Subject: U-5760 Mitigation Request

NCDOT Division 9 would like to request mitigation for impacts to 1,372 linear feet of stream at a 2:1 ratio, and 0.11 acres
of impacts to riparian wetland at a 2:1 ratio. The project is U-5760 in Forsyth County. The WBS No. is 46381.1.1. While
the project is in both the Yadkin and Roanoke River Basins, all of the impacts fall in the Yadkin Basin, and in the 8 digit
HUC is 03040101. Let me know if you have any questions or need additional information.
