the volume of wastewater to be permitted in this project has been evaluated along the route to the receiving
wastewater treatment facility and that the flow from this project is not anticipated to cause any capacity
related sanitary sewer overflows or overburden any downstream pump station en route to the receiving
treatment plant under normal circumstances, given the implementation of the planned improvements
identified in the planning assessment where applicable. This analysis has been performed in accordance
with local established policies and procedures using the best available data. This certification applies to
those items listed above in Sections I and II plus all attached planning assessment addendums for which I
am the responsible party. Signature of this form certifies that the receiving collection system or treatment
works has adequate capacity to transport and treat the proposed new wastewater.

Apartment Complex located at 3761 Raleigh Road Parkway W. in Wilson, NC. Phase 1 of the

building. This private onsite gravity sewer main extension is proposed to discharge into the existing
15" gravity main sewer line, located to the southwest of the project, via the Offsite Public Sewer
Extension permitted separately for this project.

Item V. of the application was not completed as required. Please complete this section. Item V has been
completed except for the Permit Number, as the Outfall Sewer is also currently pending review.
A Developer's Operational Agreement was included with the application. Please confirm if this document is still
required if the permit is issued. The proposed units will be for lease, not for sale. A Developers Operational
Agreement is not necessary.
A review of the N.C. Secretary of State's website shows the Applicant, Evolve Companies, LLC, was
administratively dissolved January 14, 2016 (see attached). However, an annual report for calendar year 2020
was included with the application (see attached). Please explain the status of Evolve Companies, LLC with the
N.C. Secretary of State's office. A current print out from the Secretary of the State website for "Evolve at
Heritage, LLC" is attached and the application modified accordingly.
