, north arrow, and scale
2. Distance measurements verifying all setbacks are being met
3. Stormwater drainage controls
4. Location and ownership of property boundaries within 500 feet of the disposal area (including road/rail
right-of-ways)
5. All facility -related structures and fences within the treatment, storage, and disposal areas
6. All habitable residences and places of assembly within 500 feet of all treatment, storage, and disposal sites
7. Topographic contour intervals not exceeding 10 feet or 25 percent of total site relief
8. Soil mapping units on all disposal sites
9. Location of all wells, streams (ephemeral, intermittent, and perennial), springs, lakes, ponds, ditches, and
other surface drainage features within 500 feet of all waste treatment, storage, and disposal site(s)
10. Location and identification of each monitoring well (identify any background/upgradient wells)
11. Latitude and longitude coordinates of all monitoring wells (decimal degrees to 6 digits)
12. Location and identification of major components of the waste disposal system
13. Identification of all storage or treatment earthen structures that are part of the permit
14. The wetted perimeter of all irrigation/infiltration fields with field names (named according to the approved
permit)
15. The delineation of compliance and review boundaries
16. 100-year floodplain (if applicable)

D. Facility Repairs:
1. Per the Washington Regional Office, the facility has been in non-compliance with many needed
repairs to the facility. The following schedules will be in the new permit:
Within 180 days of permit issuance, the Permittee shall make the following repairs:
i. Repair all tears and separations in the liners of the 5.4 MG upset pond and 28.1
irrigation pond
ii. Repair irrigation valves and pressure sustaining value
iii. Repair or replace the in -line automatic backwash strainer so that wastewater can
be pumped and sprayed from the 28.1 MG irrigation pond
E. Groundwater:
1. Monitoring well MW-2 consistently exceeds the groundwater standard for nitrate. Address.
