I just got your voicemail after responding to your email earlier. Working from home is great but does have its
challenges!
I already replied to your email but after hearing the voicemail I wanted to add a little more. I must have uploaded the
unsigned version of the agent authorization form with the application. I hope the one I attached to the email earlier will
suffice. It's a little crude but it has somewhat of a signature.
I now understand better what you were saying about placing boulders to divert flow away from excavation while
installing the boulder toe. That's something we could easily do.

Jake,
On March 25, 2020, the Division received your application requesting a 401 Water Quality Certification for the subject
project. The Division has determined that your application is incomplete and cannot be processes. The application is on
hold until all of the following information is received:
1. Please provide a signed agent authorization form. The form provided with the PCN is not signed by the property
owner/applicant. [15A NCAC 02H .0502(a)(11)]
Please provide additional details regarding how Condition 11.9. of 401 General Water Quality Certification 4134
will be met for the installation of the boulder toe. [15A NCAC 02H .0506 (b)(2)]
Pursuant to Title 15A NCAC 02H .0502(c) and 15A NCAC 01C .0107, the applicant shall furnish all of the above requested
information for the proper consideration of the application. If all of the requested information is not received, the
Division will be unable to approve the application and it will be denied as incomplete.
